About this file

This is a Past Performance Questionnaire (PPQ) template that the Veterans Health Administration Program Contracting Activity Central (VHA-PCAC) requires offerors to submit as part of their proposal for the EHRM Infrastructure Upgrades Construction project at James A. Haley Veterans Hospital in Tampa, Florida (Solicitation 36C77625R0092). The PPQ will be incorporated into the proposal submission and counts toward the page limitation specified in the CON 100 Instructions to Offerors.

The questionnaire consists of two main sections: Section 1 requires the contractor to provide detailed information about the offeror being evaluated, including company name, address, point of contact information, work performed percentage, prime contractor (if applicable), contract details such as title and number, contract type, location, award and acceptance dates, original and final contract prices, and project scope and relevance. Section 2 requests evaluation information from the client or project references, requiring them to assess the offeror's performance across seven criteria: Quality, Schedule/Timeliness of Performance, Communication, Management/Personnel/Labor, Cost/Financial Management, Subcontract Management, and an overall Summary rating. Evaluators must rate performance using five adjectival levels—Outstanding, Above Average, Satisfactory, Unsatisfactory, or Not Applicable—with corresponding definitions provided. The completed questionnaire must be returned to the requesting firm by the deadline specified in the solicitation, and late submissions will not be accepted or evaluated.
