Re: Proposal for Jonfor Cleaners

Annual Natural Attenuation Report

11700-1 San Jose Boulevard

Jacksonville, Florida 32223

Facility ID: ERIC_4542

Contract No. HW682

Dear Ms. Shirah:

Environmental Consulting & Technology, Inc. (ECT) has prepared this scope of work to perform the annual natural attenuation monitoring and to prepare the Annual Natural Attenuation Monitoring

(NAM) Report for the Jonfor Cleaners in Jacksonville, Florida.

SubTask 1 Project Management

Based on the below scope of work the project management costs for the proposed work are 2.8 hours.

The project management cost is calculated on the attached Cost Estimate spreadsheet.

SubTask 2 Scope of Work

ECT proposes to conduct the tasks necessary to complete the annual NAM. The monitoring program consists of measuring depth-to-water (DTW) and collecting groundwater samples at 14 monitoring wells and 4 injection wells. Due to known PFAS impacts at the site, all purge water from the groundwater sampling event will be contained in a 55-gallon drum for disposal as PFAS impacted investigative derived wastes (IDW). Water levels will be measured and groundwater samples collected at monitoring wells IW001R, IW002, IW003R, IW004, IW006, IW008, IW009R, IW010R, IW011, IW012, IW015, IW016, IW017 and IW018; and injection wells I004, I006, I009, and I012. All groundwater samples will be analyzed for the volatile organic halogens (VOHs) by United States Environmental

Protection Agency (USEPA) Method 8260. The scope of work is detailed in the attached Level of Effort

Worksheet.

Because this site is new to ECT, we are requesting additional time to generate base maps and other related figures as well as to recreate tables. In addition, ECT is requesting time to prepare a Health and Safety Plan for this site. Future monitoring activities will rely on this Health and Safety Plan so it is anticipated that this plan will need to be prepared only once. Additionally, in the previous Task

Assignment ECT attempted to obtain site access without success and that time has been included in this proposal.

Once that data has been compiled, the Quarterly NAM Report will subsequently be prepared and provided to the FDEP Project Manager.

SubTask 2 Cost

ECT proposes to perform the Task 2 scope of work for a cost of $9,248.90 in accordance with the Level of Effort and Cost Estimate Form spreadsheets attached to this proposal. A summary of SubTask 2 is provided on Table 1.

Task 3 IDW Deliverable

Given recent the turnaround time to the transport and disposal of IDW, ECT is making the IDW waste manifest a separate SubTask. This cost includes disposal of one drum of PFAS impacted purge water from the groundwater sampling event. Quotes were obtained for disposal of the IDW transport and disposal. These quotes are summarized below and are provided as an attachment to this proposal.

Based on these quotes, Erwin Remediation will be subcontracted to dispose of IDW.

Subject: FDEP IDW Drum Trans. & Disposal

Hello Greg. Thank you for your call. UES has supported other Environmental Consultants with FDEP IDW drum management in the past.

Unfortunately, we cannot currently provide this service. Our operation is undergoing expansion and we cannot hold your waste drums. We hope to have a Solid Waste Solidification facility constructed in the near future. We have a Permit to Construct from the City of Tampa & are waiting on our Operations Permit from the FDEP.

Once this facility is completed we will have a turnkey operation. We currently can manage Non Hazardous Wastewaters at our Pretreatment Wastewater Facility. Soils and sludges will be managed at the Solidification Building.

I’ve copied our Business Development Mgr., Eric Ceballos, who will contact you in the near future to discuss all of our capabilities.

Regards, Ed Kinley
