Negotiation Summary Task Assignment DC005C Former Bay’s Cleaners

ERIC_4918

SSAR

CDM Smith Inc. (CDM) will conduct Supplemental Site Assessment. Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2: Access Agreements/Right-of-Way Permitting, Utility Locate, Updated Work Plan One soil boring will be conducted on-site, and two soil borings will be off-site. Six proposed groundwater monitoring wells are to be installed at off-site locations. Off-site intrusive work will not be conducted until access is obtained. Costs in this proposal include both off-site access agreements and Right-of-Way (ROW) permitting provisions, but both may not be required. CDM Smith will subcontract a utility location services company to provide a complete survey of the utilities at each proposed drilling location and provide oversight for the Utility Survey. In addition, CDM Smith will prepare a Health and Safety Plan (HASP) prior to field work efforts and update the SAWP.

Subtask 3: Confirmatory MAGs Sampling MAGS™ testing will be completed at the existing wells installed during March 2020 by Cardno.

The previous consultant did not complete the required testing due to perched groundwater in four of the six wells. Due to the vertical thickness of the vadose zone, MAGS™ wells were installed at two discrete depth intervals. M-1, M-2, and M-3 are completed to 20 ft bls and M-4, M-5, and M-6 are completed to 40 ft bls. Each 2 inch polyvinyl chloride (PVC) well was completed with 19 ft of 0.01-inch slotted screen. Upon arrival to begin testing by CDM Smith and if perched groundwater is present, each well containing water will be backfilled with a 30/65 sand seal to the appropriate depth. Testing of the existing wells will be conducted by completing an initial step test at one well followed by MAGS™ testing at all wells. Individual step tests will be completed on M-1 and M-5.

During MAGS™ testing, soil vapors will be extracted and measured with a photoionization detector. In addition, the vapor extraction rate and vacuum levels at all observation points will be recorded. Soil vapor samples will be collected as an instantaneous grab sample at 1 pore volume at each well. The vapor samples will be analyzed for VOCs by EPA Method TO-15.

Subtask 4: Supplemental Site Intrusive Investigation, Groundwater Sampling, and Supplemental Site Assessment Report A total of three soil borings will be completed to delineate impacted soil at the location of the former dry-cleaning building. The soil borings will be advanced using sonic 4-inch core drilling to the depth of groundwater anticipated to be no more than 45 feet (ft) below land surface (bls). The soil samples will be analyzed for full volatile organic compounds (VOCs) by Environmental Protection Agency (EPA) Method 8260. Split samples will also be collected using airtight EnCore® Samplers for contingency Synthetic Precipitation Leaching Procedure (SPLP) analyses.

As discussed above and in the March 2023 SSAR, six monitoring wells were not installed due to offsite property owners not approving access. A total of three shallow zone monitoring wells, MW- 35, MW-36, and MW-37 (55 ft to 60 ft each) and three intermediate zone wells, MW-39, MW-40, and MW-42 (75 ft each) will be installed to delineate groundwater surrounding the Site after off-site access agreements or ROW permits are secured. Sonic drilling will be utilized to complete well installation activities. Each well will be constructed within a 6-inch borehole using Schedule 40 2-inch PVC casing and screens to the appropriate depth. The screens will be 10 ft in length and 0.01-inch slotted. Continuous 4-inch cores will be collected for lithology logging purposes by a CDM Smith geologist. Following installation, each monitoring well will be developed in accordance with applicable FDEP standard operating procedures (SOP) with a decontaminated stainless steel submersible pump, until the groundwater appears clear and free of silts. As documented in the SSAR, existing wells (i.e., MW-1 through MW-34, MW-38 and MW-41) were last sampled in September 2022 (except for MW-32 which was dry). CDM Smith will complete a comprehensive water level measurement and sampling event of the existing wells and the newly installed wells to document contaminant concentrations and plume stability. During purging, field measurements of pH, specific conductance, temperature, dissolved oxygen, and turbidity will be recorded in accordance with FDEP SOPs FT 1100, FT 1200, FT 1400, FT 1500, FT 1600, respectively. In addition, oxidation reduction potential measurement will be recorded during well purging and the water will be observed for evidence of sheen and odors. The groundwater samples will be analyzed by United States Environmental Protection Agency Method 8260 for volatile organic halocarbons. The SSAR will be prepared pursuant 62-780.600, F.A.C. The report will include professional certification, tabulations of current and historical field measured parameters, groundwater analytical results, water level elevations, field notes, a conceptual site model, applicable figures, permits, soil boring/monitor well construction logs, a summary of the well installation and groundwater sampling, any data gaps identified, conclusions, and recommendations. CDM Smith will also prepare a detailed cross section and Conceptual Site Model as part of this SSAR.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis.

