Professional Cleaners #2 (ERIC_4591)
6052 San Jose Boulevard, Jacksonville, Duval County, Florida
Dear Mr. Aoki:
CDM Smith Inc. (CDM Smith) is pleased to respond to the Florida Department of Environmental
Protection (FDEP) Task Assignment Request to provide supplemental site assessment (SSA) services for the Professional Cleaners #2 site under Contract Number HW681. The scope of work and project schedule are described below, and the cost estimate for is attached in Table 1.
CDM Smith submitted a Site Assessment Report (SAR) to the FDEP on April 8, 2024. On May 7, 2024, FDEP approved the site for closure and the issuance of a Site Rehabilitation Completion Order (SRCO).
Once FDEP issues the SRCO and the 21-day period with no filing for administrative hearing, the FDEP will require site restoration to include the abandonment of all monitoring wells and vapor extraction wells.
Scope of Work
CDM Smith presents the following proposed subtasks to complete site restoration.
Subtask 1 – Project Management
Project management activities performed under this task consist of administrative functions required to maintain the project schedule, adhere to budget, and deliver the quality of the work products defined by CDM Smith’s quality standards. CDM Smith will prepare and submit invoices upon approval of each deliverable or milestone completion.
Project Kickoff and Progress Meetings
CDM Smith expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.
Project Quality Technical Review
CDM Smith’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.
Mr. Benjamin Aoki
4651 Salisbury Rd., Suite 420, Jacksonville, FL 32256 | tel: 904-731-7109
Project Status Reports
CDM Smith’s Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project. The Project Manager will also discuss the status of the project with the FDEP on a bi-weekly basis until the project is complete.
Subtask 2 – Monitoring Well and Vapor Extraction Well Abandonment
CDM Smith will properly abandon five (5) monitoring wells (MWs) and five (5) Vapor Extraction Wells
(VEWs) with a licensed Florida Water Well Contractor. This includes the removal of the 2’x2’ concrete well pads when applicable, manhole covers, and the 2” diameter PVC for VEWs. All boreholes will be properly grouted to the surface via tremie pipe and restored to preexisting grade conditions with concrete patch.
A Site Restoration Report will be prepared to summarize the field activities conducted.
Deliverables: Site Restoration Report to include field documentation, and paid invoices to subcontractors.
Subcontractors:
Drillers
Bids to abandon MWs and VEWs were submitted to Groundwater Protection, Environmental Drilling
Service (EDS) and Preferred Drilling Solutions (PDS). EDS was selected as the licensed driller with the lowest bid, as shown in Attachment A, which includes a summary of the subcontractor cost comparisons.
Assumptions
CDM Smith will communicate with the FDEP in a timely manner is schedule delays are encountered and will prepare necessary paperwork for change orders.
Schedule
CDM Smith’s proposed project schedule is shown below, with targeted completion within four months.
The deliverable and due date is provided in Table 2.
