Monitoring Well Install and Annual GWM

CDM Smith Inc. (CDM) will conduct Monitoring Well Installation and Annual Groundwater Monitoring. Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2: Monitoring Well Installation Based on the current and historical volatile organic halocarbons (VOHs) concentrations at shallow monitor well MW013R and MW-14R, CDM Smith recommends the installation of an intermediate monitoring well in this vicinity to evaluate the vertical extent of VOH impacts at this location. Continuous soil cores will be collected to a depth of 25 feet below land surface (bls). The monitoring well will be installed to a total depth of 25 ft bls and consist of 1- inch diameter polyvinyl chloride (PVC) with 5 feet of 0.01-slot prepack screen. Sand pack will be installed to a depth of 1 ft above the screen followed by a 1-ft-thick fine-grained sand seal and the remainder of the borehole grouted.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis. 2 IDW bids are included and the 3rd from EnviroServe was non-responsive.

Subtask 3: Groundwater Sampling Contractor will mobilize to the Site to collect groundwater samples from monitoring wells MW009, MW010, MW013R, and newly installed monitoring well MW-13I. All monitoring wells will be sampled in accordance with FDEP SOPs (FS 2200) for low-flow sampling using a peristaltic pump. A YSI-556 multi-parameter probe and a turbidimeter will be used to collect field data. Field parameter data will be collected (dissolved oxygen, pH, conductivity, oxidation reduction potential, temperature, turbidity) using properly calibrated probes, and recorded. Depth-to-groundwater measurements will be collected from all six monitoring wells prior to the collection of groundwater samples. The groundwater samples will be collected in laboratory supplied containers, placed on ice, and delivered to a NELAP certified laboratory for analysis by United States Environmental Protection Agency (EPA) Method 8260C for VOHs.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis. 2 IDW bids are included and the 3rd from EnviroServe was non-responsive.

Subtask 4: Baseline Groundwater Sampling Upon completion of groundwater sampling field work, Contractor will prepare a Groundwater Monitoring Report. This report will summarize all monitoring work completed, field and laboratory data, investigation-derived waste (IDW) management, conclusions, and recommendations.

CDM and the FDEP confirmed on June 26th, 2023, that an Access Agreement with the current real property owner of the site has been executed.

The final cost total for Task Assignment DC018B is $18,019.45.

Subject: Task Assignment Request—Monitoring Well Installation and Annual

Groundwater Monitoring Spring Fresh Dry Cleaners 1126 West Pipkin Road Lakeland, Polk County, Florida

Dear Mr. McMurray:

CDM Smith Inc. is pleased to submit this Task Assignment Request for Monitoring Well Installation and Annual Groundwater Monitoring of the Spring Fresh Dry Cleaners (the Site) located at 1126 West Pipkin Road Lakeland, Polk County, Florida, ERIC_5382. The scope of work, proposed deliverable, and deliverable schedule are summarized below and the cost estimate is also attached.

This Task Assignment, when executed, shall be incorporated in, and become part of Contract No.

HW681, between the Florida Department of Environmental Protection (FDEP), and CDM Smith, Inc. (Contractor), dated January 5, 2022, hereafter referred to as the Contract.

Scope of Work The proposed scope of work includes the following elements:

Subtask 1: Project Management Project management activities performed under this task consist of those generally administrative functions required to maintain the project on schedule, within budget, and that the quality of the work products defined within this scope is consistent with CDM Smith’s quality standards. We will prepare and submit invoices upon approval of each deliverable or milestone completion.

Project Kickoff and Progress Meetings Contractor expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.

ERIC_5382 _Spring Fresh Cleaners_GWMR Proposal 05_23

Project Resource Management Contractor will manage, allocate, and coordinate necessary resources for completion of tasks related to groundwater sampling.

Project Quality Technical Review Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.

Project Status Reports Contractor’s Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project.

Prior to well installation, continuous soil cores will be collected to a depth of 25 feet below land surface (bls). The monitoring well will be installed to a total depth of 25 ft bls and consist of 1-inch diameter polyvinyl chloride (PVC) with 5 feet of 0.01-slot prepack screen. Sand pack will be installed to a depth of 1 ft above the screen followed by a 1-ft-thick fine-grained sand seal and the remainder of the borehole grouted. The subcontractor will develop the monitoring well upon completion.

All soil cuttings and purge water will be containerized and transferred to a Florida Department of Transportation-approved 55-gallon drums. The disposal services will be provided by Erwin Environmental and two drums of nonhazardous soil and water is assumed.

All purge water will be containerized and transferred to a Florida Department of Transportation-approved 55-gallon drums. The disposal services will be provided by Erwin Environmental and one drum of nonhazardous water is assumed.

Subtask 4: Groundwater Monitoring Report Upon completion of groundwater sampling field work, Contractor will prepare a Groundwater Monitoring Report. This report will summarize all monitoring work completed, field and laboratory data, investigation-derived waste (IDW) management, conclusions, and recommendations. Additional elements of the report will include:

Tables summarizing field and analytical data.

Groundwater sampling logs.

Laboratory analytical reports.

Deliverable: Groundwater Monitoring Report

Subject: Drum Quote Request for CDM Smith

Greetings Steve, nice to be writing you from a new place of business. I have joined the CDM Smith team and working with the dry cleaning program. We are trying to get a current quote for 2023. Kara Fincham sent you the message below back in January. I thought I would try to send the same message to your new email. See below and respond with a quote if you would like us to use you.

We are renewing the rates for IDW for 2023—I will need a general cost proposal for the pickup and disposal of all investigation derived waste associated with our FDEP projects for one year. The rates should include non-hazardous waste, liquid and solid, generally in 55-gallon drums. The majority of the IDW will most likely be contained in 55-gallon drums and consist of soil and groundwater from environmental site investigations, but we are going to start doing more

PFAS work this year, so if there is a separate cost associated with PFAS IDW (soil/Groundwater), please include it in your proposal.

The sites are located throughout the state, including Pensacola, Jacksonville, North Florida, South Florida, Tampa, and

Orlando. The lowest bidder will win the work for the remainder of 2023.

If you are interested, please submit a proposal by Thursday, March 9, 2023.

If you are not interested in bidding, please reply to let us know.
