Contractor will manage, allocate, and coordinate necessary resources for completion of tasks related to verification soil sampling.

Project Quality Technical Review

Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.

Subtask 2: Verification Soil Sampling

This subtask includes costs for the Contractor to collect soil verification samples at the following seven historical soil boring locations and depths as approved by FDEP and shown in the figure included in

Attachment A.

• M-4 at 0.5, 1, 2, 4, and 6 ft depths

• SB-6 at 0.5, 1, 4, and 6 ft depths

• SB-11 at 0.5 and 6 ft depths

• SB-20 at 6 ft depth

• SB-21 at 4 and 6 ft depths

• SB-22 at 1, 4, and 6 ft depths

• SB-28 at 2, 4, and 6 ft depths

A total of 20 soil samples will be collected and submitted to Eurofins laboratory for analysis by United

States Environmental Protection Agency (EPA) Method 8260 for Volatile Organic Halocarbons (VOHs).

Split soil samples will be collected and held for Synthetic Precipitate Leaching Procedure (SPLP) testing.

SPLP testing will be performed on samples that, in the initial data received from the laboratory, display results below the direct-contact Soil Cleanup Target Levels (SCTLs) and above the default leachability

SCTLs as established in Chapter 62-777, FAC. The removed soil will be used to backfill the location and the ground surface of the boreholes will be restored with either asphalt or concrete to match the original conditions.

4651 Salisbury Road, Jacksonville, Florida 32256| tel: 904.527.6716

The morning of the verification soil sampling, underground utilities will be located using ground penetrating radar (GPR) in the area of the proposed sampling locations. Request for Quotations (RFQs) were sent to Blood Hound, GeoView, and Site Lines LLC to complete the underground utility locating.

The lowest cost bid was from Site Lines LLC. The quotes are included in Attachment B.

RFQs were sent to three pre-qualified drilling companies (Preferred Drilling Solutions, Environmental

Drilling Service, and Groundwater Protection) for drilling services to support the verification soil sampling effort. The lowest cost bid was from Preferred Drilling Solutions. The quotes are included in

Attachment B.

All soil will be containerized and transferred to a FDOT-approved 55-gallon drum. Two drums of nonhazardous soil are assumed. Subcontracted services are required for Investigation Derived Waste

(IDW) disposal. Rates for IDW removal and disposal were previously requested for HW681 from Erwin

Remediation, Enviroserve, and Hepaco. Erwin Remediation was the lowest cost and has been selected as the IDW disposal subcontractor for this project as shown in Attachment B.

SITE LINES LLC PROPOSAL

SITE LOCATION

White Sands Dry Cleaners Site – 3333 N. Tamiami Trail, Sarasota, FL

PROPOSED SCOPE OF WORK

Site Lines (“Company”) will use Ground Penetrating Radar (GPR) and conventional electromagnetic (EM) pipe and cable locators to determine the location of underground utilities at the site identified by CDM Smith (“Client”).

RESULTS OF THE INVESTIGATION

The estimated depth and location of utilities identified during the investigation will be marked on the ground surface using spray paint or other method to be agreed upon by the parties. The American Public Works Association Uniform Color Codes for the marking of underground utilities will be used.

PROJECT FEE

The fee to complete the project as described within this proposal is:

$800.00

Client shall pay Company in full within thirty (30) days of the invoice date, unless the parties have agreed to another payment arrangement in writing. If during the investigation, Company determines that the goals of the project cannot be achieved with the resources available to Company, Company will notify Client as soon as reasonably possible. Under such circumstances, Company will charge Client only for time expended and materials used to date.

CLIENT REQUIREMENTS

Client is responsible for all site access, maintenance of traffic (MOT), site clearing and/or any other logistical considerations necessary to conduct the utility locate.

Client will also provide all pertinent maps of the project site, including those that show the boundaries of the utility locate, pertinent landmarks, and known utilities (if available). Company is not a substitute for Chapter 556, Florida Statutes.

Accordingly, Client is responsible for calling Sunshine State One Call of Florida

(811) before undertaking any invasive activities, whether through Company, any other entity, or on its own.

DESCRIPTION OF COMPANY INVESTIGATION AND LIMITATIONS

A GSSI SIR-4000 GPR system will be used with a 900, 400, or 270 megahertz antenna configuration depending on site conditions and desired targets. The GPR will assist in locating nonmetallic and metallic utilities. Physical surface conditions, subsurface conditions and technical limitations effect the ability of GPR to resolve targeted utilities and/or subsurface structures. Depth and/or diameter of the utility, soil type, moisture/ionic content of the soil, and electrical properties of the soil compared to that of the utility can prevent the GPR from identifying utilities at the site. The diameter of a utility cannot be determined by GPR data. Estimated depths of utilities are directly affected by soil conditions and the material of the utility. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities.

