Temple Terrace, Hillsborough County, Florida
Dear Ms. Jean-Baptiste:
COM Smith Inc. (COM Smith) is pleased to respond to the Florida Department of Environmental
Protection (FDEP) Task Assignment Request to provide site assessment services for the Elite
Cleaners site under Contract Number HW681. COM Smith's project understanding, scope of work, and project schedule are described below and the cost estimate is also attached.
Project Understanding
COM Smith understands that FDEP is requesting site assessment and reporting services. COM Smith submitted a Site Assessment Work Plan on June 13, 2022. COM Smith understands that FDEP would like for this site to progress through site assessment within 1 year of starting work. The work plan provided the following information and conclusions on the site status.
■ The dry cleaner is currently active.
■ A release of tetrachloroethene (PCE) to soil adjacent to the drycleaning machine was documented by FDEP in 1996 at a concentration of 104,000 µg/kg.
■ Dry cleaning at the facility began in November 1986 using PCE and switched to petroleum based solvents in 2014.
■ A 2016 well survey identified six large public wells within ½-mile of the site.
■ The groundwater depth was reported to be 8-12 feet (ft) below land surface (bis) and the site consist of sand, silt, clay, and shell in the surficial aquifer.
The site location is shown on Figure 1, and COM Smith expects a north-northeastern groundwater flow direction toward the Hillsborough River.

Site Assessment Scope of Work
CDM Smith has developed an approach to complete the site assessment within approximately 9 months, leaving 3 months as a contingency to complete the site assessment within 1 year. The field work will be completed in four stages with progress being used to continuously refine the approach. The subtasks are described below and the work sequencing and schedule are shown on
Figure 2.
Subtask 1 - Project Management
Project management activities performed under this task generally consist of administrative functions required to maintain the project schedule, adhere to budget, and deliver the quality of the work products defined by CDM Smith's quality standards. CDM Smith's project manager will be
Kara Fincham, PMP. We will prepare and submit invoices upon approval of each deliverable or milestone completion.
Project Kickoff and Progress Meetings
CDM Smith expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.
Project Quality Technical Review
CDM Smith's Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.
Project Status Reports
CDM Smith's Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project. The Project Manager will also discuss the status of the project with the FDEP on a bi-weekly basis until the project is complete.
Subtask 2 -Site Preparation
Underground Utility Survey
The work plan identified only a portion of the underground utilities. CDM Smith will subcontract a utility location services company to provide a complete survey of the utilities.
Deliverable: Surveys, paid invoices to surveyors
Subtask 3 -Stage I Field Investigation
The Stage I Field Investigation will delineate the extent of volatile organic halocarbons (VOHs) exceeding the Soil Cleanup Target Levels (SCTLs). Split soil samples will be collected and held for
Synthetic Precipitate Leaching Procedure (SPLP) testing for samples that are below the direct contact SCTL and above the default leachability SCTL.
July 22, 2022
Soil borings will be completed by COM Smith at 13 locations (Figure 3). Borings will be installed using hand augers and concrete coring will be completed at 13 locations. The borings will be plugged using cement grout and concrete patches.
Laboratory analyses will be completed at the following depths: 0-0.5 ft, 0.5-2 ft, 2-4 ft, 4-6 ft, 6-8 ft, 8-10 ft, and 10-12 ft, or until encountering groundwater. The 13 borings on paved surfaces will not have 0-0.5 ft samples. The sample quantities are summarized in Table 1. COM Smith will collect samples to be analyzed for SPLPs, but will identify which samples to run for SPLP after the data are received from the laboratory. Therefore, costs associated with SPLPs are not included in this cost estimate.
Vapor extraction wells (VEWs) for MAGs testing will also be installed at five locations shown on
Figure 3. These VEWs will require concrete coring and soil samples will be collected for VOC analyses with standard laboratory reporting turnaround time. The soil samples will be collected at the following depths: 0.5-2 ft, 2-4 ft, 4-6 ft, and 6-8 ft. Splits for SPLP will not be collected for the
VEW soil samples.
The VEWs will be 2-inch diameter PVC with a total depth of 8 ft and a minimum borehole diameter of 6 inches. The outdoor VEWs will be augered and the indoor VEW will be installed manually. The screen lengths will be 2.5 ft and a sand pack will be installed to a depth of 2 ft. The remainder of the borehole will be filled with sand/cement grout and completed with a flush-mount cover. A DPT rig will be used for outdoor locations.
COM Smith will also install 5 vapor monitoring points (VMPs) using a hand auger to an 8-ft depth.
