Supplemental Site Assessment
CDM Smith Inc. (CDM) will conduct Supplemental Site Assessment. Specific subtasks are detailed below:
Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.
Subtask 2: Groundwater Sampling, Soil Boring, and Test Pits CDM Smith personnel will collect groundwater samples from 14 monitoring wells located at the site. A round of synoptic water levels will be collected from all wells prior to sampling. Samples taken will measure dissolved oxygen, temperature, pH, oxygen reduction potential, conductivity, turbidity, and water level. All 14 groundwater samples will be analyzed for volatile organic halocarbons.
Approximately 37 soil borings will be advanced. Each soil boring will be cored with a concrete core machine prior to advancing the boring. Each soil boring will be advanced to groundwater with discrete soil samples collected at depths of 1 and 3 feet below land surface, for a total of two lab samples per boring.
Vapor extraction wells (VEWs) for MAGS testing will be installed at the locations of previous soil borings SB-21, SB-37, and SB-46. The VEWs will be installed to a terminal depth of approximately three feet bls, or one foot above the water table. The VEWs will be constructed using two feet of 2-inch diameter, 0.010-inch factory-slotted polyvinyl chloride (PVC) well screen, coupled with one foot of solid PVC riser. 20/30 graded silica sand will be placed from the bottom of the borehole to approximately 0.5 feet above the well screen, followed by 0.5 feet of neat cement grout. The VEWs will be completed within an 8-inch bolt-down flush-mount manhole.
Up to six of the proposed soil boring locations will be converted to vapor monitoring points to facilitate collection of vacuum measurements during the proposed MAGs testing activities. The VMPs will be 0.75-inch diameter PVC and installed to a terminal depth of three feet bls. Annular backfill will be the same as for the VEWs, and the VMPs will be completed with flush-mount covers. The locations of the VMPs will be determined in the field.
Geotechnical samples will be collected at select soil boring locations for engineering properties based on field observations of native soils. Three samples will be collected for sieve analysis via ASTM Method D6913 and D7928 (includes percent fines), and one sample for organic content and Atterberg limits via methods ASTM D2974 and ASTM D4318, respectively.
Two test pits will be excavated alongside the building to determine the foundation type and depth, and a preliminary structural assessment of the building will be performed to understand the building load.
Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Subcontractors for IDW disposal were chosen on a lowest-cost basis.
Subtask 3: Supplemental Site Assessment Report A Supplemental Site Assessment Report (SSAR) will be prepared to summarize the field investigation, the laboratory results and present findings and recommendations from the investigation, address data gaps, and other applicable information that will define requirements for the anticipated ISR. CDM and the FDEP confirmed on February 28, 2023, that an Access Agreement with the current real property owner of the site has been executed. Additionally, a geotechnical and structural engineer will review the data and make preliminary recommendations on safety/excavation considerations for later inclusion into the ISR plan.
CDM and the FDEP confirmed on March 8th, 2023, that an Access Agreement with the current real property owner of the site has been executed.
This work will be conducted in line with the scope of work included with CDM’s proposal, received September 15th, 2022. Documentation of the costs is included in the attached CEF, Level-of- Effort sheets, and proposal letter. The final deliverable due date is October 30th, 2023. And the completion date for the task assignment is December 20th, 2023.
The final cost total for Task Assignment HW003B is $103,860.72.

Dear Mr. McMurray:
CDM Smith Inc. (CDM Smith) is pleased to submit this Task Assignment Request for a
Supplemental Site Assessment of the First Coast Cleaners (site) located at 610 North 3rd Street, Jacksonville Beach, Florida. The site is identified by Florida Department of Environmental
Protection (FDEP) Facility ID ERIC_4535. The scope of work, proposed deliverables and deliverable schedule are summarized below to be completed in accordance with the Master
Agreement, FDEP Contract No. HW681 dated January 5, 2022.
Project Background
The former First Coast Cleaners site was located at 610 North 3rd Street in Jacksonville Beach, Florida. Prior to 1969, the facility only operated as a conventional laundry business. The site operated as a dry-cleaning facility from 1969 to 2020. Assessment activities at the site began in 2001 and limited remedial success has since been attained to approach site closure. The current site owner has indicated that as part of the planned redevelopment of the site, the current building could be demolished, or an interior excavation otherwise allowed. This would allow for excavation activities to occur at the site beneath the current building structure.
In February 2022, CDM Smith conducted site reconnaissance activity and developed a work plan based upon current site conditions. The existing soil vapor extraction (SVE) infrastructure was damaged and not in an operable condition. Additionally, the site dry-cleaning operation and equipment has been removed. The owner indicates that they are amenable to a more aggressive source removal approach as the site soils are now easily accessible. Based on correspondence between FDEP and CDM Smith, additional site assessment data is needed to prepare for an
Interim Source Removal Proposal (ISRP) and streamline the path to closure.
