Subject: Scope of Work and Level of Effort Annual Groundwater Monitoring Former Trophy Cleaners 2790A Gulf to Bay Boulevard Clearwater, Pinellas County, Florida FDEP Site ID No. ERIC_5301
Dear Ms. Plyler, Geosyntec Consultants, Inc. (Geosyntec) has prepared this s Scope of Work (SOW), Cost Estimating Form (CEF), and Level of Effort (LOE) for monitoring well installation and annual groundwater monitoring activities at the Former Trophy Cleaners Site (Site) located in Clearwater, Florida. Assessment activities proposed in this SOW are consistent with recommendations provided in the January 2023 Annual Groundwater Monitoring Report approved by the Florida Department of Environmental Protection on January 12, 2023. A copy of the CEF/LOE is included in Attachment A.
As part of SOW/LOE preparation, access agreements were evaluated to determine (i) what agreements are in place and (ii) that the existing owners associated with each agreement are current. However, at the request of FDEP an updated access agreement for the downgradient, off- Site parcel will be obtained as part of Subtask 2.
Subtask 1 – Project Management
Costs associated with this subtask are directly computed.
Subtask 2 – Monitoring Well Installation
This Subtask includes costs associated with the installation of two downgradient monitoring wells (Figure 1) at an off-Site parcel. The monitoring wells (MW032 and MW033) will be installed using hollow-stem augers to approximately 12 feet (ft) below land surface (BLS). The monitoring wells will be constructed of 10 ft of Schedule 40 polyvinyl chloride (PVC) well screen slotted at 0.010-inches and 2 ft of Schedule 40 PVC riser. The filter pack will consist of 20/30 silica sand placed around the well screen to approximately 0.5-ft above the top of the well screen, followed by a 0.5-ft thick 30/65 fine sand seal and Portland Cement grout to the surface. The monitoring wells will be competed flush-to-grade in 8-inch diameter manholes embedded in 2-ft by 2-ft concrete pads. The monitoring wells will be developed until visually clear using a combination of surging and pumping. The top-of-casing elevation and horizontal coordinates of each monitoring well will be measured by a Florida-licensed professional surveyor as part of Subtask 3.
Geosyntec obtained cost estimates from three drilling subcontractors – Action Environmental (Action), Environmental Drilling Services (EDS), and Preferred Drilling Solutions (PDS). The quotes are provided in Attachment B. Action provided the most competitive bid and has been selected for this work. Action estimated the work will take approximately 1 day to complete.
Prior to conducting monitoring well installation activities, Geosyntec will observe a subsurface survey to identify potential subsurface utilities or obstructions. Geosyntec obtained cost estimates from three utility locating subcontractors – GeoTek Services, LLC (GeoTek), GeoView, Inc., and Site Lines, LLC. The quotes are provided in Attachment C. Site Lines provided the most competitive bid and has been selected for this work.
Subtask 3 – Annual Groundwater Monitoring and Monitoring Well Surveying
Monitoring Well Sampling
This Subtask includes costs associated with annual groundwater monitoring activities tentatively scheduled for November 2023. Geosyntec will mobilize to the Site to collect depth-to-water measurements from 24 monitoring wells in the Site-wide monitoring well network (MW001 through MW004, MW008, MW009, and MW016 through MW031 and new monitoring wells MW032 and MW033), including 13 not proposed for sampling. Measurements will be collected over a 24-hour period to generate groundwater elevation contour maps to evaluate groundwater flow directions. Due to the presence of standing water historically noted at monitoring well MW001 (located in a drainage ditch next to Gulf to Bay Boulevard), we have included effort to remove water from around and within the manhole prior to sampling as part of the DTW measurements.
Groundwater monitoring well samples will be collected from 11 monitoring wells (MW001, MW008, MW018, MW019, MW024, MW026, MW027, MW028, MW031, MW032, and MW033). Groundwater samples will be collected in accordance with FDEP Standard Operating Procedures and submitted for laboratory analysis of volatile organic halocarbons by Environmental Protection Agency Method 8260B.
Following the collection of the monitoring well samples, purge water will be containerized in one 55-gallon drum for transportation off site for disposal as non-hazardous waste. Screening of per-and polyfluoroalkyl substances (PFAS) has been performed at the Site and PFAS were detected in the associated investigation derived waste (IDW) sample. Thus, the drum will be disposed of as
PFAS-containing material. Bid costs prepared by the waste disposal companies are included in Attachment C. Based on the bids, Geosyntec recommends selecting Erwin Remediation to haul and dispose of IDW generated during monitoring activities. We have assumed no oversight is required for drum pickup.
Monitoring Well Survey
This subtask includes costs for a Florida professional land surveyor to survey the horizontal coordinates, top of casing elevations, and ground surface elevations of the two (2) newly installed monitoring wells. Cost has been included for Geosyntec to oversee the survey during the annual groundwater monitoring event. A copy of the surveyor’s quote is included in Attachment D.

RE: Utility Locating Services – Former Trophy Cleaners in Clearwater, Florida
Ms. Cain, GeoTek Services is pleased to respond to your request for Utility Locating services at 2790 Gulf to Bay Boulevard located in Clearwater, Florida. We will scan a 10’ x 10’ area around the two proposed bore locations with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings, or vehicles.