The VMPs will be 2-inch diameter PVC with slotted screens, annular backfill will be the same as for the VEWs, and the VMPs will be completed with flush-mount covers. The VEWs and VMPs will be retained for potential future use for soil vapor extraction.
MAGs testing will be performed for each. A step test will be completed on VM-01 only to select the
MAGs test flow rates. The MAGs tests for each well will be completed within 1 hour or less and COM
Smith assume that setup, testing, and breakdown for all tests will require 2 days. A total of five soil gas samples will be submitted to the laboratory for TO-15 analyses with standard reporting times.
Following MAGS testing, vertical groundwater profiling will be conducted at nine locations to assess the groundwater flow direction (Figure 4). All locations will require coring. All locations will be repaired following abandonment. Groundwater samples will be collected using direct-push screen points at the following depth intervals: 6-10 ft, 16-20 ft, 26-30 ft, and 36-40 ft.
July 22, 2022
In addition, a soil boring will be sampled continuously at LB-01 to a SO-ft depth or equipment refusal, whichever comes first, before profiling. Temporary wells will be set at a 10-ft depth using 1-inch diameter PVC. Annular backfill will not be required. COM Smith will survey the casing elevations and collect water levels from the temporary wells within 24 hours and the well will be pulled and the boreholes abandoned and patched.
At the completion of field events, the utilities that were marked during mobilization and all boring locations will be surveyed. Investigation-derived waste (IDW) collected up to this point will be characterized and transported off-site for disposal. A total of five soil drums and one water drum of nonhazardous IDW are anticipated at the completion of Stage I.
Deliverable: Laboratory analytical reports, field documentation, boring logs, data tables, paid invoices for subcontractors
Subtask 4 - Stage II Field Investigation
The monitoring well locations will be finalized based on the Stage II results. COM Smith currently plans to install five shallow permanent groundwater monitoring wells and one deep well at the locations shown on Figure 4. Prior to well installation, continuous soil cores will be collected to the total depth. Five of the wells will be shallow wells installed to a total depth of 15 ft bis and one well
(MW-01O) to a total depth of 50 ft bis. All wells will be 2-inch diameter PVC and the shallow well will have 10-ft 0.01-slot screens, and the deep well will have 5 feet of 0.01-slot screen. Sand pack will be installed to a depth of 1 ft above the screen followed by a 1-ft-thick fine-grained sand seal and the remainder of the borehole grouted. The wells will be developed following completion.
The wells will be purged and sampled for VO Hs following installation and a full round of water levels will be collected to map the potentiometric surface. The monitoring wells will be surveyed at the completion of Subtask 5 (Stage III) and IDW will be characterized and transported off-site for disposal. A total of 16 soil drums and 2 water drums of nonhazardous IDW are anticipated at the completion of Stage II.
Deliverable: Completed well logs, laboratory analytical reports, paid invoices
Subtask 5 - Site Assessment Report
A conventional Site Assessment Report will be drafted for FOEP review and comment. A representative table of contents for the Site Assessment Report is shown in Attachment A.
Subcontractors
Subcontracted services are required for drilling, surveying, utility location, and IDW disposal. The bidding, results, and subcontractor selections are described below, and Attachment B includes a summary of the subcontractor cost comparisons.
July 22, 2022
Drilling
Requests for Quotation were sent to three pre-qualified drilling companies for all drilling services.
The lowest cost bid was DrillPro.
Surveying
Southeastern Surveying and Mapping is a primary subcontractor for HW681 and other bids were not solicited for this project. The total quote is show in Attachment C.
Utility Location
Bids were obtained from Blood Hound, GPRS, and Site Lines LLC to complete the utility survey. The total quotes are shown in Attachment C.
IDW Disposal
Rates for IDW removal and disposal were previously requested for HW681 from Erwin
Remediation, Clean Earth, ERS, and Clark Environmental. These rates were used to assess the current IDW disposal costs for the Elite Cleaners site assessment. Erwin Remediation was the lowest cost and has been selected as the IDW disposal subcontractor for this project at a cost of
$3,900 as shown in Attachment C.
Assumptions
This scope and schedule have been prepared predicated on unknown site conditions. The scope and schedule may change depending on surveys and the drilling conditions. Drillers and other subcontractors' availability may be limited and impact the schedule. In addition, this scope does not take into account any weather delays. CDM Smith will communicate with FDEP in a timely manner if this occurs to prepare the necessary paperwork for change orders.
Schedule
CDM Smith's proposed project schedule is shown on Figure 2 and shows targeted completion of the
Site Assessment process within 9 months. The final dates on the schedule depend on Notice to
Proceed (NTP) and subcontractor availability. Once CDM Smith receives NTP, the subcontractor's availability will be confirmed and the schedule may be adjusted. The deliverables and due dates are summarized in Table 2 below.

