About this file

This document package consists of a Task Assignment Change Order (TACO) executed between the State of Florida Department of Environmental Protection (FDEP) and CDM Smith Inc. to amend an existing task assignment under Contract Number HW681. The project involves long-term monitoring services at the Former Bay's Cleaners site (ERIC_4918) located at 219 North Magnolia Avenue, Ocala, Marion County, Florida. Benjamin Aoki serves as the FDEP Project Manager, while Kathy Leggoe represents CDM Smith as the Contractor's Project Manager. The change order was signed on May 12, 2025 (contractor) and May 9, 2025 (FDEP), with an effective completion date of June 27, 2025.

This TACO reconciles two previously issued Work Change Directives (WCD 1 and WCD 2) that authorized additional groundwater sampling activities and waste management services. The amended total task value is $37,922.20, representing an increase of $1,950.69 from the prior amount of $35,971.51. The deliverables include three subtasks: Annual Groundwater Sampling (field notes, field logs, and chain of custody due 03/27/2025, invoiced at $23,379.11); PFAS Sampling and IDW waste management (due 03/27/2025, invoiced at $3,697.58); and Annual Groundwater Monitoring Report with updated data tables and conclusions/recommendations (due 04/28/2025, invoiced at $10,845.51). Supporting documents include detailed cost estimates, labor worksheets, subcontractor invoices from Erwin Remediation ($1,715.00 for IDW disposal and PFAS waste profiling) and Cascade Drilling ($775.21 for drum materials), and laboratory analyses for volatile organic compounds and PFAS parameters.

CDM Smith observed insufficient drums included for purchase and disposal for the site based on previous sampling addition requested for both. Also, drum purchase is limited in the vicinity of the site. A local driller can deliver the up to 5 drums to the site for a nominal fee.

Costs to purchase and dispose of up to 5 drums (2 in original TA, 3 additional requested). Costs to deliver 5 drums to the site as part of purchase.
