Connect to Municipal Sewer Manatee Springs State Park – Change Order 1

Upon completion of Subtask 1: Site Investigation, it was determined by members of Hydra and the FDEP Project Manager that new systems at the four sites will be necessary. It was determined onsite that the previously listed “#12 Hickory Dump Station” site location is not a location in which the Park has requested updating. The proposed sites for new septic systems are as follows, #1 Ranger Station, #10 APM Residence, #6 Restroom/Concession Stand and #3 Hickory Camping Area. The purpose of this change order is to include possible relocation areas for the new septic systems in the survey boundary and the septic system permitting fees for Levy County Department of Health based on all new system (four) applications.

Subtask 2: Survey (Subtask to be Removed)

Description: O’Neal Surveying & Mapping, Inc. will establish Horizontal and Vertical control based on the Florida State Plane Coordinate System, NAD 83 (2011 Adjustment) NGVD Vertical. A minimum of 2 Benchmarks will be set onsite at all 4 locations in areas where they should not be disturbed by future construction. Topographic Survey for 4 septic tanks and drain field locations extending 10 feet beyond and including all utility lines, electric panels and cleanouts. Locations are shown on the attached Exhibit “A”. Septic and drain field locations to be marked in the field by the client, or representatives thereof, prior to or at the time of the field survey.

Deliverable: Subtask to be removed.

Performance Measure: Subtask to be removed.

Credit Cost: $5,733.29

Subtask 2B: Additional Survey

Description: O’Neal Surveying & Mapping, Inc. will establish Horizontal and Vertical control based on the Florida State Plane Coordinate System, NAD 83 (2011 Adjustment) NGVD Vertical. A minimum of 2 Benchmarks will be set onsite at all 4 locations in areas where they should not be disturbed by future construction. Survey will include all grade breaks, extended to the centerline of adjacent roadways or outside edge of sidewalks, trails, storm structures, signs, above ground utilities, buildings, parking, sidewalks, septic tanks, drain field locations, all above ground visible utility lines, electric panels and cleanouts or any improvements within the limits of survey sufficient to produce 1-foot contours, supplemented with spot shots, as needed.

Tree species that are 6” DBH or greater in size will be located.

Approximate location survey limits are attached in exhibits A and B below.

Deliverable: Consultant will provide electronic copies (AutoCAD and pdf) digitally signed and sealed of the topographic survey.

Performance Measure: Deliverables will be reviewed by the Department to confirm they are complete, accurate, and timely.
