About this file
This document is a Task Assignment (TA) Change Order entered into between the State of Florida Department of Environmental Protection (FDEP) and Professional Service Industries, Inc. (Intertek-PSI) under Contract Number HW686, with Task Assignment Number DC021B. The change order amends the original task assignment for operations and maintenance activities at the Coastal Cleaners site (ERIC_5272) located at 2166 Main Street, Dunedin, Pinellas County, Florida. The project manager for FDEP is Kelly Crain (850-245-8753), and the contractor's project manager is Nana G. Westmark (813-886-1075). The change order was executed on September 17, 2024, and is effective through February 28, 2025.
The contractor will perform four quarters of operations and maintenance (O&M) activities, two semi-annual Remedial Action Status Summary (RASS) reports including annual groundwater monitoring for volatile organic hydrocarbons (VOHs) and per- and polyfluoroalkyl substances (PFAS), project management, and utilities cost reimbursement. The amended task value increased from $77,423.97 to $82,830.46, representing a cost increase of $5,406.51. This increase reflects the subtraction of four laboratory samples from Subtask 2 and the addition of labor and materials for repairs to the soil vapor extraction (SVE) system under Subtask 6. Payment is structured as fixed unit rate work totaling $70,590.46 and cost reimbursement for utilities not to exceed $12,240.00 with a $480.00 handling fee. Deliverables are invoiced upon FDEP approval, with final deliverable due January 15, 2025, and task completion by February 28, 2025.

TO: ERIC_5272

FROM: Mary Uribe, Project Manager
SUBJECT: Negotiation Summary
HW686 DC021B
DATE: September 11th, 2024
PSI will perform 4 quarters of O&M activities and two semi-annual RASS reports.
The second RASS report will include results of the annual groundwater monitoring for both VOHs and PFAS.
Summary of changes includes subtracting the cost of four samples from Subtask 2 as well as adding the cost for labor and materials to complete repairs on the SVE system under Subtask 6.
Total task amount is $82,830.46 with a change in cost of + $5,406.51 from the initial TA. PSI’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date is January 15, 2025. Task completion date is February 28, 2025.
