Initial Site Assessment
This Task Assignment authorizes CDM Smith Inc. to conduct initial site assessment activities at the subject site in accordance with the November 15, 2022, revised cost proposal. CDM and the DEP confirmed on October 20, 2022, that a Site Access Agreement with the current real property owner of the subject site has been executed.
Specific subtasks are detailed below:
Subtask 1: Project Management Project Management is automatically calculated at 13% of the direct labor.
Subtask 2: Site Survey and Site Preparation Three bids were solicited for utility location services Site Lines LLC was selected as the lowest responsive and responsible bidder. Southeastern Surveying and Mapping was selected to perform the site survey.
Subtasks 3 and 4: Stage I and II Field Investigations CDM Smith will complete ten (10) soil borings using hand augers. Concrete coring will be completed at each of the nine locations. The soil samples will be submitted for laboratory analysis by EPA Method 8260 for VOHs. CDM Smith assumes up to 40 samples will be collected. A duplicate sample will be collected at each interval for SPLP analysis. Those samples to be run for SPLP analysis will be identified based upon the initial analytical data. Costs associated with SPLPs are not included in this cost estimate and will be addressed via work change directive if determined necessary.
One (1) soil boring in the vicinity of the former drycleaning machine will be completed as a 1-inch diameter monitoring well and sampled for VOH analyses by EPA Method 8260. Five (5) of the soil borings will be completed as vapor extraction wells (VEW) for Modified Active Gas Sampling (MAGS™) testing. Two
(2) vapor monitoring points (VMPs) will be installed to assess radius of influence.
One (1) soil gas sample from each VEW will be submitted for TO-15 analysis.
Following MAGS™ testing, vertical groundwater profiling will be conducted at six
(6) locations using a direct-push technology (DPT). Groundwater samples will be collected, and laboratory analyzed for VOCs by EPA Method 8260. In addition, a continuous lithologic boring to 50’ below ground surface or refusal will be installed to obtain site-specific lithologic and stratigraphic information. Upon completion of the field events, the marked utilities and all boring locations will be surveyed.
The monitoring well locations will be finalized based on the Stage I results. Four
(4) monitoring wells will be installed. Prior to well installation, continuous soil cores will be collected to the total depth form each location. The wells will be purged and sampled for VOHs following installation and a full round of water levels will be collected to map the potentiometric surface. The monitoring wells will be surveyed at the completion of installation and investigation derived waste (IDW) will be characterized and transported off-site for disposal. CDM Smith estimates 14 soil drums and 4 water drums of nonhazardous IDW will be generated.
Ambient Technologies Inc. (ATI) provided the lowest original bid, however their bid did not include the required Geoprobe sampling activities cost, equipment decontamination costs, and IDW drum costs. ATI was contacted by CDM Smith to revise their original bid but declined so CDM Smith had to adapt line-item costs from the incomplete submitted bid to the CDM Smith Bid Summary Table to develop a complete bid comparable to that of the other bidders for comparison purposes. Betts Environmental, the second lowest bid, was selected as the lowest responsive and responsible bidder. Erwin Remediation was selected as the lowest responsive and responsible bidder for IDW transport and disposal services.
The final cost total for Task Assignment DC028B is $96,194.58. Documentation of costs is included in the attached Cost Estimate Form (CEF), Level-of-Effort sheets (LOEs), and cost proposal. The final deliverable (Site Assessment Report) will be submitted on or before May 31, 2023. The Task Assignment Completion Date is August 29, 2023.

Subtask 1 – Project Management Project management activities performed under this task generally consist of administrative functions required to maintain the project schedule, adhere to budget, and deliver the quality of the work products defined by CDM Smith's quality standards. We will prepare and submit invoices upon approval of each deliverable or milestone completion.
Project Kickoff and Progress Meetings CDM Smith expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.
Project Quality Technical Review CDM Smith's Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.
