SSAR

CDM Smith Inc. (CDM) will conduct Supplemental Site Assessment. Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2: Baseline Groundwater Monitoring and MAGs Sampling re-testing Groundwater samples will be collected from the 13 existing monitoring wells and analyzed for VOHs by EPA Method 8260B. Water quality data will be collected during well purging including dissolved oxygen, pH, conductivity, oxidation-reduction potential, temperature, and turbidity. Well purging and sample collection will be completed in accordance with FDEP Standard Operating Procedure FS 2200. Water quality data will be collected during well purging including dissolved oxygen, pH, conductivity, oxidation-reduction potential, temperature, and turbidity. MAGS testing will be performed for existing MAGS™ wells M-2, M-3, M-4, and M-5 to confirm the results collected during the 2020 SAR activities, general accordance with the Modified Active Gas Sampling Manual dated October 12, 2004, prepared by HSA Engineers & Scientists. Prior to initiating Subtask 3 monitoring well installation, CDM Smith and FDEP will perform a technical review of the groundwater data to determine the locations and depths where additional assessment is needed to complete delineation of the contaminants of concern at the site. This task includes a teleconference between CDM Smith and FDEP.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis. 2 IDW bids are included and the 3rd from EnviroServe was non-responsive.

Subtask 3: Monitoring Well Installation CDM Smith proposes to install five additional monitoring wells at the locations recommended in the SAR. Monitoring wells MW-22 and MW-23 are within the site property boundary. The remaining three monitoring wells are located offsite, as summarized below, and will require offsite access agreements. Contractor will subcontract utility location services to clear the boring locations prior to well drilling. In addition, Contractor will also request Sunshine 811 services to mark the public utilities. The total well depths range from 40 feet (ft) to 100 ft below land surface (bls). Sonic drilling was used to install the monitoring wells during the site assessment and

Contractor recommends using the same method for the new well installations. Continuous formation samples will be collected to the total well depth and a boring log will be recorded. Sonic 8-diameter override casing has been used at this site to install monitoring wells below a depth of approximately 35 ft bls. PFAS free water for this scope of work was $2,375. The monitoring wells will be completed as 2-inch diameter PVC wells. The same screen lengths installed during the site assessment will be installed for the new wells and all screens will be 5 ft long. Sand pack and a fine-grained sand seal will be installed, and the remainder of the borehole annulus grouted. The five new monitoring wells (MW-19, MW-20, MW-21, MW-22, and MW-23) will be surveyed by Southeastern Surveying and mapping.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis. 2 IDW bids are included and the 3rd from EnviroServe was non-responsive.

Subtask 4: Groundwater Monitoring Water levels will be recorded from the 18 monitoring wells. The five new monitoring wells (MW- 19, MW-20, MW-21, MW-22, and MW-23) will be sampled and analyzed for VOHs by EPA Method 8260B. In addition, existing monitoring wells MW-11 and MW-17 will be sampled for natural attenuation parameters, as recommended in the SAR. Water quality data will be collected during well purging including dissolved oxygen, pH, conductivity, oxidation-reduction potential, temperature, and turbidity. Well purging and sample collection will be completed in accordance with FDEP Standard Operating Procedure FS 2200.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis. 2 IDW bids are included and the 3rd from EnviroServe was non-responsive.

Subtask 5: Reporting Contractor will prepare a SSAR in letter format to document the completion of the site assessment work phase and recommend the next steps. This report will document all field activities and results and will include updated figures and data tables along with any notable exceedances and CDM Smith’s conclusions and recommendations.

CDM and the FDEP confirmed on June 26th, 2023, that an Access Agreement with the current real property owner of the site has been executed.

The final cost total for Task Assignment DC031B is $89,596.27.

Mr. Kyle McMurray Florida Department of Environmental Protection Division of Waste Management 2600 North Blair Stone Road Tallahassee, Florida 32399

Subject: Task Assignment Request – Supplemental Site Assessment

Norgetown Cleaners 4336 South Manhattan Avenue, Tampa, Hillsborough County, Florida

Dear Mr. McMurray:

CDM Smith Inc. is pleased to submit this Task Assignment Request for Supplemental Site Assessment including monitoring well installation and groundwater sampling activities at the Norgetown Cleaners (the Site) located at 4336 South Manhattan Avenue, Tampa, Hillsborough County, Florida, ERIC_4745.

The scope of work, proposed deliverable, and deliverable schedule are summarized below and the cost estimate is also attached.