A Supplemental Site Assessment Workplan (SSAWP) dated March 1, 2022, was prepared by CDM Smith and submitted to FDEP. On April 7, 2022, FDEP approved the workplan with additional clarifications including that Synthetic Precipitation Leaching Procedure (SPLP) analysis will be done on all soils with concentrations between the residential and default
Mr. McMurray
FCC Proposal Final March 2023 leachability Cleanup Target Levels (CTLs). This task request is to implement the approved
SSAWP. Initially, drilling was anticipated to collect soil data. Based on the limited availability of subcontractors and scope of work, CDM Smith revised the SSAWP approach presented herein to allow for CDM Smith to self-perform to most of the field work in order to expedite project execution.
In late 2022 and early 2023, CDM Smith revised the technical approach several times under the direction of FDEP through a series of emails and conference calls. This scope reflects changes required by DEP.
Scope of Work
The proposed scope of work includes the following elements:
Subtask 1: Project Management
Project management activities performed under this task consist of those generally administrative functions required to maintain the project on schedule, within budget, and that the quality of the work products defined within this scope is consistent with CDM Smith’s quality standards. Contractor’s project manager will be Clint Noble, PG, PMP. We will prepare and submit invoices upon approval of each deliverable or milestone completion.
Project Kickoff and Progress Meetings
Contractor expects to have a kickoff meeting or telephone conference with FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.
Project Resource Management
Contractor will manage, allocate, and coordinate necessary resources for completion of tasks related to the proposed field activities.
Project Quality Technical Review
Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on assessment findings and reports.
Project Status Reports
Contractor’s Project Manager will prepare status reports to accompany each invoice throughout the duration of this project. The Project Manager will also discuss the status of the project with FDEP on a bi-weekly basis until the project is complete.
Subtask 2: Supplemental Site Assessment Field Work
The general scope of work is outlined in the SSAWP dated March 1, 2022, approved with comment by FDEP on April 7, 2022. The additional dataset required to proceed with remedial planning are:
1. Sampling of 14 monitoring wells for contaminants of concern and geochemical characterization parameters to establish baseline conditions.
2. Installation of shallow soil borings to characterize post-remediation concentrations of contaminants. Collection of select samples for geotechnical parameters to support excavation design.
3. Conduct two test pits to evaluate foundation conditions.
4. Install vapor extraction wells and conduct MAGS testing.
5. Evaluate geotechnical data, including a review by a Senior Geotechnical Engineer and
Structural Engineer for recommendations.
6. Conduct a teleconference with the FDEP in order to review data summaries prior to preparation of final SSA Report and necessary revisions.
Subtask 2 results will be used to define the Subtask 3 deliverable outline.
Additional details for each step of Subtask 2 are provided below:
2.1 - Groundwater Sampling. CDM Smith personnel will collect groundwater samples from 14 monitoring wells located at the site. A round of synoptic water levels will be collected from all wells prior to sampling. Samples will be collected via low flow method using a peristaltic pump and ¼ inch outer diameter high density polyethylene tubing; a YSI water quality multimeter measuring dissolved oxygen, temperature, pH, oxygen reduction potential, and conductivity; a stand-alone turbidity meter; and a water level indicator. All 14 groundwater samples will be analyzed for volatile organic halocarbons (VOHs) by United States Environmental Protection
Agency (EPA) Method 8260. At three monitoring well locations, the groundwater samples will also be submitted for analysis by EPA Method 9060 for total organic carbon and dissolved gasses (methane, ethane, and ethene). To establish baseline Underground Injection Control conditions, the groundwater samples from five of the monitoring wells will be submitted for analysis by laboratory method Florida Petroleum Residual Organic (FL-PRO) for total recoverable petroleum hydrocarbons. This UIC parameter is consistent with the application
EOSPRO. A groundwater sampling schedule is provided as Table 1. Monitor well locations are shown on attached Figure 3. All purge water will be containerized in 55-gallon drums and staged on site for pickup and disposal.
2.2 - Soil Borings and Test Pits. Prior to invasive work, a private utility locator will be used to locate utilities on the three parcels associated with this work using EM, ER, and GPR techniques. Utilities will be attempted to be located inside the former First Coast Cleaners building. CDM Smith will escort the locators and develop a map.
Approximately 37 soil borings will be advanced using a stainless-steel hand auger, a core drill, and saw cutter equipped with a generator. Each soil boring will be cored with a concrete core machine prior to advancing the boring. Each soil boring will be advanced to groundwater with discrete soil samples collected at depths of 1 and 3 feet below land surface, for a total of two lab samples per boring. The proposed soil boring locations are shown on Figures 2A and 2B. The samples will be analyzed for VOHs by EPA Method
8260. Pending laboratory results, and discussion with the FDEP, select soil samples with a concentration exceeding the default leachability will be submitted for SPLP analysis for
VOHs. The concrete slab and asphalt parking area will be repaired with concrete and asphalt patch, respectively. A saw cutter will be used to facilitate the clean patching of boreholes where appropriate. A subcontractor with expertise in pavement coring will be assist with the soil borings. A drilling subcontractor will be used to perform the concrete coring activities to facilitate the soil borings. CDM Smith estimates approximately four days will be required to complete the soil boring activities, as delays are expected due to this being an active retail facility.
