Subject: Negotiation Summary

HW686 DC007A

This Task Assignment authorizes PSI to conduct groundwater monitoring activities in accordance with the March 29, 2022, Scope of Work, and Cost Estimate.

Subtask 1: Project Management

An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Annual Groundwater Monitoring and IDW Treatment

PSI will collect groundwater samples from monitoring wells MW001 – MW003. Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). Investigation-derived waste

(IDW) will be temporarily stored in a 55-gallon drum, which will be transported and disposed of at an approved facility. A top of casing elevation survey will be performed by SurveTech.

Surveying is a professional service as defined in chapter 287.055, in Florida Statutes so only one bid is required.

Subtask 3: Reporting

PSI will submit the Annual Groundwater Monitoring Report and ADaPT and GIS deliverables to

FDEP.

PSI confirmed on March 10, 2022, that an Access Agreement with the current real property owner of the Fashion Cleaners (ERIC_5245) site has been executed.

The original Task Assignment totaled $7,834.82 with 40 labor hours but after negotiations and corrections with contractor costs are reflected as below:

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task

Assignment. The final cost for this Task Assignment is $7,979.89 with 39 labor hours.

(PSI), an Intertek company, is pleased to provide the proposal contained herein to provide groundwater sampling and analysis services for the Fashion Cleaners at 6502 Massachusetts Avenue, New Port Richey, Florida. Included with this proposal is a discussion of project history to date, proposed scope of work, cost estimate and a proposed schedule for completion.

PROJECT BACKGROUND

The Fashion Cleaners (Former) was located in the East Richey Square Shopping Center on the southeast corner of Massachusetts Avenue and Congress Street in New Port Richey, Pasco County, Florida. The Fashion Cleaner operated in the westernmost tenant space in the shopping center which currently houses a bar, the Home Saloon. Based on the FDEP records, drycleaning began at the facility in 1978 and utilized perchloroethene (PCE) as the drycleaning solvent. The drycleaner operated from 1978 to approximately 2007 or 2008. No drycleaning equipment remains on-site.

A Site Screening Report dated January 30, 1997 was submitted which identified PCE in a soil sample at 57 ug/kg (0.057 mg/kg) at a depth of 2.5 feet below land surface (bls) outside the back door of the facility.

The Department of Health (DOH) completed potable well surveys in 2004 and 2013, which indicates there are three PWS wells within one half mile of the site. According to the 2004 and subsequent 2013 well surveys, none of the three wells were sampled and were listed as inactive in both surveys. Two potable wells were listed in the 2004 survey with chloroform detected at low levels in the well at 6540 Corbin Lane mailto:Skylar.Murphy@FloridaDEP.gov

FDEP – Fashion Cleaners and the well at 6541 Berea Lane exhibiting detections of trichloroethylene, 1,1-dichloroethylene and methylene chloride at concentrations below the Groundwater Cleanup Target Levels (GCTLs). In the 2013 survey, the 6541 Berea Lane well was not listed. The 6540 Corbin Lane well did not have detections of contaminants of concern in 2013.

PSI completed Site Assessment activities in November 2020 through February 2021, with a Site Assessment Report submitted in March 2021. The results identified low-level PCE concentrations (below SCTLs) within the 0.5-2 feet bls to 8-10 feet bls intervals from soil samples collected beneath the building and soil samples collected outside of the former drycleaning tenant space. Modified Active Gas (MAGs) testing was also conducted at five extraction locations which indicated PCE in all five air samples at concentrations ranging from 1.79 to 4.92 mg/m3, which exceeds the recommended action level of 1 mg/m3. The highest soil vapor PCE concentrations were detected in the samples collected from MAG003 (4.16 mg/m3) and MAG005 (4.92 mg/m3), both located inside the building near floor drains. The former location of the dry clean machine exhibited the lowest soil vapor PCE concentration among the test samples (MAG004, 1.79 mg/m3).