We will scan as close as reasonably possible to fixed objects such as brush, obstructions, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.
We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.
We request access to any as-built site plans of the scan areas prior to rendering services. Our quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Thanks Olivia, cost for the utility markout only (no figure) would be $1,300. If an AutoCAD site map is required to document the study results, the cost would be $1500. A small world, I was at that same site in 2008 doing a utility locating job inside of the Subway and the commercial space to the north, which was unoccupied at the time.
Let me know if you need a formal proposal and appreciate the opportunity, MJW

SITE LINES LLC PROPOSAL
SITE LOCATION
Former Trophy Cleaners Site – 2790 Gulf to Bay Boulevard, Clearwater, FL
PROPOSED SCOPE OF WORK
Site Lines (“Company”) will use Ground Penetrating Radar (GPR) and conventional electromagnetic (EM) pipe and cable locators to determine the location of underground utilities at the site identified by Geosyntec (“Client”).
RESULTS OF THE INVESTIGATION
The estimated depth and location of utilities identified during the investigation will be marked on the ground surface using spray paint or other method to be agreed upon by the parties. The American Public Works Association Uniform Color Codes for the marking of underground utilities will be used.
PROJECT FEE
The fee to complete the project as described within this proposal is: Five Hundred Dollars [$500.00].
Client shall pay Company in full within thirty (30) days of the invoice date, unless the parties have agreed to another payment arrangement in writing. If during the investigation, Company determines that the goals of the project cannot be achieved with the resources available to Company, Company will notify Client as soon as reasonably possible. Under such circumstances, Company will charge Client only for time expended and materials used to date.
CLIENT REQUIREMENTS
Client is responsible for all site access, maintenance of traffic (MOT), site clearing and/or any other logistical considerations necessary to conduct the utility locate.
Client will also provide all pertinent maps of the project site, including those that show the boundaries of the utility locate, pertinent landmarks, and known utilities (if available). Company is not a substitute for Chapter 556, Florida Statutes.
Accordingly, Client is responsible for calling Sunshine State One Call of Florida
(811) before undertaking any invasive activities, whether through Company, any other entity, or on its own.
DESCRIPTION OF COMPANY INVESTIGATION AND LIMITATIONS
A GSSI SIR-4000 GPR system will be used with a 900, 400, or 270 megahertz antenna configuration depending on site conditions and desired targets. The GPR will assist in locating nonmetallic and metallic utilities. Physical surface conditions, subsurface conditions and technical limitations effect the ability of GPR to resolve targeted utilities and/or subsurface structures. Depth and/or diameter of the utility, soil type, moisture/ionic content of the soil, and electrical properties of the soil compared to that of the utility can prevent the GPR from identifying utilities at the site. The diameter of a utility cannot be determined by GPR data. Estimated depths of utilities are directly affected by soil conditions and the material of the utility. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities.
An RD 8000 will be used for the EM survey. Electromagnetic induction which transmits a signal directly into the desired target and passive power detection which locates naturally occurring magnetic fields in energized lines will be identified by detecting and following the signals with a receiver. The EM survey will assist in finding only metallic utilities. The magnetic field and/or induced signal should emanate from the target in a circular geometry. When this occurs, the receiver will receive its peak signal directly above the target. However, when metallic utilities are running near each other, or there is environmental interference from fences, cars or overhead power lines, the signal can become distorted and the peak no longer can be directly over the utility. Care must be taken when doing any invasive work around the utilities. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities. Company’s personnel are highly experienced and certified operators of GPR and EM pipe and cable locators.
Due to the limitations described above, when Client is working around indicated utilities, a minimum of two (2)-feet clearance from either side of the marked utility will be maintained. For absolute confirmation, Client is responsible for exposing any indicated underground utility.
Company will conduct utility investigations in accordance with the level of care and generally accepted professional practices as ordinarily exercised by members of the same profession practicing in the same locality under similar conditions.
REPRESENTATIONS AND WARRANTIES
Neither Company nor its employees or agents are licensed surveyors under Florida law, nor do they represent themselves as such. Accordingly, the work Company is to perform is not a substitute for work typically completed by a licensed surveyor.
Company specifically and expressly disclaims any liability for any type of property damage or personal injury, including but not limited to death, arising from client’s failure to determine that the services of a licensed surveyor are necessary or to retain a licensed surveyor to perform necessary services.
Limitations in technology, complexity of the physical characteristics of the subsurface conditions, and circumstances beyond Company’s control may limit the performance of Company’s utility investigation. For these reasons, Company expresses no guarantee that all utilities and underground structures will be identified and located during this project.
Company is responsible only for locating underground utilities, the presence of which is both known and indicated on site maps to be provided by Client to Company before Company performs the utility locate. Company will use its best efforts to locate both known and unknown utilities within the project boundaries. However, Company will only be responsible for locating utilities, the presence of which is known before Company undertakes its investigation.
Company expressly disclaims any and all warranties regarding specific or desired results.