CDM and the FDEP confirmed on June 7th, 2023, that an Access Agreement with the current real property owner of the site has been executed.

The final cost total for Task Assignment DC005C is $166,611.42.

Subject: Supplemental Site Assessment Proposal Former Bay’s Cleaners

ERIC_4918

214 North Magnolia Avenue Ocala, Marion County, Florida

Dear Mr. McMurray:

CDM Smith Inc. (CDM Smith) is pleased to respond to the Florida Department of Environmental Protection (FDEP) Task Assignment Request to provide supplemental site assessment services for the Former Bay’s Cleaners site under Contract Number HW681. The scope of work, the project schedule is described below, and the cost estimate form is also attached in Table 1.

CDM Smith submitted a Site Assessment Work Plan (SAWP) to the FDEP on June 24, 2022 which was approved. CDM Smith conducted environmental assessment activities at the Site during September 2022 through January 2023 and the results were documented in a Supplemental Site Assessment Report (SSAR) that was submitted to the FDEP on March 23, 2023. The SSAR recommended further delineation of the horizontal extent of impacted soil at the location of the former dry cleaner building and the installation of additional off-site groundwater monitoring wells (vertical and horizontal extent) that were not installed as part of the original CDM Smith SAWP due off-site property owners not approving access.

Supplemental Site Assessment Scope of Work The subtasks associated with the proposed scope of work are described below.

Subtask 1 – Project Management Project management activities performed under this task consist of administrative functions required to maintain the project schedule, adhere to budget, and deliver the quality of the work products defined by CDM Smith's quality standards. CDM Smith will prepare and submit invoices upon approval of each deliverable or milestone completion.

Mr. Kyle McMurry

Project Kickoff and Progress Meetings CDM Smith expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.

Project Quality Technical Review CDM Smith's Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.

Project Status Reports CDM Smith's Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project. The Project Manager will also discuss the status of the project with the FDEP on a bi-weekly basis until the project is complete.

Subtask 2 – Access Agreements/Right-of-Way Permitting, Private Utility Locate, Updated Work Plan One soil boring will be conducted on-site and two soil borings will be off-site. Six proposed groundwater monitoring wells are to be installed at off-site locations. Off-site intrusive work will not be conducted until access is obtained. Costs in this proposal include both off-site access agreements and Right-of-Way (ROW) permitting provisions, but both may not be required. Any level of effort adjustments will be reconciled via a task assignment change order.

The off-site access will attempt to be secured by the following steps:

CDM Smith personnel will conduct a site visit up to two times to meet with the site owners and/or tenant to get a signed off-site access agreement. If the access agreement is not signed during the first two times in person, CDM Smith send access request letters to each applicable property owner via certified US Mail or Federal Express so receipt of the letter can be verified and placed in the project files. Each letter will include the standard FDEP language, Non- Source permission to enter property forms for signature, and a proposed map that will show the location of the proposed groundwater monitoring well(s).

If these three attempts are unsuccessful, the FDEP will be notified and CDM Smith will relocate the proposed wells to the closest ROW location. CDM Smith will then submit the necessary ROW permit application/maintenance of traffic plan in order to complete proposed groundwater monitoring well (s). This process may take 1-2 months to complete.

The statewide utility location service (Sunshine 811) will be notified a minimum of 48 hours in advance of any subsurface intrusion work to identify and mark the location of public underground utilities.

