DD 1354 Quick Notes for Sep 2009 Version
BLOCK 1: Three options.
1. CoE
2. SABER Office and address, then representative from CEP completes their information and signs in Block 24 (a,b,&c).
3. Construction company and address, their representative information and signature in Block 24 (a,b,&c).
BLOCK 2: Date the DD 1354 is being prepared/typed.
BLOCK 3: Project number, example KWRD 03-0167. Installations are:
KWRD for Holloman main base projects.
BXSR for Boles Well Field projects.
BYPV for Bonito Pipeline projects.
OBGR for Oscura Range TUAL for RAMS projects.
ALAY for RATSCAT projects.
BLOCK 4: Serial number if applicable.
BLOCK 5: Real Estate Office address.
BLOCK 6: Will be the installation where the work was completed.
ALAY, BXSR, BYPV, KWRD, OBGR or TUAL
BLOCK 7: Contract number if applicable.
BLOCK 7a: Date the government accepts the project.
BLOCK 8: Self-Explanatory for a, b, & c.
BLOCK 9: This column will start with line item 1, next row, line item 2, etc.
BLOCK 10a: Correct facility number is placed with each line item. NOTE: If the work is outside the facility beyond 5 feet you will have a different facility number. Example:
Water line, sewer line, electric, sidewalk, curbs, storm drains, etc.
BLOCK 11: Category code for the facility number listed.
BLOCK 12: Official nomenclature of the category code.
BLOCK 13: Type of construction.
P = Permanent S = Semi-permanent T = Temporary
BLOCK 14 – 17: Unique to the category code used. If the category code only has one primary unit of measure (UOM) you will used block 14 & 15. If the category code has dual UOM’s both will be listed, with the secondary UOM in block 16 & 17. Note: You can use the DD 1354 checklist for a quick reference on the commonly used category codes. If you have something that is not listed, used the category codes list that was sent to you or if you still need assistance please call the Real Estate Office (572-7166).
BLOCK 18: All costs associated with the line item are placed here. Included parts & labor.
BLOCK 19: Check with you 49 CES Engineer POC to confirm. Commonly used are:
01 = MILCON
03 = O & M
04 = RDTE
10 = Foreign Funds
11 = NAF
13 = OTHER
BLOCK 20: Commonly used are:
00 = Army Active 03 = Air Force Active
23 = DECA
33 = DESC
63 = Other Nations
BLOCK 21: Commonly used are:
01 = Owned by DoD 02 = Owned by Federal Government (non-DoD) 05 = Owned by Foreign Government
BLOCK 22: Self explanatory.
BLOCK 23: Two options.
1. If SABER Office and address, is in Block 1 then representative from CEP completes their information and signs in a, b, & c.
2. If Construction company and address, is in Block 1 their representative information and signature in a, b, & c.
BLOCK 24: Normally BCE or Deputy BCE information and signature.
BLOCK 25: ACES-RP journal number is inserted once information is picked up on Air Force records.
BLOCK 26: Construction deficiencies MUST be listed here.
BLOCK 27: Area for additional information as required. Normally a detailed description of work and sub contractor info.