This Task Assignment, when executed, shall be incorporated in, and become part of Contract No.

HW681, between the Florida Department of Environmental Protection (FDEP), and CDM Smith, Inc.

(Contractor), dated January 5, 2022, hereafter referred to as the Contract.

Project Understanding Contractor understands that FDEP is requesting well installation and groundwater monitoring services at the Norgetown Cleaners Site located in Tampa, Florida. A revised Site Assessment Report (SAR) was submitted in August 2021 that recommended installation of additional offsite monitoring wells to delineate the extent of volatile organic halogens (VOHs) in groundwater. This task assignment will fulfill the well installation and groundwater monitoring requirements and a letter report will be submitted as a Supplemental SAR (SSAR) to complete the site assessment phase at this site.

Scope of Work The proposed scope of work includes the following elements:

Subtask 1: Project Management Project management activities performed under this task generally consist of administrative functions required to maintain the project schedule, adhere to budget, and deliver the quality of the work products defined by Contractor's quality standards. Contractor's project manager will be Kara Fincham, PMP. We will prepare and submit invoices upon approval of each deliverable or milestone completion.

Project Kickoff and Progress Meetings Contractor expects a kickoff meeting or telephone conference with the FDEP to review the project status and address FDEP comments regarding the Task Assignment. Contractor will prepare meeting agendas and notes as part of this task.

Project Quality Technical Review Contractor 's Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.

Project Status Reports Contractor 's Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project. The Project Manager will also discuss the status of the project with the FDEP on a bi-weekly basis until the project is complete.

Subtask 2: Baseline Groundwater Monitoring and MAGS sampling re-testing Groundwater at the site has only been sampled once in August 2020 and as discussed with FDEP, a groundwater sampling event will be performed prior to well installation to determine current conditions. Water levels will be recorded from the 13 existing monitoring wells prior to sampling.

Groundwater samples will be collected from the 13 existing monitoring wells and analyzed for VOHs by EPA Method 8260B.

Contractor will employ the low-flow purge and sampling approach using a peristaltic pump and flow-through cell. A YSI-556 multi-parameter probe and a turbidimeter will be used to measure the water quality. All instrumentation will be properly calibrated and documented.

All purge water will be containerized and transferred to a Florida Department of Transportation (FDOT)-approved 55-gallon drum. Contractor assumes that less than two drums of groundwater IDW will be collected and combined with the drilling IDW to be disposed of under Subtask 3.

Modified Active Gas Sampling™ Re-Testing

MAGS testing will be performed for existing MAGS™ wells M-2, M-3, M-4, and M-5 to confirm the results collected during the 2020 SAR activities, general accordance with the Modified Active Gas Sampling Manual dated October 12, 2004, prepared by HSA Engineers & Scientists. More specifically:

To begin the MAGS testing, the Contractor will set up optimum vacuum and flow rates based on the test performed by Cardno in 2020. Using the previously determined flow rate and vacuum, a vapor sample will be collected at each MAGS™ well. The vapor samples will be collected using a 6-liter Summa canister. A purge time will be calculated based on the flow rate and estimated radius of influence, to ensure the collection of steady-state data. Vacuum and flow rate readings will be recorded, along with vacuum readings at select monitor wells and designated vacuum monitoring points, at 15-minute intervals for a total duration of one hour. Influent PID readings will also be recorded.

CDM Smith assumes that setup, testing, and breakdown for the test will require one day. Four soil gas samples will be submitted to the laboratory for TO-15 analyses with standard reporting times.

Well purging and sample collection will be completed in accordance with FDEP Standard Operating Procedure FS 2200. Water quality data will be collected during well purging including dissolved oxygen, pH, conductivity, oxidation-reduction potential, temperature, and turbidity.

Deliverable: Field documentation, groundwater sampling logs, tables, figures, laboratory report, IDW disposal documentation, MAGS™ Field Testing Results

Technical Review and Meeting: Prior to initiating Subtask 3 monitoring well installation, CDM Smith and FDEP will perform a technical review of the groundwater data to determine the locations and depths where additional assessment is needed to complete delineation of the contaminants of concern at the site. This task includes a teleconference between CDM Smith and FDEP.

