Document Summary
This Task Assignment Change Order (TACO #1) represents an amendment to Task Assignment DC005B under Florida Department of Environmental Protection (FDEP) Contract HW681 with contractor CDM Smith Inc. The document reconciles three previously issued Work Change Directives (#1, #2, and #3) for environmental assessment work at Bay's Cleaners (ERIC_4918), located at 219 N Magnolia Avenue, Ocala, Marion County, Florida. The change order is effective March 31, 2023, with key contacts including Kyle McMurray (FDEP Contract Manager) and Shayne Wood (CDM Smith Contract Manager).
The amended task assignment reflects an overall cost reduction of $26,769.98, bringing the total TA value from $185,025.33 down to $158,255.34, representing a fixed unit rate compensation structure. The cost adjustment accounts for originally planned well installations that were reduced from eight to two wells, offset by additional expenses including 46 groundwater sampling events, 78 SPLP soil sample encores, supplemental IDW drums, plywood, additional Portland cement (40 bags), hole plugs (50 bags), and increased CDM labor and subcontractor time. Deliverables include documentation of baseline groundwater sampling (Subtask 2), supplemental site assessment field activities with monitoring well installation (Subtask 3), and a Supplemental Site Assessment Report with project management (Subtasks 1 and 4), with a completion date of March 15, 2023. Invoice amounts are structured as $29,657.57 for Subtask 2, $111,409.42 for Subtask 3, and $17,188.35 for Subtasks 1 and 4.

TACO #1
ERIC_4918
This Task Assignment Change Order reconciles WCD #1, #2, and #3 which authorizes an overall reduction in cost for 6 wells not being installed. Addition cost was accrued for 46 GW sampling events, 78 encores for SPLP soil samples, additional drums, Plywood, CDM labor, 40 additional bags of Portland cement, 50 additional bags of hole plug, and subcontractor time. In accordance with the CDM Smith (CDM) Change Order Request and cost estimate forms received on March 6th, 2023.
Changes to the original CDM scope of work include the following:
Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.
Subtask 2: Site Preparation and Baseline Groundwater Sampling Subtask 2 has an overall cost increase for additional GW sampling events and associated travel, materials, and labor. An additional 5 IDW drums were required.
Subtask 3: Field Investigation Subtask 3 has an overall cost reduction due to only 2 of the proposed 8 wells being installed.
Additional cost was accrued for additional grout, encore sampling, plywood, ROW permitting, CDM labor, and subcontractor time.
The overall cost reduction related to this Change Order is $26,769.98.
The FDEP project manager agrees to the scope of work and final cost associated with the Task Assignment Change Order. The final cost total for Task Assignment DC023A is $158,255.34.
There is a $0.01 difference in the CEF due to rounding errors.

More IDW generated than anticipated; the site is located in an unsafe neighborhood--several homeless people have approached staff in a hostile manner during this sampling event. Teams cannot split up to sample as originally planned, which will mean more time will be required to complete the baseline sampling event.
Additional labor time for CDM Smith staff to sample wells safely. Additional drums, subcontractors needed due to more IDW than anticipated.

Hey John, How have you been? I have some work that I thought you may be interesting in bidding on concerning our 10 year FDEP contract work.
If you are interested, I would like to request a cost proposal for the pickup and disposal of all investigation derived waste associated with our FDEP projects for one year.
The proposal should include both non-hazardous and hazardous waste (you can hold off on the hazardous if it’s an issue), liquid and solid, and varying quantities (drums, roll-offs, 250-gallon totes). It should also include rates for sites located throughout the state.
If accepted, the costs outlined in the proposal would be agreed upon for a year before renegotiations. The sites will be spread throughout Florida and include Pensacola, Jacksonville, Tampa, Orlando and South Florida.
The majority of the waste will be non-haz soil and groundwater in 55-gallon drums.
If you are interested, please submit a proposal by the end of next week. Please feel free to reach out with any questions.
Thanks, Joe Kinard, GIT Geologist III CDM Smith 4651 Salisbury Road, Suite 420 Jacksonville, FL 32256 O: 904.527.6714 M: 904.316.7522
