J-A-15 SAFETY & HEALTH PLAN OUTSTANDING ITEMS CHECKLIST (MAY 2019)

With the submittal of the initial Site Specific Safety and Health Plan (SSSHP), the contractor shall provide a tracking matrix that includes all outstanding submittal items required of the SSSHP. These items may include, but are not limited to, certifications, job hazard analysis’, training, etc. necessary to perform work on the project. The contractor shall submit the required documents as necessary to perform the work and shall submit an updated matrix with each SSSHP document submittal. The tracking matrix shall be kept up to date on the project’s Sharepoint site for review. The matrix can be in the form of a checklist or spreadsheet, but should include the required submittal, the estimated submittal date, the actual submittal date, and a space to indicate review by the government with the reviewer’s name and review date.

J-A-16 REQUEST FOR INFORMATION/CLARIFICATION (RFIC) (JAN 2018)

The Contractor shall promptly report to the Contracting Officer all problems or conflicting technical information encountered during the contract performance so that the Government may provide solutions or appropriate direction. Such problems shall be reported on KSC Form No. 8-268, "Request for Information/Clarification," to be provided by the Government. A copy of each RFIC will be provided to the Contracting Officer’s Representative (COR) concurrently with the transmittal to the Contracting Officer.

The Contractor shall log and control each Request for Information/Clarification (RFI/C), including those generated by subcontractors.

J-A-2117 DEVIATIONS AND WAIVERS (JAN 2018)

(a) When the Contractor proposes to perform work which does not conform to the requirements of the applicable contract drawings and specifications, the Contractor shall submit to the Contracting Officer for approval, a written request for deviation or request for waiver on the nonconforming work.

(b) All requests by the Contractor shall be submitted on KSC Form 50-314 (Contractor Request For

Deviation/Waiver) including an offer of consideration to the Government. The request must be technically supported by justification, rationale, design considerations, calculations and other data which permits ready and conclusive evaluation by the Government as to acceptability or nonacceptability.

(c) Where a requested deviation or waiver on a particular aspect of the work has a relation to, or affects, other aspects of the work, those other aspects of the work shall be clearly identified and referenced.

And, if the requested deviation or waiver necessitates a deviation or waiver on other aspects, requests for all such deviations and waivers must be submitted concurrently.

(d) Any request not submitted in strict accordance with this provision will not be considered.

ATTACHMENT J-A PROJECT DELIVERABLES

(End of Project Deliverable Requirement)

J-A-2218 CONCRETE PLACEMENT INSPECTION AND PLAN (JAN 2018)

(a) All forms, reinforcement and embedded items shall be placed and ready for inspection 24 hours to 48 hours prior to any concrete placement. After Government inspection of forms and embedded items and at least 24 hours before concrete placement the Contractor shall meet with the Contracting Officer's Representative and provide a written plan for concrete placement including the following information:

(1) Volume of concrete to be placed and calculations used in determining volume.

(2) Verification from concrete supplier of volume of concrete to be delivered and delivery date.

(3) Labor breakdown for crew to be used on concrete placement and finishing.

(4) Equipment to be used during placement and finishing.

(b) The Contractor's plan for concrete placement must be approved by the Contracting Officer's Representative prior to placement of concrete. The Government reserves the right to waive the above requirement for placement of less than 3 cubic yards. The Contractor shall place no concrete without an approved concrete placement plan or a written waiver from the Contracting Officer.

ATTACHMENT J-B – KSC PROJECT SPECIFIC REQUIREMENTS

Access to existing facilities for coordination of work shall be provided on an as needed basis only with prior approval from the contracting officer. The Contractor shall notify the contracting officer a minimum of 7 days prior to requiring such access.

J-B-40 NASA EMPLOYEE ACCESS, ENTRANCES AND WALKWAYS (MAR 2014)

The Contractor shall take steps to maintain full and open access to the existing facilities at all times for Government Employees. Construction and demolition work that will impact the exterior of the work area (adjacent walkways, parking, etc.) shall be clearly identified in all adjacent areas that work is taking place above or near this location.

J-B-41 SPECIFIC ISSUES FOR MAINTAINING EGRESS (MAR 2014)

During the course of this contract, egress around the work site must be clearly marked in order to keep employees safely out of the work areas. The Contractor shall manage this by the use of fences, cones and barricades.

J-B-42 WORK AREA ACCESS CONSTRAINTS (MAR 2014)

Access to the work site for pre-construction activities occurring before the construction work window, such as field measurements and field investigations, must be coordinated with the Contracting Officer at least seven working days prior to desired access.

J-B-43 USE OF SHAREPOINT (JUL 2017)

This project will use a government provided SharePoint website for the electronic submittal and repository of most project documentation required by the contract, including, but not limited to, shop drawings, safety plans, environmental plans, demolition plans, certifications, proposals, deviation waivers, pay requests, requests for proposals, and requests for information. In order to accomplish this, the contractor shall designate two administrators, with up to date IT security training, and maintain IT resources that will preserve the integrity of the KSC security firewall. When certification of fulfilling these requirements is received by the government, the government will provide the contractor Cisco VPN software to install on its computers for logging into the SharePoint system and the two administrators will be provided RSA SecurID tokens. The vast majority of documents to be submitted in SharePoint will require the Contractor to use the latest edition of Adobe Acrobat software. Microsoft Explorer will also be required for SharePoint web access.

