About this file

This is a Task Assignment (TA) document under Contract Number HW681 between the Florida Department of Environmental Protection (FDEP) and CDM Smith Inc. The task assignment, designated DC005E, is for ISRP (Interim Source Removal Plan) pre-design activities at the Former Bay's Cleaners site located at 214 North Magnolia Avenue, Ocala, Marion County, Florida. The project number is ERIC_4918, with Benjamin Aoki serving as the FDEP project manager and Kathy Leggoe as the contractor's project manager. The task assignment is effective through September 4, 2026.

The scope of work consists of two subtasks: Subtask 1 involves project management activities calculated at 13% of direct labor, including bi-weekly meetings, progress reporting, resource management, and quality technical review; Subtask 2 encompasses ISRP pre-design activities requiring the contractor to communicate with the property owner regarding development plans and conceptual ISRP ideas, review existing site data, and prepare a letter report with pre-design recommendations. The total task assignment value is $3,310.28 on a fixed unit rate basis, representing 16 labor hours. The deliverable—a letter report describing the site redevelopment plan with pre-design ISRP recommendations—is due 90 days from notice to proceed, with FDEP receipt and review serving as the invoice trigger. The document reflects a cost reduction of $2,589.89 from the original proposal through communications between CDM Smith and FDEP.

Scope of Work/Level of Effort

Former Bay’s Cleaners (ERIC_4918)

214 North Magnolia Avenue, Ocala, Marion County, Florida

Dear Mr. Aoki:

CDM Smith Inc. is pleased to submit this Task Assignment Request for Interim Source Removal Plan

(ISRP) Pre-Design Activities at the Former Bay’s Cleaners Site (the Site) located at 214 North Magnolia

Avenue, Ocala, Florida. The scope of work, proposed deliverable, and deliverable schedule are summarized below, and the cost estimate is attached.

This Task Assignment, when executed, shall be incorporated in, and become part of Contract No.

HW681, between the Florida Department of Environmental Protection (FDEP), and CDM Smith, Inc.

(Contractor), dated January 5, 2022, hereafter referred to as the Contract.

On January 12, 2026, CDM Smith confirmed a Site Access Agreement (SAA) was executed with JALMJ 21, LLC (Aaron Campo, signee) on August 12, 2021, as the real property owner of the Site listed above. The

Marion County property appraiser database indicates the property owner since October 2005, is

Thomas D. and Jeanne D. Sawaya, et. al. and that the property has not changed ownership since. As a result, CDM Smith believes the current SAA remains valid and in effect.

Project Understanding

CDM Smith performed site assessment activities at the Site from September 2022 to August 2023. Soil impacts in the shallow zone (0-24 feet below land surface [bls]) are horizontally delineated on Site. Soil impacts are not horizontally delineated and extend offsite at depths 24 to 45 feet bls and additional assessment is needed. Groundwater sampling at the site was last performed in January 2023. Depth to water at the site is deep (35-45 feet bls) and groundwater impacts exceed Natural Attenuation Default

Concentrations, extend offsite in each cardinal direction, and extend to multiple depth levels.

Groundwater flow during site assessment sampling in September 2022 was to the east-northeast in the shallow zone and southwest in the intermediate zone. PFAS screening was performed at the site in May

2023. Groundwater samples were collected from shallow monitoring wells MW-3, MW-4, and MW-5, and intermediate monitoring well MW-6. All wells showed a presence of PFAS with results above 70 ng/L in monitoring wells MW-3 and MW-5, and groundwater flow was generally east-southeast. A review of the 2023 Potable well survey shows no private or public water supply wells within the 0.25-mile and 0.5-mile the respective radius of the Site.

Mr. Benjamine Aoki

4651 Salisbury Road, Jacksonville, Florida 32256| tel: 904.527.6716

Completion of the soil and groundwater assessment has been hindered at off-site properties where owners have denied access. CDM Smith submitted a Supplemental Site Assessment Report (SSAR) to the

FDEP in August 2024. The report presented the August 2023 MAGS testing and recommended additional assessment including soil borings, soil sampling, monitor well installation, and groundwater sampling. As of the date of this proposal FDEP has not provided technical review of the report.

Sitewide groundwaters sampling was performed in March 2025. During the sampling event Contractor spoke with the Property Owner who stated he has plans to redevelop the Site once cleanup has been completed. This proposal includes costs to perform preliminary remedial design and coordinate with the

Property Owner to establish a plan to achieve site cleanup and site redevelopment.

Scope of Work

The proposed scope of work includes the following elements:

Subtask 1: Project Management

Project management activities performed under this task consist of those general administrative functions required to maintain the project on schedule, within budget, and that the quality of the work products defined within this scope is consistent with Contractor’s quality standards. Contractor’s project manager will be Kathy Leggoe, PMP. We will prepare and submit invoices upon approval of each deliverable or milestone completion.

Project Progress Meetings and Status Reports

Contractor expects to have bi-weekly meetings or telephone conferences with the FDEP to review the status of the project and identify and resolve any outstanding issues. Contractor will prepare meeting agendas and progress reports as part of this task throughout the duration of this project.

Project Resource Management

Contractor will manage, allocate, and coordinate necessary resources for completion of tasks related to groundwater sampling.

Project Quality Technical Review

Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.

Subtask 2: ISRP Pre-Design Activities

Contractor will contact the Site property owner to discuss site redevelopment plans, to request those plans, and will touch on conceptual ISRP ideas. Contractor will review existing data and determine excavation depth and area, and vapor barrier placement. Contractor will review possible SVE well placement and depths required, potential location of treatment system. Contractor will provide tables and figures needed to illustrate and will prepare brief narrative describing the findings and proposed path forward.

Deliverable: Contractor will prepare letter report describing the findings and proposed path forward for

Site redevelopment inclusive of site remediation.

Mr. Benjamine Aoki

4651 Salisbury Road, Jacksonville, Florida 32256| tel: 904.527.6716

Assumptions:

• The current SAA remains valid and in effect.

Schedule:

Contractor will schedule the meeting with the Site property owner after the receipt of formal notice to proceed (NTP). The deliverable will be submitted as detailed below:

Subtask 2: Site Redevelopment Plan with Pre-Design ISRP recommendations

Deliverable: Letter Report

Due Date: 90 days from NTP

CDM Smith appreciates this opportunity to work with FDEP on this contract. If you have any questions, please do not hesitate to contact us.

Sincerely, Approved By:
