Hi Charles, There will be 10 DPT groundwater sampling locaon that included DPT at the Temp well. Figure 2 show the DPT GW samples and Temp well. We intend to sample at the vadose zone/water table which we esmate to be at 2 feet below land surface. Depending on the actual DTW we will collect sample 4 samples at each locaon at 2- intervals so for the DTW being 2 BLS we would collect samples a 5-, 8-, and 11-.

However, given the variability of DTW it would be safe to say that the deepest we would have to go would be 20- and would sll use 4 DTP groundwater samples at each locaon.

Good afternoon Kevin, ECT is requesting quotes for Miller Machinery drilling scope of work (SOW). Miller Machinery is located at 601 East Church St., Jacksonville, FL 32202. Figures of the sampling locations are attached and SOW is as follows:

ECT proposes having a concrete cutting contractor core through the concrete deck to advance soil borings for soil assessment activities. A hand dolly DPT rig will be used to collect DPT groundwater samples and to install the proposed temporary monitoring well. Figure 1 shows the proposed soil boring locations. The locations may be subject to change based on the double tee flanges of the concrete deck. It is expected that the temporary well will be installed between former soil borings SB-2 and SB-4, however, field data will indicate the actual location during the event.

ECT plans to advance soil boring adjacent to the original SB-1 through SB-4 (SB-1R through

SB-4R) and two locations at the dock (SB-5 and SB-6).

Sampling vadose soil sampling will be determined by the current depth of water (DTW), however, ECT assumes that the DTW will be 2.5 feet below land surface (BLS).

Mr. Sattler:

The price for the transportation and disposal (T&D) of 55-gallon drums of non-hazardous PFAS/PFOS impacted soil, mud, and water is listed below:

Impacted with Non-Hazardous PFAS/PFOS:

Description Rate Unit Qty Subtotal

Profile Fee (Charged once per site) $ 350.00 Per Site 1 $ 350.00

T&D - Non-Haz PFOS/PFAS Impacted Soil $ 195.00 Per Drum 1 $ 195.00

T&D - Non-Haz PFOS/PFAS Impacted Mud/Water $ 195.00 Per Drum 1 $ 195.00

False Mobilization (drum not found upon arrival) $ 85.00 Per Drum 1 $ 85.00

All drum lids should be properly secured, and drums should be placed on level ground (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.

Please feel free to contact me if you have any questions.

Subject: FDEP IDW Drum Trans. & Disposal

Hello Greg. Thank you for your call. UES has supported other Environmental Consultants with FDEP IDW drum management in the past.

Unfortunately, we cannot currently provide this service. Our operation is undergoing expansion and we cannot hold your waste drums. We hope to have a Solid Waste Solidification facility constructed in the near future. We have a Permit to Construct from the City of Tampa & are waiting on our Operations Permit from the FDEP.

Once this facility is completed we will have a turnkey operation. We currently can manage Non Hazardous Wastewaters at our Pretreatment Wastewater Facility. Soils and sludges will be managed at the Solidification Building.

I’ve copied our Business Development Mgr., Eric Ceballos, who will contact you in the near future to discuss all of our capabilities.

Re: Proposal for Supplemental Site Assessment

Miller Machinery, ERIC_4589 601 East Church Street, Jacksonville, Florida 32202

Dear Ms. Shirah:

Environmental Consulting & Technology, Inc. (ECT) has prepared this scope of work (SOW) and the attached Level of Effort Worksheet and Cost Estimate Forms/Expense Worksheet for the initial soil & groundwater site assessment of the facility referenced above. The proposed SOW will be performed to identify whether the Miller Machinery (Site) can be conditionally closed with the Florida

Department of Environmental Protection (FDEP) Risk Management Option (RMO) II/III in Chapter

62-780.680(2) and (3) Florida Administrative Code. The Level of Effort Worksheet and Cost

Estimate Forms/Expense Worksheet are attached. Subcontractor quotes are provided in

Attachment 1 and a Site Maps with sampling locations is included in Attachment 2.

Subtask 1 – Project Management

This subtask consists of the calculated project management labor, based upon 13% of the direct labor costs in this proposal, and includes fringe, overhead, other direct costs (ODCs) and completion fees.

Subtask 2 – HASP, Preparation, Site Assessment Activities

This subtask consists of the labor proposed for generating a site-specific Health and Safety Plan

(HASP), obtaining permits or additional site access agreements that may be required. Based on discussions with the FDEP soil and Direct Push Technology (DPT) groundwater sampling on southeast corner to the building and loading dock area are proposed. Within each soil boring soil samples are proposed to be collected within the vadose zone, at the vadose and water table interface and saturated samples at minimum of 1-foot and maximum of 2-foot into the saturated zone. Eight soil boring are proposed for this subtask. DPT groundwater samples will be collected at the vadose zone interface and three additional sample collections will be determined based on the current water table condition. A total of 10 DPT groundwater locations have been proposed, including the temporary well location. ECT does not expect to collect DPT groundwater samples deeper than 15-feet below land surface. The soil boring locations are shown on Figure 1 and the DPT groundwater locations are shown on Figure 2 in Attachment 2. Soil and DPT groundwater sample will be submitted for analysis by United States Environmental Protection Method 8260 volatile organic halogens (VOHs).

Additionally, depth to water measurements will be performed at monitoring wells MW004, MW007, MW010, MW100, MW200, MW201, MW311, MW312, MW401, and MW402. Groundwater samples will be collected from monitoring wells MW007, MW010, MW100, MW200, MW201, MW311, MW312, MW401, and MW402 submitted for analysis by USEPA Method 8260 VOHs.