Three shallow groundwater monitor wells were installed with screens from 27 – 37 feet (bls). The water table was measured at approximately 28 feet bls. Groundwater samples from the shallow wells contained measurable concentrations of PCE, however, all results were below the GCTL in January 2021, and just above the GCTLs during the second round of groundwater sampling in August 2021. Of note, is that a groundwater well was not installed inside the building due to the ceiling height and depth to the water table which prevented manually installing a well.

Recent discussions with the FDEP resulted in a request to conduct additional groundwater sampling, as well as a more comprehensive groundwater flow evaluation using monitoring wells from nearby FDEP petroleum program sites.

PROPOSED SCOPE OF WORK

The proposal contained herein consists of conducting groundwater sampling and analysis, along with a groundwater flow evaluation. The results will be used to confirm current groundwater quality conditions.

Specifically, the proposed scope of work consists of the following subtasks:

Subtask 1 - Project Management

This scope is provided for in the template worksheets.

Subtask 2 – Professional Survey and Groundwater Sampling and Analysis

PSI will complete the following:

a. Mobilize to the site.

b. Provide oversight for a Professional Survey to obtain top of casing elevations at select wells on nearby petroleum sites. We anticipate measuring the top of casing at approximately five shallow wells per site (Chevron to the west of Fashion Cleaners and the A-1 Discount (7- Eleven) to the north), as well as the three wells at the Fashion Cleaners. We understand that site access has been previously granted to the Department and its contractors such that it is not necessary for PSI to obtain additional access documentation other than notification (already obtained).

c. Collect groundwater elevation measurements at three shallow monitor wells at the Fashion Cleaners, approximately five wells each at Chevron and the A-1 Discount.

d. Collect groundwater samples from 3 monitor wells: MW001, MW002, and MW003.

e. Submit groundwater samples to a NELAC-approved laboratory for analysis of Volatile Organic

Halogens (VOHs) via EPA Method 8260.

f. Containerize the investigation derived waste (IDW) in a 55-gallon drum for characterization, which will be transported and disposed of at an approved facility. A waste manifest will be provided upon receipt.

g. Develop ADaPT package deliverable.

Subtask 3 – Data Evaluation and Report Deliverable

Upon receipt of the Professional Survey and analytical laboratory results, PSI will develop groundwater contour maps, tabulate the laboratory data into standard FDEP formats, compare data to GCTLs and provide the report deliverable which will include current a groundwater elevation map, current groundwater quality data, and recommendations.

COST ESTIMATE AND SCHEDULE

The estimated cost to perform the above-described services is shown on the attached Cost Estimate and Level of Effort sheets which includes the Groundwater Sampling and Report deliverables. All PSI services will be conducted in accordance with the terms and conditions contained within DEP Contract No. HW 686, Waste Cleanup Program Services, dated December 21, 2021. We anticipate 3 months to perform the requested services and to submit the report deliverable, including waste manifest. If the Task Assignment is authorized soon, we will be able to mobilize with the surveyor and perform groundwater sampling in early May, obtain the analytical results and arrange for IDW transport, and provide letter report deliverable in late June. The manifest will be provided to us upon pick-up which could take approximately 2 months. Therefore, the TA should include a final deliverable date of July 15, 2022.

PSI appreciates the opportunity to be of service to the FDEP. If you have any questions, please feel free to contact us at (813) 886-1075.

Respectfully submitted, PROFESSIONAL SERVICE INDUSTRIES, INC.

From: Nana Westmark Sent: Thursday, January 13, 2022 8:14 AM To: Natacha Jackson Cc: Greg Lassi Subject: Annual IDW prices for FDEP Drycleaning Contract for PSI Team

Good Morning Natacha – Hope you are doing well these days.

The PSI Team (PSI and Chastain Skillman) have been awarded one of the FDEP “Drycleaning Contracts” and we are anxious to get started.

We are inviting your firm to provide costs for pick-up, transport, and disposal to an approved facility of Investigation- Derived Waste (IDW) on a per 55-gallon drum basis from drycleaning sites and state funded sites, and site investigation sites that are in the programs.

If you are interested in providing a quote, we would appreciate it. I am attaching a form in which to submit back to us, along with the General Conditions.

If you could provide these prices to us by January 21, 2022, we would appreciate it.

Thank you, Nana
