Task Assignment DC034B East Bay Dry Cleaners

ERIC_5277

Long Term Monitoring (LTM) will permit and abandon wells that are located within the excavation area. Wells will be abandoned bottom up using Portland cement grout. Completion reports will be obtained following well abandonment at wells MW002S, MW004S, MW004I, MW004D, MW005S, MW005I, MW005D, MW008S, MW008I, and MW008D.

This Task Assignment authorizes CDM Smith Inc., hereby referred to as CDM, to conduct Long Term Monitoring (LTM) at East Bay Dry Cleaners, Pinellas County, Florida (FL). Erwin Remediation (Erwin) was selected as the lowest responsive and responsible bidder. Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2: Annual Groundwater Sampling CDM will mobilize to the Site to collect groundwater samples from 10 monitoring wells. CDM will perform monitoring well inspections to determine the condition and gauge depth-to-groundwater in all 10 monitoring wells at the Site.

A YSI-556 multi-parameter probe and a turbidimeter will be used to collect field data.

Field parameter data to be collected will include dissolved oxygen, pH, conductivity, oxidation reduction potential, temperature, and turbidity.

All groundwater samples will be collected and delivered to a NELAP certified laboratory for analysis by United States Environmental Protection Agency Method 8260C for volatile organic halocarbons (VOHs).

Subtask 3: Annual Groundwater Monitoring Report CDM will prepare an Annual Groundwater Monitoring Report. The report will include updated figures and data tables, field notes, a copy of the laboratory analytical report and chain of custody, a summary of sampling methodologies, quality assurance/quality control evaluation of data, groundwater samplings logs and field equipment calibration records, conclusions, a summary of the file review, and recommendations.

Subtask 4: PFAS Impacted IDW Disposal This subtask is for costs associated with a PFAS Waste Profile and PFAS impacted IDW disposal.

The total cost for Task Assignment DC034B is $15,928.96 with 80 labor hours.

recommendations, source removal activities will be conducted. The following sequence of activities is proposed for the source removal. The excavation activities will be conducted using a track-mounted excavator. Initially the deeper soils (up to 12 feet below land surface) with impacts will be excavated, stockpiled, and loaded for offsite disposal. Once the deeper

Re: Non-Hazardous Drum Disposal of PFOS/PFAS Impacted Materials

Mr. Turner:

The price for the transportation and disposal (T&D) of non-hazardous drums of soil, mud, and water located in the State of Florida is listed below:

Non-Hazardous PFAS/PFOS Impacted Material:

Description Rate Unit

Profile Fee (Charged once per site) $ 350.00 Per Site

T&D - Non-Haz PFOS/PFAS Impacted Soil/ Mud/Water $ 195.00 Per Drum

Non-Hazardous Material not Impacted with PFAS/PFOS:

Description Rate Unit

T&D - Non-Haz (no PFOS/PFAS) Impacted Mud/Water $ 125.00 Per Drum

T&D - Non-Haz (no PFOS/PFAS) Impacted Soil $ 110.00 Per Drum

Please ensure that all drum lids are properly secured and that drums are on a level surface like the surrounding area (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.

The prices listed are all inclusive without any additional fees or charges (fuel surcharge, low volume stop fee etc.) and are valid for all regions in the State of Florida except for the Florida Keys. Pricing for sites located in the Florida Keys will be bid on a case-by-case basis dependent on the volume of drums and the location.

This quote is valid for 90 days.

Respectfully, Robert Donado Erwin Remediation, Inc.
