Elite Cleaners (ERIC_4708)

Dear Mr. Aoki:
CDM Smith Inc. (CDM Smith) is pleased to submit this Task Assignment Request for the preparation of an Interim Source Removal Plan (ISRP) for the Elite Cleaners (ERIC_4708) (the Site) located at 11301 N
56th St, Temple Terrace, Hillsborough County, Florida. The scope of work, proposed deliverable, and deliverable schedule are summarized below, and the cost estimate is attached.
This Task Assignment (TA), when executed, shall be incorporated in, and become part of Contract No.
HW681, between the Florida Department of Environmental Protection (Department), and CDM Smith
(Contractor), dated January 5, 2022, hereafter referred to as the Contract.
On August 13, 2025, Contractor confirmed a Site Access Agreement (SAA) was executed with Saad
Investment, Inc, on May 20, 2021, as the real property owner of the Elite Cleaners (ERIC_4708) located at the above address. The Hillsborough County Property Appraiser database shows that the property owner since March 28, 2024, is Saad Investments, Inc. Therefore, Contractor assumes the SAA is valid.
Project Understanding:
A Site Assessment Report (SAR) was submitted by the Contractor in April 2024. Laboratory analytical results from groundwater and soil samples showed the Site meets the regulatory criteria for No Further
Action in accordance with Chapter 62-780.680 F.A.C.; however, MAGS™ testing indicated PCE soil vapor contamination in the vadose zone above the current FDEP guidance criteria. CDM Smith and the FDEP discussed the potential need for additional soil samples to pinpoint the source of soil vapor contamination prior to remedial action. During the discussion, both FDEP technical staff and CDM Smith determined that due diligence was completed under the SAR, and that further soil sampling was not needed. CDM Smith recommended preparing an Interim Source Removal Plan (ISRP) to include the design and installation details of an active soil vapor extraction (SVE) system. Elevated soil concentrations near the rear of the drycleaning facility indicated the presence of impacts in the vadose zone, between land surface and approximately 30 ft bls. Site conditions are not optimal for excavation due to the vast number of underground utilities serving the strip mall and retail suites. SVE appears to

Contractor performed PFAS screening at the Site in October 2023. No concentrations were detected above the Provisional Groundwater Cleanup Target Level (PGCTL) of 70 nanograms per liter (ng/L).
Scope of Work
The proposed scope of work includes the following elements:
Subtask 1: Project Management
Project management activities performed under this task consist of those general administrative functions required to maintain the project on schedule, within budget, and that the quality of the work products defined within this scope is consistent with Contractor’s quality standards. Contractor’s project manager will be Kathy Leggoe, PG, PMP. We will prepare and submit invoices upon approval of each deliverable or milestone completion.
Project Progress Meetings and Status Reports
Contractor expects to have bi-weekly meetings or telephone conference with the Department to review the status of the project and identify and resolve any outstanding issues. Contractor will prepare meeting agendas and progress reports as part of this task throughout the duration of this project.
Project Resource Management
Contractor will manage, allocate, and coordinate necessary resources for completion of tasks ISRP design.
Project Quality Technical Review
Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.
Subtask 2: Interim Source Removal Plan Preparation
Per the request of the Department, Contractor submitted a SVE Presumptive Remedy Approval Form to the FDEP for completion and approval in April 2024. The Department signed the Presumptive Remedy
Approval Form on February 12, 2026. At the Annual Site Review Meeting in May 2025, FDEP stated this proposal should be submitted once the PRF was approved with the consensus that Contractor would submit a proposal for the completion of an ISRP, which would include an engineering assessment as to whether sufficient MAGSTM data exists to complete the design of a SVE system, and whether any additional vapor extraction wells would be needed.
Contractor anticipates that the ISRP will include an SVE system consisting of 12 vapor extraction wells, including existing MAGSTM wells VEW-01 to VEW-07, and VEW-03I to VEW-07I. The system will also include an SVE blower, an air injection blower, granulated activated carbon treatment, and all ancillary equipment.
Deliverable: Interim Source Removal Plan

Assumptions
• This scope and schedule may change depending on surveys and the drilling conditions. Drillers and other subcontractors’ availability may be limited and impact the schedule. In addition, this scope does not consider any weather delays. CDM Smith will communicate with the FDEP in a timely manner if this occurs to prepare the necessary paperwork for change orders.
Schedule:
Contractor will schedule the field events after the receipt of formal notice to proceed (NTP). The deliverables will be submitted as detailed below:
Subtask 2: Interim Source Removal Plan Preparation
Deliverable: Interim Source Removal Plan
Due Date: 120 days following NTP
CDM Smith appreciates this opportunity to work with FDEP on this contract. If you have any questions, please do not hesitate to contact us.