MW-19: 4315 South Manhattan Avenue, Tampa 14 Properties, LLC MW-20 and MW-21: 4300 South Manhattan Boulevard, Manhattan Baptist Church

The off-site access attempts will be secured by the following steps:

1. CDM Smith personnel will conduct a site visit up to two times to meet with the site owner and/or tenant to get a signed off-site access agreement. If the access agreement is not signed during the first two in-person visits, CDM Smith send access request letters to each applicable property owner via certified US Mail or Federal Express so receipt of the letter can be verified and placed in the project files. Each letter will include the standard FDEP language, non-Source permission to enter property forms for signature, and a proposed map that will show the location of the proposed groundwater monitoring well(s).

2. If these three attempts are unsuccessful, the FDEP will be notified and CDM Smith will relocate the proposed wells to the closest Right-of-Way (ROW) location. CDM Smith will then submit the necessary ROW permit application/maintenance of traffic plan to complete proposed groundwater monitoring well (s). CDM Smith has included provisions in this task assignment to administer the access agreements and ROW permits with the applicable property owners/parties. This process could take 1-2 months to complete.

Underground Utility Survey Contractor will subcontract utility location services to clear the boring locations prior to well drilling. In addition, Contractor will also request Sunshine 811 services to mark the public utilities.

Drilling The total well depths range from 40 feet (ft) to 100 ft below land surface (bls). Sonic drilling was used to install the monitoring wells during the site assessment and Contractor recommends using the same method for the new well installations. Continuous formation samples will be collected to the total well depth and a boring log will be recorded. Sonic 8-diameter override casing has been used at this site to install monitoring wells below a depth of approximately 35 ft bls. PFAS free water will be utilized during the well installation activities and is included in the costs provided by the Driller. The estimated cost for

PFAS free water for this scope of work was $2,375. The proposed monitoring well construction for this task assignment is summarized below.

Table 1: Proposed Monitoring Well Construction Well Override Casing Total Depth Screen Length

► MW-19 40 ft 50 ft 5 ft

► MW-20 30 ft 40 ft 5 ft

► MW-21 50 ft 60 ft 5 ft

► MW-22 90 ft 100 ft 5 ft

► MW-23 30 ft 40 ft 5 ft

Totals 240 ft 290 ft 25 ft

The monitoring wells will be completed as 2-inch diameter PVC wells. The same screen lengths installed during the site assessment will be installed for the new wells and all screens will be 5 ft long. Sand pack and a fine-grained sand seal will be installed, and the remainder of the borehole annulus grouted. The subcontractor will develop the wells following completion. The wells will be surveyed for location and elevation by a professional surveyor.

Investigation-Derived Waste (IDW) All investigation-derived waste (IDW) including soil cuttings and well development water will be containerized in FDOT approved 55-gallon drums and staged onsite. Based on the Driller proposal, the Contractor estimates 25 drums of IDW will be collected for disposal as nonhazardous waste. One soil and one water sample will be collected for disposal characterization. The soil sample will be submitted for analysis using the Toxicity characteristic leaching procedure (TCLP) for volatile organic compounds (VOCs) by EPA Method 8260. The water sample will be submitted for analysis of VOCs by EPA Method 8260.

Surveying The five new monitoring wells (MW-19, MW-20, MW-21, MW-22, and MW-23) will be surveyed by Southeastern Surveying and mapping.

Deliverable: Field documentation, offsite access agreements, boring logs, well construction details, and IDW disposal documentation

Subtask 4: Groundwater Monitoring Water levels will be recorded from the 18 monitoring wells. The five new monitoring wells (MW-19, MW-20, MW-21, MW-22, and MW-23) will be sampled and analyzed for VOHs by EPA Method 8260B. In addition, existing monitoring wells MW-11 and MW-17 will be sampled for natural attenuation parameters, as recommended in the SAR. The additional natural attenuation assessment will include the following parameters.

Alkalinity by SM2320B Iron by SW-846 Method 6010 Nitrate by Method 9056A Sulfate by Method 9056A Total organic carbon by SW846 Method 9060A Dissolved gases (methane, ethane, and ethene) by RSK-175 Degrading bacteria and functional genes by QuantArray®-Chlor

Water quality data will be collected during well purging including dissolved oxygen, pH, conductivity, oxidation-reduction potential, temperature, and turbidity.

Monitoring Procedures Well purging and sample collection will be completed in accordance with FDEP Standard Operating Procedure FS 2200. Water quality data will be collected during well purging including dissolved oxygen, pH, conductivity, oxidation-reduction potential, temperature, and turbidity. Contractor will employ the low-flow purge and sampling approach using a peristaltic pump and flow-through cell. A YSI-556 multi-parameter probe and a turbidimeter will be used to measure the water quality. All instrumentation will be properly calibrated and documented.