Subject: Request for Bid - Former Bays Cleaners, Metro Cleaners, and Elite Cleaners
Blake, COM Smith is soliciting bids for environmental drilling services at three sites listed below.
COM Smith is a prime contractor to the Florida Department of Environmental Protection under its Dry Cleaning Solvent
Cleanup Program and these sites are being managed under the program. COM Smith is soliciting a bid from your company because we believe that you are qualified to complete the required work, as described below. If you wish to remain on CDM Smith's list of pre-qualified drilling companies for this type of work, please respond to this email indicating whether your company will or will not submit a response to this request.
Please complete each tab (one for each site) in the attached bid document and return to me by Monday, June 6th. Also, please provide us with your earliest availability for each project.
Summaries of work requested at each site are presented below:
Former Bays Cleaners (210 North Magnolia Avenue, Ocala, FL)
• (13x) Continuous soil borings to approximately 46' bis. Locations may require coring through concrete or asphalt. All locations must be repaired with like material following proper abandonment.
• (3x) Installation of 2" monitoring wells, 50' bis with ten feet of 0.01-slot screen. Sand pack and a fine-grained sand seal will be installed to above the screen and the remainder of the borehole will be grouted.
• (3x) Installation of 2" monitoring wells, 75' bis with ten feet of 0.01-slot screen. Sand pack and a fine-grained sand seal will be installed to above the screen and the remainder of the borehole will be grouted.
• (lx) Installation of 2" monitoring well, 285' bis with five feet of 0.01-slot screen. Sand pack and a fine-grained sand seal will be installed to above the screen and the remainder of the borehole will be grouted.
Former Metro Cleaners (3054 West New Haven Avenue, Melbourne, FL)
• (lx) Lithologic boring: continuous soil core via DPT to approximately 45' bis or until refusal. Grouted up upon completion.
• (Gx) Vapor Extraction Wells: 2" PVC to a depth of approximately 5 feet with a minimum borehole diameter of 6".
2.5' of screen (can be cut from a longer section) and capped with a slip cap using stainless steel screws. A sand pack will be installed to a depth of 2' bis and then grouted to surface. One location may be inside the facility and access for a rig may be a problem.
• (7x) Groundwater profiling locations: water samples collected from a screen point at water level (approx. 5 ft bis) via DPT rig and at various lowest points, possibly up to 40' bis. Locations may require coring through concrete or asphalt. All locations must be repaired with like material following proper abandonment.
• (4x) Abandonment of monitoring wells (2" to approximately 12' bis assumed) via tremie grouting including removal of manhole.
• (4x) Installation of 2" shallow monitoring wells, 12' bis with ten feet of 0.01-slot screen. Sand pack and a fine grained sand seal will be installed to above the screen and the remainder of the borehole will be grouted.
• (lx) Installation of 2" deep monitoring well, 40' bis with five feet of 0.01-slot screen. Sand pack and a fine grained sand seal will be installed to above the screen and the remainder of the borehole will be grouted.
Elite Cleaners (11301 North 56th Street, Temple Terrace, FL)
• (lx) Lithologic boring: continuous soil core via DPT to approximately 50' bis or until refusal. Grouted up upon completion.
• (7x) Vapor Extraction Wells: 2" PVC to a maximum depth of approximately 10 feet with a minimum borehole diameter of 6". 2.5' of screen (can be cut from a longer section) and capped with a slip cap using stainless steel screws. A sand pack will be installed to a depth of 2' bis and then grouted to surface. One location may be inside the facility and access for a rig may be a problem.
• (9x) Groundwater profiling locations: water samples collected from a screen point at water level (approx. 5-10 ft bis) via DPT rig and at various lowest points, possibly up to 50' bis. Locations may require coring through concrete or asphalt. All locations must be repaired with like material following proper abandonment.
• (5x) Installation of 2" shallow monitoring wells, 15' bis with ten feet of 0.01-slot screen. Sand pack and a fine grained sand seal will be installed to above the screen and the remainder of the borehole will be grouted.
• (lx) Installation of 2" deep monitoring well, up to 50' bis with five feet of 0.01-slot screen. Sand pack and a fine grained sand seal will be installed to above the screen and the remainder of the borehole will be grouted.
The subcontractor should assume that a water supply may not be available at the sites. The subcontractor will be required to obtain any permits required for the drilling services. All soil cuttings and groundwater from development will be containerized in drums and placed in the rear of each site for future disposal by COM Smith. Standard FDEP decontamination requirements apply for each site.