An RD 8000 will be used for the EM survey. Electromagnetic induction which transmits a signal directly into the desired target and passive power detection which locates naturally occurring magnetic fields in energized lines will be identified by detecting and following the signals with a receiver. The EM survey will assist in finding only metallic utilities. The magnetic field and/or induced signal should emanate from the target in a circular geometry. When this occurs, the receiver will receive its peak signal directly above the target. However, when metallic utilities are running near each other, or there is environmental interference from fences, cars or overhead power lines, the signal can become distorted and the peak no longer can be directly over the utility. Care must be taken when doing any invasive work around the utilities. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities. Company’s personnel are highly experienced and certified operators of GPR and EM pipe and cable locators.

Due to the limitations described above, when Client is working around indicated utilities, a minimum of two (2)-feet clearance from either side of the marked utility will be maintained. For absolute confirmation, Client is responsible for exposing any indicated underground utility.

Company will conduct utility investigations in accordance with the level of care and generally accepted professional practices as ordinarily exercised by members of the same profession practicing in the same locality under similar conditions.

REPRESENTATIONS AND WARRANTIES

Neither Company nor its employees or agents are licensed surveyors under Florida law, nor do they represent themselves as such. Accordingly, the work Company is to perform is not a substitute for work typically completed by a licensed surveyor.

Company specifically and expressly disclaims any liability for any type of property damage or personal injury, including but not limited to death, arising from client’s failure to determine that the services of a licensed surveyor are necessary or to retain a licensed surveyor to perform necessary services.

Limitations in technology, complexity of the physical characteristics of the subsurface conditions, and circumstances beyond Company’s control may limit the performance of Company’s utility investigation. For these reasons, Company expresses no guarantee that all utilities and underground structures will be identified and located during this project.

Company is responsible only for locating underground utilities, the presence of which is both known and indicated on site maps to be provided by Client to Company before Company performs the utility locate. Company will use its best efforts to locate both known and unknown utilities within the project boundaries. However, Company will only be responsible for locating utilities, the presence of which is known before Company undertakes its investigation.

Company expressly disclaims any and all warranties regarding specific or desired results.

TERMS AND CONDITIONS

Additional Insured: If requested, Company will name Client as an additional insured with respect to the services to be performed under this agreement.

Indemnity: Company agrees to indemnify, protect and hold harmless Client from and against all liabilities, claims or demands of every kind of injury, including death, or damages to any person or property related in any way to Company’s performance of this Agreement, except to the extent such liabilities, claims or demands are caused by the negligence or willful misconduct of Client.

Client agrees to indemnify, protect and hold harmless Company from and against all liabilities, claims or demands of every kind of injury, including death, or damages to any person or property related in any way to Client’s performance of this Agreement, except to the extent such liabilities, claims or demands are caused by the negligence or willful misconduct of Company.

Proposal

This document shall serve as a proposal for work to be performed by GeoView Associates, Inc. (GeoView) for CDM Smith.

Scope of Work

The project site is referred to as the White Sands Cleaners Site and is located at 3333 North Tamiami Trail in Sarasota, Florida. The purpose of the geophysical survey will be to determine the location of underground utilities in the area of twelve planned borings at the subject site. Five of the borings are inside of the building and seven are outside of the building in an asphalt parking lot. The location of the study area is provided on Figure 1 (page 6).

The geophysical survey will be conducted using ground penetrating radar (GPR) and conventional electromagnetic (EM) pipe and cable locators. The GPR survey will be conducted using either a Mala and/or a GSSI GPR system. The appropriate GPR antenna configuration and time range and processing parameters will be determined in the field by the project manager.

The EM survey will be performed using a Subsite 95R and/or a Radiodetection RD (8000, 8100 or 8200) system. As appropriate, the EM survey will be conducted using both passive and active modes. The passive mode is used to find cables and conduits carrying live power (50-60 Hz) and communication (29-33Hz) signals. For the active mode survey, a current is transmitted directly or broadcast into using electromagnetic induction a metal pipe or conduit and the path of the pipe/conduit is identified by following the signal with a portable receiver.