Vapor extraction wells (VEWs) for MAGS testing will be installed at the locations of previous soil borings SB-21, SB-37, and SB-46, as shown on Figure 2A. The VEWs will be installed to a terminal depth of approximately three feet bls, or one foot above the water table. The actual installation depth may be adjusted based on where groundwater is encountered. The VEWs will be constructed using two feet of 2-inch diameter, 0.010-inch factory-slotted polyvinyl chloride (PVC) well screen, coupled with one foot of solid PVC riser. Each of the VEWs will be installed using a hand bucket auger. 20/30 graded silica sand will be placed from the bottom of the borehole to approximately 0.5 feet above the well screen, followed by 0.5 feet of neat cement grout. The VEWs will be completed within an 8-inch bolt-down flush-mount manhole. CDM Smith estimates that one day will be required to complete the installation of the three MAGS wells. Soil Samples will be collected during the installation of each VEW at 1 and 3 feet bls. The samples will be analyzed for VOHs by EPA Method 8260.
CDM Smith estimates that two days will be required to complete the installation of six
VMPs.
MAGS testing will be performed for each VEW in general accordance with the Modified
Active Gas Sampling Manual dated October 12, 2004, prepared by HSA Engineers &
Scientists. More specifically:
• To begin the MAGS testing, a step test will be performed at VEW001, where a vacuum of approximately 15 inches of water (“H2O) will be applied. Vacuum and flow rate readings will be recorded, along with vacuum readings at select monitor wells and designated vacuum monitoring points. Influent PID readings will also be recorded. The vacuum will be increased to approximately 30 “H2O, where additional data like that specified above will be recorded. The final step will be to increase the vacuum to approximately 45 “H2O, with the final set of data recorded.
Actual vacuums applied during each step may vary based on site conditions. Each step is expected take 15 minutes, for a total of 45 minutes for the step test.
• Using the optimum flow rate and vacuum determined from the step test, vapor samples will be collected at each VEW. The vapor samples will be collected using a
6-liter Summa canister. A purge time will be calculated based on the flow rate and estimated radius of influence, to ensure the collection of steady-state data. Vacuum and flow rate readings will be recorded, along with vacuum readings at select monitor wells and designated vacuum monitoring points, at 15-minute intervals for a total duration of one hour. Influent PID readings will also be recorded.
CDM Smith assumes that setup, testing, and breakdown for all tests will require one day. A total of three soil gas samples will be submitted to the laboratory for TO-15 analyses with standard reporting times.
Geotechnical samples will be collected at select soil boring locations for engineering properties based on field observations of native soils. Three samples will be collected for sieve analysis via ASTM Method D6913 and D7928 (includes percent fines), and one sample for organic content and Atterberg limits via methods ASTM D2974 and ASTM
D4318, respectively. The samples will be delivered to CDM Smith’s geotechnical laboratory in Orlando, Florida. CDM Smith geotechnical lab rates are provided in the attachments. All drill cuttings will be containerized in 55-gallon drums for later disposal.
All soil boring locations will be surveyed using a GPS.
Test Pits. Two test pits will be excavated alongside the building to determine the foundation type and depth, and a preliminary structural assessment of the building will be performed to understand the building load. The test pits will be two feet long, one foot wide, and two feet deep with one test pit being completed per day. These tasks will be performed under the direction and/or supervision of a CDM Smith Senior Geotechnical
Engineer and a Structural Engineer. Soils will be evaluated for structural properties and foundation conditions photographed. The pits will be backfilled, compacted and pavement patched. A subcontractor (Hepaco) will be used to dig the pits, to assist with the installation of soil borings through pavement, and to repair asphalt and interior soil cores in pavement.
A cursory geotechnical and structural review of the soil data will be completed. The data gathered in this Subtask should be sufficient to define the lateral and vertical limits of the excavation and coupled with the engineering properties. The review will result in excavation safety recommendations for the anticipated ISR or the request for additional data to define engineering safety requirements.
A teleconference with the FDEP will be conducted to discuss the field investigation and define data quality objectives, and potential data gaps as it relates to site assessment, anticipated
ISR actions, and excavation/risk management options associated with the site. The call will outline the content of Subtask 3. CDM Smith will provide draft tables and figures in advance of this conference call for FDEP review.
Deliverables: Laboratory analytical results, and field documentation, data tables, and paid invoices to subcontractors
Subtask 3: Supplemental Site Assessment Report with Geotechnical/Structural Evaluation
A Supplemental Site Assessment Report (SSAR) will be prepared to summarize the field investigation, the laboratory results and present findings and recommendations from the investigation, address data gaps, and other applicable information that will define requirements for the anticipated ISR. The SSAR will include analytical data summaries, site maps depicting soil contamination, groundwater plume map, groundwater flow map(s), estimated quantities for ISR planning, groundwater geochemical conditions for enhanced attenuation, boring logs, and associated appendices typically required of SSARs.
Additionally, a geotechnical and structural engineer will review the data and make preliminary recommendations on safety/excavation considerations for later inclusion into the ISR plan.