Deliverables: Access Agreements or ROW Permits, Utility Locate, and paid invoices. Updated

SAWP

Subtask 3 – Confirmatory MAGS sampling The fieldwork will consist of the following components. The site location is shown on Figure 1. All proposal figures are located in Appendix A.

MAGS™ Testing

As requested by the FDEP in December 2022, MAGS™ testing will be completed at the existing wells installed during March 2020 by Cardno. The previous consultant did not complete the required testing due to perched groundwater in four of the six wells. CDM Smith assumes these wells are present on-site (Figure 4) and still remain in adequate condition for testing. Due to the vertical thickness of the vadose zone, MAGS™ wells were installed at two discrete depth intervals. M-1, M-2, and M-3 are completed to 20 ft bls and M-4, M-5, and M-6 are completed to 40 ft bls. Each 2-inch polyvinyl chloride (PVC) well was completed with 19 ft of 0.01-inch slotted screen. Upon arrival to begin testing by CDM Smith and if perched groundwater is present, each well containing water will be backfilled with a 30/65 sand seal to the appropriate depth (which can be removed if groundwater levels decline over time).

Testing of the existing wells will be conducted by completing an initial step test at one well followed by MAGS™ testing at all wells. Individual step tests will be completed on M-1 and M-5 and will consist of extracting vapors at the following vacuums.

15 inches of water

30 inches of water

45 inches of water

Maximum achievable

During each individual step, the vacuum flow rate will be recorded, and vacuum measurements will be collected from all MAG wells regardless of depth. The expected total duration of the step test is less than 15 minutes. The field team will then select the appropriate vacuum/flow rate from the step test for MAGS™ testing and estimate the time required to achieve 0 pore volume for vapor sample collection. An air-filled pore space of 15 percent will be assumed to calculate 1 pore volume within the radius of influence established from the step test.

Deliverables: Laboratory analytical COCs, and field documentation.

Subtask 4 – Supplemental Site Intrusive Investigation and Groundwater Sampling Soil Borings

Due to safety issues at this site, two people will be needed all times during fieldwork. A total of three soil borings will be completed in an attempt to delineate impacted soil at the location of the former dry cleaning building. The soil borings will be advanced using sonic 4-inch core drilling to the depth of groundwater anticipated to be no more than 45 feet (ft) below land surface (bls). At onsite location SB-45, soil samples will be collected from 0.5 ft bls, 2 ft bls, at 2-foot intervals thereafter to 10 ft bls, and at 5 ft intervals to 24 ft bls as shown on Figure 2. At SB-43, SB-44, and SB-45, soil samples will be collected beginning at 25 ft bls on five ft intervals to 45 ft bls as shown on Figure 3. SB-43 and SB-44 are off-site locations and will have appropriate ROW permitting as detailed in Subtask 2.

The soil samples will be analyzed for full volatile organic compounds (VOCs) by Environmental Protection Agency (EPA) Method 8260. Split samples will also be collected using airtight EnCore®

Samplers for contingency Synthetic Precipitation Leaching Procedure (SPLP) analyses. The SPLP extraction and analyses will be completed for VOC samples that exceed the default leachability Soil Cleanup Target Level (SCTL) and are below the residential direct contact SCTL. Following completion of the soil sampling effort, each boring will be abandoned using a cement/bentonite grout (bottom to top) via a tremie pipe.

Monitoring Well Installation

As discussed above and in the March 2023 SSAR, six monitoring wells were not installed due to off-site property owners not approving access. A total of three shallow zone monitoring wells, MW-35, MW-36, and MW-37 (55 ft to 60 ft each) and three intermediate zone wells, MW-39, MW-40, and MW-42 (75 ft each) will be installed in an attempt to delineate groundwater surrounding the Site after off-site access agreements or ROW permits are secured. Potable water used during the drilling and decontamination efforts will be from a FDEP approved water source that is essentially free of per-and polyfluoroalkyl substances compounds The proposed wells locations are shown on Figure

5. The intervals will be adjusted in the field based on the actual depth to groundwater and lithology, if necessary.

Sonic drilling will be utilized to complete well installation activities. Each well will be constructed within a 6-inch borehole using Schedule 40 2-inch PVC casing and screens to the appropriate depth.

