About this file

This document contains a past performance questionnaire template and details of a solicitation for the conversion of an offshore support vessel into a U.S. Navy training support vessel.

The past performance questionnaire is a template to be completed by customers evaluating a contractor's performance on prior contracts. It includes sections for the contractor and contract details, the customer's role, a project description, performance period, ratings on a scale of exceptional to unsatisfactory for 18 performance criteria, and four narrative questions regarding cure notices, future awards, reporting in past performance databases, and comments on technical quality, schedule, and management.

The solicitation seeks proposals for the fiscal year 2022 regular overhaul of the USNS Vindicator (TSV 5) including drydocking, pier-side maintenance, repairs, alterations, equipment upgrades and modernization. The NAICS code is 336611 and inquiries should be directed to the two listed Navy contacts by email. Proposals are due based on the standard timeline and terms within the full solicitation document. The Navy department and command issuing this opportunity are also provided.