The active mode is used to trace out cast iron pipes, metal water and sewer lines, utilities that have trace wires, and inactive power and communication lines.

Based on information provided by CDM Smith, it does not appear that the work at any of the sites will require MOT (traffic calming efforts). This proposal does not include the costs for the design or implementation of an MOT program.

If requested, GeoView can provide vacuum excavation services to determine the exact horizontal location, top elevation, outside diameters and material type of specific underground utilities of any utilities identified at the project site. The vacuum excavation will be performed using the Vermeer CV573GT vacuum excavation system. It is noted that the vacuum excavation equipment used by GeoView can only identify utilities above the water table or to a maximum depth of 7 to 8 ft in unconsolidated soils. It will not be possible to confirm (using vacuum excavation) the presence, location or depth of utilities that are located below these depths or in consolidated soil/rock. The cost for vacuum excavation will be $2,800 per day or range in general from $450 to $550 per hole, with a $1,800 minimum. A full proposal for vacuum excavation services can be provided upon request.

Presentation of Results

The location and estimated depths of underground utilities or any other identified structures shall be indicated on the ground surface using spray paint or other methods as specified by CDM Smith. No report shall be prepared.

Compensation

Cost to complete the survey will be $900. If a scaled AutoCAD site map is required to document the study results, an additional fee of $200 will be added.

This price is inclusive of all charges associated with the project. Unless otherwise agreed upon, GeoView shall be compensated for all services within 30 days of the invoice date. GeoView shall be paid in full regardless of whether the results of the geophysical survey are what CDM Smith anticipated. If it is determined during the survey that the geophysical survey will not achieve the objectives of the project, CDM Smith will immediately be notified. If a decision is made to discontinue the survey, only charges for time and materials costs to that point will be submitted.

These prices are based upon GeoView being able to access the site during normal business hours, Monday through Friday. It is also based upon the assumption that no site-specific training or drug testing will be required. If there are any such requirements, additional costs will be incurred.

Requirements of Client

CDM Smith will provide a scaled map and digital file, if available, of the project site showing the boundaries of the project site, areas of specific concern and pertinent landmarks. CDM Smith will also supply all available information regarding the presence and history of underground utilities at the project site (see limitations for further discussion). Unless otherwise requested, CDM Smith will be responsible for the coordination of site access, traffic control, clearing of onsite obstructions or any other logistical consideration necessary to conduct the survey.

If any test drilling, monitor well installation or other excavation activities are planned it will be the responsibility of CDM Smith to submit a notification (dig ticket) to the appropriate 811 One-Call agency. It is recommended that such notification be filed sufficiently in advance so that GeoView will have the opportunity to incorporate the 811 findings into their results.

Limitations

The objective of the geophysical survey is to determine the presence and configuration of underground utilities. However, the geophysical response of these features may be highly complex depending upon on the physical characteristics of the underground utilities, the electrical properties of the surrounding soils, the depth of burial of the utilities, and the proximity of the utilities to each other.

GeoView shall conduct the geophysical survey using the most “up-to-date” geophysical equipment in a manner consistent with the level of care and skill ordinarily exercised by members of the geophysical profession practicing in the same locality under similar conditions. GeoView’s utility locating personnel are all certified GPR and conventional pipe and cable locator operators with extensive in-house training.

It is acknowledged that GeoView is not an 811 Sunshine One-Call or equivalent utility mark-out service. GeoView does not have access to any utility-related records for either public or private properties. It is the responsibility of CDM Smith to obtain such records and provide to GeoView prior to the site investigation. GeoView will use these records during the site investigation and report to CDM Smith any utilities that are shown on the site records but cannot be confirmed by GeoView. GeoView will not mark on the ground surface any utilities which it cannot confirm.

GeoView shall only be liable for the successful locating of underground utilities or any other underground structures whose presence is both known and indicated on site maps to be provided prior to the performance of the geophysical survey. Every attempt will be made to locate both known and unknown utilities within the survey area. However, GeoView will only be liable for utilities whose presence is known prior to the survey. CDM Smith accepts that due to the inherent limitations of non-intrusive geophysical technologies GeoView cannot be held liable for damages to utilities that cannot identified using non-intrusive technologies.

CDM Smith accepts that a non-intrusive utility designation study does not replace the need to employ widely accepted industry safety standards for subsurface work. Concerning the installation of well or test borings this standard of care includes post-holing or hand auguring at the planned test locations to the minimum depth at which underground utilities are anticipated at the project site.