J-B-44 SITE SPECIFIC SAFETY PLAN (SSSP) Sections Requirements Checklist, Appendix D????

J-B-4544 FIELD COORDINATION

The contractor shall participate in a daily field coordination meeting for LC 39B and provide information regarding planned operations, locations, major material deliveries and other activities relevant for the overall performance of the work in joint occupancy conditions. A detailed three-day look ahead information shall be provided for the following three days. Field work interference with other contractors working in the same areas will be resolved real time at the daily meeting to eliminate conflicts. The contractor shall also provide general activity information for the next two weeks for planning purposes. All contractor operations requiring support from KSC, such as outages or fire and safety standbys for contractor operations shall be identified.

Additional work activities will be on-going during the period of performance in the same location as this work activity. These work activities may impede personnel and vehicle access to the site. As such, in addition to the field coordination meeting, the contractor shall coordinate daily with other contractors in the work area to eliminate any disruptions to all work activities in the area. If a work activity must be delayed due to coordination, contact the Contracting Officer or their representative to identify the delay.

J-B-4645 RESTORING SURFACES EXPOSED TO LAUNCH RESIDUE

Launch of SLS Artemis-1 will deposit launch residue at LC 39B. The launch residue is not hazardous if not ingested or particulated/vaporized by removal processes and inhaled. It is probable that launch residue will be present on trailers, equipment, stored materials, and partially constructed structures or systems belonging to the contractor after a launch. The contractor is responsible for safe cleaning and resurfacing of any items stored or partially constructed exposed to launch residue in order for them to be in compliance with the cleanliness requirements of contract documentation for their items in their final configurations.

J-B-4746 WEATHER PROTECTION OF EXPOSED SURFACES

Contractor to provide a means of protecting equipment, insulation, etc. or any other surface that normally is not exposed to the elements during construction activities that may expose these surfaces.

J-B-4847 CONSTRUCTABILITY REVIEW

The Contractor is to perform a constructability review of the existing design. The review is intended to identify any existing issues including but not limited to identifying errors and clashes, proposing alternative means for fabrication and/or installation, identifying phasing options for the identified work scope and identifying constructability issues within the design in an attempt to reduce the construction schedule as much as possible. The review is anticipated to be completed two weeks after award. Following the review period, the Contractor is to present their findings and proposed modifications. In addition, the Contractor may submit any value engineering change proposals (VECPs) that may arise from the constructability review to the Government for consideration pursuant to FAR clause 52.248-3 Value Engineering – Construction.

Any cost sharing from accepted VECPs will be provided in a subsequent contract modification to incorporate all of the approved changes.

J-B-4948 MEI INTEGRATOR

Mechanical, Electrical, Instrumentation/Control (MEI) Integrator – Contractor shall provide a trade integrator to ensure all components will successfully fit in and are properly configured as required when constructed in accordance with the contract documents. The MEI integrator shall be responsible for the coordination of trades, ensuring that all components are installed in the proper sequence and will construct a complete ECS system as depicted in the contract documents. The integrator shall provide quality review of all shop drawings including the submitted product data, fabrication, installation and coordination drawings focused in the specific issue of coordination of trades, sequence of construction and configuration of the equipment.

J-B-5049 ADDITIONAL ACCESS RESTRAINTS TO THE LC39B WORK SITE

LAUNCH COMPLEX 39B ACCESS RESTRICTIONS AND ADDITIONAL DOWNTIME

CONSTRAINTS

During Space Launch System (SLS) Artemis-1 processing and launch operations the government will restrict all activity on or near the LC-39B Pad. The restrictions are necessary to avoid interference with SLS processing operations. The government will provide as much advance notice as possible regarding these access restrictions. It is the responsibility of the contractor to plan accordingly for access restrictions.

Resulting from the above processing and launch activities, and in addition to the 20 days of downtime specified in Article F.3, Downtime and Excavation Holds, the contractor shall specifically account in his base bid for one (1) additional downtime period expected to total between 15 and 30 days in which all activity on-site at LC-39B will be prohibited. The contractor shall plan to allow work off-site on those days to continue with other activities which do not require access to the LC-39B area. The downtime window is expected to occur between March 15, 2021, and October 1, 2021.

DOWNTIME WINDOWS AND DEMOBILIZATION FROM WORKSITE

During the downtime window, the contractor shall be required to partially demobilize and prepare the work sites and staging areas in preparation for launch from the LC 39B area. The contractor shall account in his base bid for these required demobilizations, mobilizations, and preparations. Materials and equipment may be authorized to remain on-site on a non-interference basis as determined by the Contracting Officer.

Prior to the downtime period, the contractor's work shall include a joint contractor-Government walk down of all work and staging areas within the pad perimeter to determine the required activities in preparation for leaving the work site for an extended period. These activities shall include the removal or securing of materials and equipment that can be displaced by the launch environment. Large pieces of steel, and other materials stored on site shall be tied down to secure them from displacement.

Miscellaneous parts may be placed within a chain link enclosure that completely surrounds the material with a maximum height of 3 feet suitably anchored to prevent displacement. All tools shall be stored at the contractor's storage area or otherwise removed from the work site. If the contractor decides to leave equipment on site such as cranes or lifts; booms and other appurtenances shall be lowered to the ground and anchored to prevent displacement. All trailers and equipment shall be structurally sound and shall be installed and anchored in accordance with KSC-PLN-1904, Trailer/Equipment Tie Down Plan for the John F.
