Subject: Modified Active Gas Sampling™ (MAGS™) Retesting Scope of Work/Level of Effort Carriage Trade Dry Cleaners (ERIC_5232) 38225 12th Avenue
Zephyrhills, Pasco County, Florida
Dear Mr. Aoki:
CDM Smith Inc. (CDM Smith) is pleased to submit this Task Assignment Request for MAGS™ Retesting at Carriage Trade Dry Cleaners (ERIC_5232) (the Site) located at 38225 12th Avenue, Zephyrhills, Pasco County, Florida. The scope of work, proposed deliverable, and deliverable schedule are summarized below, and the cost estimate is also attached.
In the 2025 Annual meeting, the Florida Department of Environmental Protection (FDEP) requested that CDM Smith perform a MAGS™ retest to confirm the July 2024 (and the July 2015) testing results. In the (November) 2024 Annual Groundwater Monitoring, Soil Sampling, and MAGS™ Retesting Report, CDM Smith confirmed soil and groundwater meet cleanup criteria, and only soil vapor remained at the Site.
This Task Assignment, when executed, shall be incorporated in, and become part of Contract No. HW681 between the FDEP, and CDM Smith (Contractor), dated January 5, 2022, hereafter referred to as the Contract.
On July 21, 2025, CDM Smith confirmed that the previous Site Access Agreement (SAA) executed with Kairali Drycleaners, Inc. on February 4, 2016, appears to be valid. On September 10, 2015, the property was sold by Carriage Trade, Inc. to Kairali Drycleaners, Inc., who remains the real property owner of the Site located at the above address.
Scope of Work The proposed scope of work includes the following elements:
Subtask 1: Project Management Project management activities performed under this task consist of those general administrative functions required to maintain the project on schedule, within budget, and that the quality of the work products defined within this scope is consistent with Contractor’s quality standards. Contractor’s project

Project Kickoff and Progress Meetings Contractor expects a kickoff meeting and progress meetings with the FDEP to review the project status and address FDEP comments regarding the Task Assignment.
Project Resource Management Contractor will manage, allocate, and coordinate necessary resources for completion of tasks related to groundwater sampling.
Project Quality Technical Review Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.
Project Status Updates Contractor’s Project Manager will prepare and submit monthly status updates to the FDEP. Invoices will be processed following milestone deliverable approvals throughout the duration of this project.
Subtask 2: MAGS™ Re-Testing There are six existing vapor extraction wells (MAGS-2R, MAGS-3R to MAGS-4R) on Site. A figure is included in Attachment A. CDM Smith will perform a supplemental MAGS™ test to duplicate the test formerly performed in July 2024, and documented in the 2024 Annual Groundwater Monitoring, Soil Sampling, and MAGS™ Retesting Report. The vapor extraction well locations are shown on the included Figure (Figure 6: MAGS Analytical Summary). Using the previously determined flow rate and vacuum, a vapor sample will be collected at each MAGS™ well. The vapor samples will be collected using a 1-liter Summa canister. A purge time will be calculated based on the flow rate and estimated radius of influence, to ensure the collection of steady-state data. Vacuum and flow rate readings will be recorded, along with vacuum readings at select monitor wells and designated vacuum monitoring points, at 15-minute intervals for a total duration of one hour. Influent PID readings will also be recorded.
CDM Smith assumes that setup, testing, and breakdown for the test will require one day. Three soil gas samples will be submitted to the laboratory for TO-15 analyses with standard reporting times.
Deliverable: A field package and data deliverable will be submitted to include field notes, laboratory chain of custody, and MAGS™ field testing results.
Subtask 3: MAGS Retesting Report
Deliverable: Contractor will prepare a MAGS™ Retesting Report. The report will include updated figures and data tables, field notes, a copy of the laboratory analytical report and chain of custody, a summary of sampling methodologies, quality assurance/quality control evaluation of data, field documentation, conclusions, and recommendations.
A copy of the FDEP Cost Estimate Form and Level of Effort is included in Attachment B.
