Subject: Proposal For SVE System Rebound Evaluation
Nelsons Cleaners (ERIC_4822)
1203 West Highway 50
Clermont, Lake County, Florida
Dear Mr. Aoki:
CDM Smith Inc. is pleased to submit this Task Assignment Request for Soil Vapor Extraction (SVE) System
Rebound Evaluation at Nelsons Cleaners (the Site) located at 1203 West Highway 50, Clermont, Lake
County, Florida (ERIC_4822). The scope of work, proposed deliverable, and deliverable schedule are summarized below, and the cost estimate is also attached.
This Task Assignment, when executed, shall be incorporated in, and become part of Contract No. HW681 between the Florida Department of Environmental Protection (Department), and CDM Smith Inc.
(Contractor), dated January 5, 2022, hereafter referred to as the Contract.
On February 2, 2026, Contractor confirmed a Site Access Agreement (SAA) was executed with Markus
Williams with Lou Natem LLC on May 14, 2025, as the real property owner of the Site listed above. The
Lake County property appraiser database indicates the property owner since 2019, is Lou Natem LLC and that the property has not changed ownership since. As a result, Contractor assumes the current SAA remains valid and in effect.
Project Understanding
Contractor understands that the Department requires the Operations and Maintenance (O&M) of the
SVE system that consists of 5 SVE wells (VE01 through VE05), SVE blower, air injection blower, moisture separator, and a granulated activated carbon (GAC) vapor treatment drum. This proposal includes costs associated with 1 month of SVE system O&M, SVE system monitoring and optimization, Rebound
Evaluation, and a Rebound Evaluation Technical Memorandum. The project duration will be approximately 3 months. Contractor understands that the Site is an actively operating facility, and the
Site work must accommodate Site operations.
Scope of Work
The proposed scope of work includes the following elements:
4651 Salisbury Rd., Suite 420, Jacksonville, FL 32256 | tel: 904-731-7109
Subtask 1: Project Management
Project management activities performed under this task consist of those general administrative functions required to maintain the project on schedule, within budget, and that the quality of the work products defined within this scope is consistent with Contractor’s quality standards. Contractor’s project manager will be Kathy Leggoe, PG, PMP. We will prepare and submit invoices upon approval of each deliverable or milestone completion.
Project Progress Meeting and Status Reports
Contractor will hold bi-weekly meetings or telephone conferences with the Department to review the status of the project and identify and resolve outstanding issues. Contractor will prepare meeting agendas and progress reports as part of this task throughout the duration of this project.
Project Resource Management
Contractor will manage, allocate, and coordinate necessary resources for completion of tasks related to
SVE System O&M.
Project Quality Technical Review
Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.
Subtask 2 Rebound Evaluation
Contractor will mobilize to the Site to conduct a Rebound Evaluation. The SVE system will be brought online, and soil vapor samples will be collected from the SVE influent and soil vapor extraction wells
VE01 through VE05. Soil samples will be submitted to Eurofins Environmental Testing (Eurofins) laboratory for analysis by Method TO-15. Once O&M, including the soil vapor sample collection, is complete, the SVE system will be shut down until results can be discussed with the Department.
Following the completion of the rebound evaluation, Contractor will prepare a Rebound Evaluation
Technical Memorandum. The report will include updated figures and data tables, field forms, a copy of the laboratory analytical report and chain of custody, a summary of sampling methodologies, quality assurance/quality control evaluation of data, conclusions, and recommendations.
Subtask 3 – Utilities (Cost Plus Fixed Fee)
Contractor will provide utilities for the SVE system and assume $1,300 per month including administrative costs. This proposal includes costs associated with two months of utilities to account for the Rebound Evaluation.
Assumptions
The current SAA remains valid and in effect.
Preparing a Groundwater Monitoring Report is not included in this Task Assignment.
It is assumed that only two months of utilities will be provided and the total cost including the utilities handling fees ($20 per month) will not exceed a total of $1,320 per month.
