Document Summary
This document is a Task Assignment Change Order (TACO #1) issued by the State of Florida Department of Environmental Protection (FDEP) to CDM Smith Inc., a contractor under Contract Number HW681. The change order modifies a Task Assignment (DC020B) related to environmental monitoring services at Touch of Class Cleaners located at 8420 Lockwood Ridge Road in Sarasota, Florida. The Department's Project Manager is Kyle McMurray, and the Contractor's Project Manager is Shayne Wood. This TACO tasks CDM with reinstalling two MAGS (Mobile Air Sampling Grab System) wells and extends the project timeline.
The amended Task Assignment value increases from $18,086.60 to $29,869.85, representing a cost increase of $11,783.25 attributed to increased vehicle, subcontractor, lab, equipment, and CDM labor costs for the MAGS reinstall and testing subtask. The deliverables include field notes and air sample data due August 15, 2023; an Annual Groundwater Monitoring Report due December 15, 2023; and MAGS reinstall and sampling data due February 14, 2024. The project completion date was extended from January 15, 2024, to April 17, 2024, providing an additional 93 days. Invoice payments are structured as $2,928.01 for field notes and sampling data and $26,941.84 for the monitoring report and reinstall work. The FDEP Project Manager approved the scope and cost of this change order on November 2, 2023.

EXTERNAL MESSAGE
This email originated outside of DEP. Please use caution when opening attachments, clicking links, or responding to this email.
Good morning, Kyle. I am pleased to submit the TACO for TOC Sarasota.
In August when we were trying to schedule work at this site, we initially ran into issues with gaining access to the restaurant by the site tenant. FDEP assisted and CDM attempted to perform MAGS testing on 8/17, not all wells were present, this was discussed with FDEP, and the team determined new wells need to be installed. This TACO is to add in Subtask 5 to reinstall two MAGS wells inside the restaurant and perform MAGS testing.
Initial payment of Subtask 2 (invoiced 9/18/23) included 1 air sample and 1 summa canister, and only 1 additional of each are requested in this TACO.
The TA currently includes Subtask 1,3,4 Annual Groundwater Monitoring Report due 12/15/2023 - groundwater last sampled 10/26/22, CDM sampling in late October, no change to this deliverable.
CDM is currently scheduling the work Subtask 5 MAGS reinstall & sampling for November and December 2023.
Subtask 5 MAGS Install & Sample, the requested due date is 2/14/2024.
The end of the TA is 1/15/2024, please extend to 4/17/2024 to accommodate the schedule and allow review and final billing.
Very Respectfully, Kathy R. Leggoe, PG, PMP Geologist Project Manager CDM Smith 4651 Salisbury Road, Suite 420 Jacksonville, Florida 32256 (office) 904.527.6727, (cell) 904.651.0740 cdmsmith.com