GENERAL TERMSANDCONDITIONS
These standard terms and conditions ("STCS") are incorporated by reference into the foregoing proposal, along with any future modifications or amendments (the "Agreement") between Southeastern Surveying and Mapping Corporation ("SSMC") and its Client ("You" or "Your") for the performance of surveying services ("Services"). These STCS are fully binding upon you just as if they were fully outlined in the body of the proposal letter and shall supersede any term or provision elsewhere in the agreement in conflict with these
STCS.
SCOPE OF SERVICES
For the fee outlined in the Agreement, you agree that SSMC shall only be obligated to render the Services expressly described in the Agreement. Unless the Agreement explicitly requires, in no event does SSMC has any obligation or responsibility for:
a. The correctness and completeness of any document which was prepared by another entity.
b. The correctness and completeness of any drawing prepared by SSMC, unless it was duly signed and sealed by a registered professional on SSMC's behalf.
c. Favorable or timely comment or action by any governmental entity on the submission of any construction documents, land use or feasibility studies, appeals, petitions for exceptions or waivers, or other requests or documents of any nature whatsoever.
d. Taking into account off-site circumstances other than those clearly visible and actually known to SSMC from on-site work.
e. The actual location (or characteristics) of any portion of a utility that is not entirely visible from the surface.
f. Site safety or construction quality, means, methods, or sequences.
g. The correctness of any geotechnical services performed by others, whether or not performed as SSMC's subcontractor.
Should shop drawing review be incorporated into the Services, SSMC shall pass on the shop drawings with reasonable promptness. Checking and approval of shop drawings will be general, for conformance with the design concept of the project to which this Agreement relates ("Project") and compliance with the information given in the construction documents, and will not include quantities, detailed dimensions, nor adjustments of dimensions to actual field conditions. Approval shall not be construed as permitting any departure from contract requirements, nor as relieving the Contractor of the sole and final responsibility for any error in details, dimensions, or otherwise, that may exist. SSMC does not provide legal, accounting, or insurance services.
YOUR ORAL DECISIONS
You, or any of your directors, officers, partners, members, managers, employees or agents having apparent authority from you, may orally: (a) make decisions relating to Services or the Agreement; (b) request a change in the scope of Services under the Agreement; or (c) request SSMC to render additional services under the Agreement, subject to our right to require you to submit the request in writing before your decision or request shall be considered to have been effectively made. You may, at any time, limit the authority of any or all persons to act orally on your behalf under this Paragraph, by giving SSMC seven (7) days advance written notice.

PAYMENT
SSMC may submit invoices at any time to you for Services and reimbursable expenses incurred. Invoices are payable within 30 days of the invoice date. Invoices may be based either upon our estimate of the proportion of the total services completed at the time of billing for lump sum or fixed fee services, or in the case of hourly services, upon rendering of the Services. If any invoice is not paid within 30 days of the invoice date, SSMC shall have the right either to suspend the performance of our Services until all invoices more than 30 days past due are fully paid or to terminate the agreement and to initiate proceedings to recover amounts owed by you. Additionally, SSMC shall have the right to withhold from you the possession or use of any drawings or documents prepared by SSMC for you under this or any other agreement with you until all delinquent invoices are paid in full.
You shall not offset payments of our invoices by any amounts due or claimed to be due for any reason.
If you do not give SSMC written notice disputing an invoice within 20 days of the invoice date, the invoice shall conclusively be deemed correct. All payments made by you should specify the invoice numbers being paid. If SSMC receives payments that do not specify the invoices being paid, you agree that SSMC may apply payments in our sole discretion.
Time is of the essence of your payment obligations, and your failure make full and timely payment shall be deemed a material breach.
PROPRIETARY RIGHTS
The drawings, specifications and other documents prepared by SSMC under this Agreement are instruments of SSMC's service for use solely for the Project and, unless otherwise provided, SSMC shall be deemed the author of these documents and shall retain all common law, statutory, and other reserved rights, including the copyright and rights to any SSMC trademarks. You shall be permitted to retain copies, including reproducible copies of SSMC's instruments of service for information and reference for the Project. SSMC's drawings, specifications, or other documents shall not be used by you or others on other projects for any reason or for completion of this Project by other professionals unless you enter into a written agreement with SSMC allowing for such use. ASSIGNMENT
Neither party shall assign or transfer any rights, interests or claims arising under this Agreement without the written consent of the other. This Agreement shall not confer any benefit or right upon any person or entity other than you, SSMC, and its officers, employees, agents, and subcontractors. SSMC's officers, employees, agents, and subcontractors shall have and shall be entitled to the protections afforded SSMC under this Agreement.