Mr. Aaron Cohen, FCCM October 24, 2022
Project Status Reports CDM Smith's Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project. The Project Manager will also discuss the status of the project with the FDEP on a bi-weekly basis until the project is complete.
Subtask 2 – Site Survey Underground Utility Survey The work plan identified only a portion of the underground utilities. CDM Smith will subcontract a utility location services company to provide a complete survey of the utilities and provide oversight for the Utility Survey.
Deliverable: Surveys, paid invoices to surveyors
Subtask 3 – Stage I Field Investigation The Stage I Field Investigation will delineate the extent of volatile organic halocarbons (VOHs) exceeding the Soil Cleanup Target Levels (SCTLs). Split soil samples will be collected and held for Synthetic Precipitate Leaching Procedure (SPLP) testing for samples that are below the direct-contact SCTL and above the default leachability SCTL.
Soil borings will be completed by CDM Smith at ten locations (Figure 1). Borings will be installed using hand augers and concrete coring will be completed at each of the ten locations.
Soil samples will be collected for laboratory analyses at the following depths: 0 to 0.5 feet below land surface (bls), 0.5 to 2 feet bls, and at 2-foot intervals thereafter to the water table. The soil samples will be submitted for laboratory analysis by United States Environmental Protection Agency (EPA) Method 8260 for VOHs. CDM Smith assumes up to 40 samples will be collected. The sample quantities are summarized in Table 1. CDM Smith will collect duplicate samples at each interval to be analyzed by SPLP but will identify which samples to run for SPLP after the initial data are received from the laboratory. Therefore, costs associated with SPLPs are not included in this cost estimate.
One soil boring in the vicinity of the former drycleaning machine will be completed as a 1-inch diameter monitoring well. The well will be installed in the upper shallow zone to a depth of 15 feet bls using a 420M portable drill rig. The well will be developed and sampled for VOH analyses by EPA Method 8260.
Five of the soil borings will be completed as vapor extraction wells (VEW) for Modified Active Gas Sampling (MAGS™) testing. CDM Smith will also install two vapor monitoring points (VMPs) to provide additional data to assess radius of influence (Figure 2). The VEW and VMP well screens will be installed to a depth of one foot above the water table and the top of the screen will terminate at a depth of approximately 1-foot bls. The total depth of the VEWs and VMPs is expected to be 6 feet bls. The VEWs will be constructed using 2-inch diameter polyvinyl chloride (PVC) and the VMPs will be 0.75-inch diameter PVC. The annulus of each VEW and VMP will be backfilled with sand pack and sealed at land surface with a sand/cement mortar mix.
MAGS™ testing will be performed at each proposed VEW. A step test will be completed on VEW- 001 only to select the MAGS™ test flow rates. The MAGS™ tests for each VEW will be completed within one hour or less, and CDM Smith assumes that setup, testing, and breakdown for all tests will require 2 days. A total of one soil gas sample from each VEW will be submitted to the laboratory for TO-15 analyses with standard reporting times.
Following MAGS™ testing, vertical groundwater profiling will be conducted at six locations using a direct-push technology (DPT) drill rig equipped with a groundwater screen point sampling system.
Groundwater samples will be collected using DPT screen points at the following depth intervals: 6 to 10 feet bls, 11 to 15 feet bls, 21 to 25 feet bls, and 31 to 35 feet bls or to depth of refusal or limestone, whichever occurs first. The groundwater samples will be collected directly, and laboratory analyzed for VOCs by EPA Method 8260. (Figure 3)
In addition, a continuous lithologic boring will be cored using DPT to obtain detailed site-specific lithologic and stratigraphic information for the site at a location immediately north of the facility (Figure 1). The target boring depth will be 50 feet bls, refusal, or the top of the SAS limestone. A detailed geologic log will be recorded, and the boring will be grouted using a tremie pipe.
At the completion of field events, the utilities that were marked during mobilization and all boring locations will be surveyed. Investigation-derived waste (IDW) collected up to this point will be characterized and transported off-site for disposal. A total of four soil drums and one water drum of nonhazardous IDW are anticipated at the completion of Stage I.
