Annual Groundwater Monitoring
CDM Smith Inc. (CDM) will conduct groundwater monitoring. Specific subtasks are detailed below:
Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.
Subtask 2: Annual Groundwater Sampling Contractor will mobilize to the Site to collect groundwater samples from monitoring wells MW001, MW004, MW005, MW007, MW010, MW011, MW012, MW013, MW018, MW019, MW021, MW022, MW023, MW024, and MW026. All monitoring wells will be sampled in accordance with FDEP SOPs. A YSI-556 multi-parameter probe and a turbidimeter will be used to collect field data. Field parameter data will be collected (dissolved oxygen, pH, conductivity, oxidation reduction potential, temperature, turbidity). Depth-to-groundwater measurements will be collected from all 15 monitoring wells at the Site prior to the collection of groundwater samples. In addition, the site owner identified a pipe that needed to be capped, so FDEP asked CDM Smith to cap the piping that is believed to be associated with the old SVE system. Costs to repair this are included in this subtask.
Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis.
Subtask 3: Annual Groundwater Monitoring Report Upon completion of groundwater sampling field work, Contractor will prepare a Annual Groundwater Monitoring Report. This report will summarize all monitoring work completed, field and laboratory data, investigation-derived waste (IDW) management, conclusions, and recommendations.
FDEP Technical Staff has approved of the proposal dating May 11th, 2023.
CDM and the FDEP confirmed on May 23rd, 2023, that an Access Agreement with the current real property owner of the site has been executed.
The final cost total for Task Assignment DC017B is $15,763.88.

Subject: Task Assignment Request—Annual Groundwater Monitoring
Former Dry Cleaners
2626 Hillsborough Avenue Tampa, Hillsborough County, Florida
ERIC_4794
Dear Mr. McMurray:
CDM Smith Inc. is pleased to submit this Task Assignment Request for Annual Groundwater
Monitoring of the Former Dry Cleaners (the Site) located at 2626 Hillsborough Avenue Tampa, Hillsborough County, Florida, ERIC_4794. The scope of work, proposed deliverable, and deliverable schedule are summarized below and the cost estimate is also attached.
This Task Assignment, when executed, shall be incorporated in, and become part of Contract No.
HW681, between the Florida Department of Environmental Protection (FDEP), and CDM Smith, Inc. (Contractor), dated January 5, 2022, hereafter referred to as the Contract.
Scope of Work
The proposed scope of work includes the following elements:
Subtask 1: Project Management
Project management activities performed under this task consist of those generally administrative functions required to maintain the project on schedule, within budget, and that the quality of the work products defined within this scope is consistent with CDM Smith’s quality standards. We will prepare and submit invoices upon approval of each deliverable or milestone completion.
Project Kickoff and Progress Meetings
Contractor expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM
Smith will prepare meeting agendas and notes as part of this task.
Project Resource Management
Contractor will manage, allocate, and coordinate necessary resources for completion of tasks related to groundwater sampling.
ERIC_4794_Former Dry Cleaner_GWMR Proposal 04_23
Project Quality Technical Review
Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.
Project Status Reports
Contractor’s Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project.
Subtask 2: Annual Groundwater Sampling
Based on the recommendation from the CDM Smith Annual Groundwater Monitoring Report dated December 9, 2022, Contractor will mobilize to the Site to collect groundwater samples from monitoring wells MW001, MW004, MW005, MW007, MW010, MW011, MW012, MW013, MW018, MW019, MW021, MW022, MW023, MW024, and MW026 in October 2023. All monitoring wells will be sampled in accordance with FDEP SOPs (FS 2200) for low-flow sampling using a peristaltic pump. This groundwater sampling event has been recommended six months after the sampling event in October 2022 as additional downgradient monitoring wells may be proposed to delineate the plume. A YSI-556 multi-parameter probe and a turbidimeter will be used to collect field data. Field parameter data will be collected (dissolved oxygen, pH, conductivity, oxidation reduction potential, temperature, turbidity) using properly calibrated probes, and recorded. Depth-to-groundwater measurements will be collected from all
15 monitoring wells at the Site prior to the collection of groundwater samples.
The groundwater samples will be collected in laboratory supplied containers, placed on ice, and delivered to a NELAP certified laboratory for analysis by United States Environmental
Protection Agency (EPA) Method 8260C for volatile organic halocarbons (VOHs).
All purge water will be containerized and transferred to a Florida Department of
Transportation-approved 55-gallon drums. The Site Owner identified an unknown drum that has been at the site prior to CDM Smith working at the site. FDEP asked CDM Smith to remove that drum. The disposal services will be provided by Erwin Environmental and two drums of nonhazardous water for both events are assumed.
In addition, the site owner identified a pipe that needed to be capped, so FDEP asked CDM Smith to cap the piping that is believed to be associated with the old SVE system. Costs to repair this are included in this subtask.
Subtask 3: Annual Groundwater Monitoring Report
Upon completion of groundwater sampling field work, Contractor will prepare a Annual
Groundwater Monitoring Report. This report will summarize all monitoring work completed, field and laboratory data, investigation-derived waste (IDW) management, conclusions, and recommendations. Additional elements of the report will include:
Tables summarizing field and analytical data.
ERIC_4794_Former Dry Cleaner_GWMR Proposal 04_23
A statistical analysis of the attenuation rate of MW07D
Groundwater sampling logs
Laboratory Analytical Reports
Deliverable: Groundwater Monitoring Report
Schedule:
Contractor will schedule the field reconnaissance after the receipt of formal notice to proceed
(NTP). The deliverables will be submitted as detailed below:
Subtask 3: Annual Groundwater Monitoring Report

T&D - Non-Haz Soil $ 125.00 Per Drum
All drum lids should be properly secured, and drums should be placed on level ground (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.
The prices listed are all inclusive without any additional fees or charges (fuel surcharge, low volume stop fee etc.) and are valid for all regions in the State of Florida except for the Florida Keys.
Pricing for sites located in the Florida Keys will be bid on a case-by-case basis dependent on the volume of drums and the location.
Respectfully, Robert Donado Erwin Remediation, Inc.

Subject: Drum Quote Request for CDM Smith
Greetings Steve, nice to be writing you from a new place of business. I have joined the CDM Smith team and working with the dry cleaning program. We are trying to get a current quote for 2023. Kara Fincham sent you the message below back in January. I thought I would try to send the same message to your new email. See below and respond with a quote if you would like us to use you.
We are renewing the rates for IDW for 2023—I will need a general cost proposal for the pickup and disposal of all investigation derived waste associated with our FDEP projects for one year. The rates should include non-hazardous waste, liquid and solid, generally in 55-gallon drums. The majority of the IDW will most likely be contained in 55-gallon drums and consist of soil and groundwater from environmental site investigations, but we are going to start doing more
PFAS work this year, so if there is a separate cost associated with PFAS IDW (soil/Groundwater), please include it in your proposal.
The sites are located throughout the state, including Pensacola, Jacksonville, North Florida, South Florida, Tampa, and
Orlando. The lowest bidder will win the work for the remainder of 2023.
If you are interested, please submit a proposal by Thursday, March 9, 2023.
If you are not interested in bidding, please reply to let us know.
Very Respectfully, Kathy R. Leggoe, PG, PMP