All purge water will be containerized and transferred to a FDOT-approved 55-gallon drum. Contractor assumes that less than one drum of IDW will be collected and combined with the drilling IDW and will require disposal under Subtask 3.

Deliverable: Field documentation, groundwater sampling logs, laboratory report, and IDW disposal documentation.

SITE LINES LLC PROPOSAL

SITE LOCATION

Norgtown Cleaners Site – 4336 South Manhattan Avenue, Tampa, FL.

PROPOSED SCOPE OF WORK

Site Lines (“Company”) will use Ground Penetrating Radar (GPR) and conventional electromagnetic (EM) pipe and cable locators to determine the location of underground utilities at the site identified by CDM Smith (“Client”). Of specific concern is the location of utilities within a 10-foot radius of 5 proposed well drilling locations.

RESULTS OF THE INVESTIGATION

The estimated depth and location of utilities identified during the investigation will be marked on the ground surface using spray paint or other method to be agreed upon by the parties. The American Public Works Association Uniform Color Codes for the marking of underground utilities will be used.

PROJECT FEE

The fee to complete the project as described within this proposal is:

Six Hundred Dollars [$600.00]

Client shall pay Company in full within thirty (30) days of the invoice date, unless the parties have agreed to another payment arrangement in writing. If during the investigation, Company determines that the goals of the project cannot be achieved with the resources available to Company, Company will notify Client as soon as reasonably possible. Under such circumstances, Company will charge Client only for time expended and materials used to date.

CLIENT REQUIREMENTS

Client is responsible for all site access, maintenance of traffic (MOT), site clearing and/or any other logistical considerations necessary to conduct the utility locate.

Client will also provide all pertinent maps of the project site, including those that show the boundaries of the utility locate, pertinent landmarks, and known utilities (if available). Company is not a substitute for Chapter 556, Florida Statutes.

Accordingly, Client is responsible for calling Sunshine State One Call of Florida

(811) before undertaking any invasive activities, whether through Company, any other entity, or on its own.

DESCRIPTION OF COMPANY INVESTIGATION AND LIMITATIONS

A GSSI SIR-4000 GPR system will be used with a 900, 400, or 270 megahertz antenna configuration depending on site conditions and desired targets. The GPR will assist in locating nonmetallic and metallic utilities. Physical surface conditions, subsurface conditions and technical limitations effect the ability of GPR to resolve targeted utilities and/or subsurface structures. Depth and/or diameter of the utility, soil type, moisture/ionic content of the soil, and electrical properties of the soil compared to that of the utility can prevent the GPR from identifying utilities at the site. The diameter of a utility cannot be determined by GPR data. Estimated depths of utilities are directly affected by soil conditions and the material of the utility. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities.

An RD 8000 will be used for the EM survey. Electromagnetic induction which transmits a signal directly into the desired target and passive power detection which locates naturally occurring magnetic fields in energized lines will be identified by detecting and following the signals with a receiver. The EM survey will assist in finding only metallic utilities. The magnetic field and/or induced signal should emanate from the target in a circular geometry. When this occurs, the receiver will receive its peak signal directly above the target. However, when metallic utilities are running near each other, or there is environmental interference from fences, cars or overhead power lines, the signal can become distorted and the peak no longer can be directly over the utility. Care must be taken when doing any invasive work around the utilities. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities. Company’s personnel are highly experienced and certified operators of GPR and EM pipe and cable locators.

Due to the limitations described above, when Client is working around indicated utilities, a minimum of two (2)-feet clearance from either side of the marked utility will be maintained. For absolute confirmation, Client is responsible for exposing any indicated underground utility.

Company will conduct utility investigations in accordance with the level of care and generally accepted professional practices as ordinarily exercised by members of the same profession practicing in the same locality under similar conditions.

REPRESENTATIONS AND WARRANTIES

Neither Company nor its employees or agents are licensed surveyors under Florida law, nor do they represent themselves as such. Accordingly, the work Company is to perform is not a substitute for work typically completed by a licensed surveyor.

Company specifically and expressly disclaims any liability for any type of property damage or personal injury, including but not limited to death, arising from client’s failure to determine that the services of a licensed surveyor are necessary or to retain a licensed surveyor to perform necessary services.