Deliverable: Laboratory analytical reports, field documentation, boring logs, data tables, paid invoices for subcontractors
Subtask 4 – Stage II Field Investigation The monitoring well locations will be finalized based on the Stage I results. CDM Smith currently plans to install four additional permanent groundwater monitoring wells. Prior to well installation, continuous soil cores will be collected to the total depth. Three of the wells will be shallow wells installed to a total depth of 15 feet bls and one intermediate well to a total depth of 25 feet bls. All wells will be 2-inch diameter PVC and have 10-foot screens. Sand pack will be installed to a depth of 1 ft above the screen followed by a 1-foot-thick fine-grained sand seal and the remainder of the borehole grouted. The wells will be developed following completion.
The wells will be purged and sampled for VOHs following installation and a full round of water levels will be collected to map the potentiometric surface. The monitoring wells will be surveyed at the completion of Subtask 4, and IDW will be characterized and transported off-site for disposal. A total of 14 soil drums and 4 water drums of nonhazardous IDW are anticipated at the completion of Stage II.
Deliverable: Completed well logs, laboratory analytical reports, paid invoices
Subtask 5 – Site Assessment Report CDM Smith will prepare a Site Assessment Report in general accordance with the requirements of FDEP Chapter 62-780, FAC. The report will summarize the history of the Site, physical and geologic conditions, all field work completed by CDM Smith, field and laboratory data, conclusions, and recommendations. Additional elements of the report will include the following:
Figures depicting site conditions
Figures depicting soil and groundwater data and estimated extent of impact
Groundwater elevation contour map
Tables summarizing field and analytical data
Soil/lithological logs
Monitor well completion reports
Groundwater sampling logs and field equipment calibration records
Laboratory analytical reports and chain of custody documentation
Waste disposal manifests and weigh tickets
Subcontractors Subcontracted services are required for drilling, surveying, utility location, sewer camera survey, and IDW disposal. The bidding, results, and subcontractor selections are described below, and Attachment A includes a summary of the subcontractor cost comparisons.
Drilling Request for Quotations (RFQs) were sent to three pre-qualified drilling companies for all drilling services. The lowest cost bid was from Ambient Technologies Inc. (ATI). However, ATI did not provide costs for Geoprobe sampling, required decontamination, and did not account for the number of drums required to complete the work. Betts Environmental Recovery, Inc. (Betts) was the next lowest bid at 17 percent higher than ATI. CDM Smith discussed the bid and project details with Betts, and we are satisfied that Betts can do the required work. Betts is the recommended drilling subcontractor.
Surveying Southeastern Surveying and Mapping is a primary subcontractor for HW681 and other bids were not solicited for this project. The total quote is included in Attachment A.
Utility Location Bids were obtained from Blood Hound, GeoView Inc., and Site Lines LLC to complete the utility survey. The total quotes are shown in Attachment A.
IDW Disposal Rates for IDW removal and disposal were previously requested for HW681 from Erwin Remediation, Clean Earth, ERS, and Clark Environmental. These rates were used to assess the current IDW disposal costs for the HandiCraft Cleaners site assessment. Erwin Remediation was the lowest cost and has been selected as the IDW disposal subcontractor for this project as shown in Attachment A.
Assumptions This scope and schedule have been prepared predicated on unknown site conditions. The scope and schedule may change depending on surveys and the drilling conditions. Drillers and other subcontractors’ availability may be limited and impact the schedule. In addition, this scope does not consider any weather delays. CDM Smith will communicate with the FDEP in a timely manner if this occurs to prepare the necessary paperwork for change orders.
Schedule CDM Smith’s proposed project schedule is shown below, with targeted completion of the Site Assessment process within eight months. The final dates on the schedule depend on Notice to Proceed (NTP) and subcontractor availability. Once CDM Smith receives NTP, the subcontractor’s availability will be confirmed, and the schedule may be adjusted. The deliverables and due dates are summarized in Table 2 below.