The screens will be 10 ft in length and 0.01-inch slotted. All proposed monitoring wells will be completed using a 20/30 silica sand filter pack, followed by a 30/65 fine silica sand seal and neat cement grout, and finished with flush-mounted bolt-down manholes inside 2 ’x 2’x 0.5” concrete pads with a locking caps. The monitoring wells will be surveyed at the completion of Subtask 3.

Groundwater Sampling

As documented in the SSAR, existing wells (i.e., MW-1 through MW-34, MW-38 and MW-41) were last sampled in September 2022 (except for MW-32 which was dry). CDM Smith will complete a comprehensive water level measurement and sampling event of the existing wells and the newly installed wells to document contaminant concentrations and plume stability. The monitoring wells will be sampled in accordance with FDEP SOP FS 2200. Stainless steel submersible pumps and new high-density polyethylene tubing will be used to purge the wells and collect the groundwater samples. Grab groundwater samples will be collected after purge parameters stabilized. Copies of equipment calibration forms and field sample logs will be maintained.

The groundwater samples will be placed into laboratory supplied containers, preserved as necessary, and transported under proper chain of custody procedures to a NELAC-approved laboratory for analysis. The groundwater samples will be analyzed by United States Environmental Protection Agency Method 8260 for volatile organic halocarbons.

Investigative-Derived Waste

Investigation -derived waste consisting of drill cuttings and groundwater will be placed in 55-gallon, Florida Department of Transportation approved drums for disposal pending laboratory analysis. The drums will be temporary staged at the former dry cleaning building (currently an empty lot). All waste soil cuttings, well development water, and well purge water will be disposed off-site at an appropriate waste facility by the selected subcontractor.

Deliverables: Laboratory analytical results, and field documentation, boring logs, monitoring well completion logs, survey results, IDW manifests, data tables, and paid invoices to subcontractors

Subtask 4 – Supplemental Site Assessment Report The SSAR will be prepared pursuant 62-780.600, F.A.C.

Deliverable: Supplemental Site Assessment Report

Subcontractors Subcontracted services are required for drilling, surveying, utility location, and IDW disposal. The bidding, results, and subcontractor selections are described below, and Attachment B includes a summary of the subcontractor cost comparisons.

Drilling Request for Quotations were sent to three pre-qualified drilling companies for all drilling services.

The lowest cost bid was from Groundwater Protection. The quotes are included in Attachment B.

Surveying Southeastern Surveying and Mapping is a primary subcontractor for HW681, and other bids were not solicited for this project. The quote is included in Attachment B.

Utility Location Bids were obtained from Blood Hound, GPRS Inc., and Site Lines LLC to complete the utility survey.

The lowest cost bid was from Site Lines LLC. The quotes are included in Attachment B.

IDW Disposal Rates for IDW removal and disposal were previously requested for HW681 from Erwin Remediation, EnviroServe (formerly Clark Environmental), and ERS Corp. Erwin Remediation was the lowest cost and has been selected as the IDW disposal subcontractor for this project as shown in Attachment B. Ten drums of soil cuttings and twelve drums of groundwater are estimated in this proposal to complete the investigation.

Assumptions This scope and schedule have been prepared/predicated on unknown site conditions. The scope and schedule may change depending on surveys and the drilling conditions. Drillers and other subcontractors’ availability may be limited and impact the schedule. In addition, this scope does not consider any weather delays. CDM Smith will communicate with the FDEP in a timely manner if schedule delays are encountered and will prepare the necessary paperwork for change orders, if needed.

Schedule CDM Smith’s proposed project schedule is shown below, with targeted completion of the Site Assessment process within four months. The final dates on the schedule depend on Notice to Proceed (NTP) assumed to be 10 days and subcontractor availability. Once CDM Smith receives NTP, the subcontractor’s availability will be confirmed, and the schedule may be adjusted. The deliverables and due dates are summarized in Table 2.
