Summary of Task Assignment Change Order HW681, DC002E-1
This document is a Task Assignment Change Order issued by the State of Florida Department of Environmental Protection (FDEP) to CDM Smith Inc. for environmental remediation monitoring services at the First Coast Cleaners site located at 610 3rd Street North, Jacksonville Beach, Duval County, Florida. The change order amends the original Task Assignment (TA) Number DC002E under Contract Number HW681. Benjamin Aoki serves as the FDEP Project Manager (850-245-7527), while Clint Noble is the contractor's Project Manager (904-527-6715). Joshua Hirten is listed as the CDM Smith Contract Manager. The change order was executed on December 9, 2025, by FDEP and December 10, 2025, by the contractor.
The change order reduces the total task assignment value from $41,595.12 to $40,240.07, representing a cost decrease of $1,355.05. This reduction eliminates sampling and analytical costs for Monitoring Well MW-020 due to the well being obstructed and unusable. The amendment modifies the sampling plan to remove labor and laboratory expenses associated with MW-020 across multiple subtasks while maintaining the same completion date of December 7, 2026. The total task assignment remains a fixed price of $40,240.07, with invoicing tied to specific deliverables including quarterly data deliverables, a monitoring well installation report, semi-annual reports, and a Year 1 PARM report. Attachment 1 of the change order has been replaced with Attachment 1-1 to reflect the revised scope of work and updated cost estimates.

Benjamin, Please see attached TACO per your revised sampling plan. Associated changes were made on expenses and labor and overall this is a net reduction of -$1,355.07, primarily from analytical fees and a little labor.
As soon as this is processed CDM will submit the deliverable and invoice for ST 2. The single water drum was removed yesterday from the site.

Good Morning Clint, Talked with our tech staff, and we’ll go with the following.
Continue sampling MW002 and MW019 for UIC parameters, but do not include MW005.
MW019 is enough to be representative of the interior for UIC.
Add MW012 to the semi-annual sampling, but only for VOHs and do not sample for UIC parameters.
We will wait to abandon MW020; more than likely it will be abandoned during site restoration.
This should answer your questions from below but let me know if you need anything else.
Thank you, Benjamin Aoki

Happy Friday Benjamin, Any update on this? I will need to proceed with a TACO before we take the next action.

Benjamin, We conducted the 1st GW sampling event at FCC - PARM on Oct 2. Our approved plan called for 3 wells - MW002, MW019, MW-020 for UIC parameters. These wells are also called for in our semi-annual sampling for CVOCs.
Upon inspection MW-020 is obstructed at depth. The well is located inside the building in the new floor. Logan tried to dislodge the obstruction for a while to no avail. It appears the well is breached and concrete settled in. This well was not sampled and we only sampled two wells. I have dug into the historical data for an alternative, see attached.
MW-020 is in the source zone but it is not a critical well. Last several sampling events show MW-020 to be below GCTLs except for VC. Approximately 15 feet away is our hottest source zone well (MW-
002) with historical concentrations of PCE ranging upwards of several thousand ppb and as high as 40,0000 ppb in 2019. This well is intact and it was sampled. MW-020 is hydraulically between MW-002 and MW-019, therefore the concentrations in these wells bound it.
My suggestion is that MW-005 is used in place of MW-020. It will provide an upgradient data point for UIC background concentrations.
I suggest a TACO that removes MW-020 from the sampling plan. MW-005 will be sampled instead of MW-020 during quarterly events and during semi-annual events MW-012 is added to the plan to track any source zone vertical migration. Historically MW-012 had detections of contaminants.
The TACO can also be used to add the proper well abandonment of MW-020 (as it is inside the building) when the drilling rig is out in the next event (in scope to re-install MW-03 in ST 3), and to subtract the analytical from this sampling event for MW-020 since I cannot invoice without this reduction.
Thank you, Clint Noble, P.G., PMP Principal Geologist
