Negotiation Summary Task Assignment DC036B Creech’s Ideal Cleaners

ERIC_5395

SAR

CDM Smith Inc. (CDM) will prepare a Site Assessment Report. Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2: Site Survey CDM Smith will subcontract a utility location services company to provide a complete survey of the utilities and provide oversight for the Utility Survey. CDM Smith will have a professional land surveyor (PLS) perform a boundary survey prior to starting work.

Subtask 3: Stage I Field Investigation:

A continuous lithologic boring will be cored using DPT to obtain detailed site-specific lithologic and stratigraphic information for the site at a location immediately north of the facility. A detailed geologic log will be recorded, and the boring will be backfilled with grout using a tremie pipe. The Stage I Field Investigation will delineate the extent of volatile organic halocarbons (VOHs) exceeding the Soil Cleanup Target Levels (SCTLs). Split soil samples will be collected and will be held for Synthetic Precipitate Leaching Procedure (SPLP) testing for samples that are below the direct-contact SCTL and above the default leachability SCTL. Soil borings will be completed by CDM Smith at 17 locations. Borings will be installed using hand augers and concrete coring will be completed at each of the ten locations. Soil samples will be collected for laboratory analyses at the following depths: 0 to 0.5 feet bls, 0.5 to two feet bls, and at 2-foot intervals. CDM Smith assumes up to 68 samples will be collected. One soil boring near the former drycleaning machine will be completed as a 1-inch diameter monitoring well. The well will be installed in the upper shallow zone to a depth of eight feet bls using a 3.5-inch diameter stainless steel hand auger. The well will be developed and sampled for VOC analyses by EPA Method 8260. Nine of the soil borings will be completed as vapor extraction wells (VEW) for Modified Active Gas Sampling (MAGS™) testing. MAGS testing will be performed for each VEW in general accordance with the Modified Active Gas Sampling Manual dated October 12, 2004.

CDM Smith assumes that setup, testing, and breakdown for all tests will require one day. A total of three soil gas samples will be submitted to the laboratory for TO-15 analyses with standard reporting times. vertical groundwater profiling will be conducted at ten locations using a direct-push technology (DPT) drill rig equipped with a groundwater screen point sampling system.

Groundwater samples will be collected. In addition, a continuous lithologic boring will be cored using DPT to obtain detailed site-specific lithologic and stratigraphic information for the site at a location immediately north of the facility.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis.

Subtask 4: Stage II Field Investigation:

CDM Smith currently plans to install five additional permanent groundwater monitoring wells.

Prior to well installation, continuous soil cores will be collected to the total depth. Using direct push, four shallow zone wells will be installed to a total depth of 15 feet bls. Sand pack will be installed to a depth of one foot above the screen, followed by a 1-foot-thick fine-grained sand seal, and the remainder of the borehole will be grouted. The wells will be purged and sampled for VOHs following installation and a full round of water levels will be collected to map the potentiometric surface.

Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Erwin Remediation for IDW disposal was chosen on a lowest-cost basis.

Subtask 5: Site Assessment Report:

CDM Smith will prepare a Site Assessment Report in general accordance with the requirements of FDEP Chapter 62-780, FAC. The report will summarize the history of the Site, physical and geologic conditions, all field work completed by CDM Smith, field and laboratory data, conclusions, and recommendations.

FDEP Technical Staff has approved of the proposal dating May 10th, 2023.

CDM and the FDEP confirmed on May 18th, 2023, that an Access Agreement with the current real property owner of the site has been executed.

The final cost total for Task Assignment DC036B is $132,408.30.

Mr. Kyle McMurray Florida Department of Environmental Protection Hazardous Waste Cleanup Section 2600 North Blair Stone Road, MS 4520 Tallahassee, Florida 32399-2400

Subject: Creech’s Ideal Cleaners Site Assessment Proposal 501 Main Street, Palatka, Putnam County, Florida

ERIC_5395

Dear Mr. McMurray:

CDM Smith Inc. (CDM Smith) is pleased to respond to the Florida Department of Environmental Protection (FDEP) Task Assignment Request to provide site assessment services for the Creech’s Ideal Cleaners Site Assessment Work Plan site under Contract Number HW681. The scope of work, and project schedule are described below, and the cost estimate is also attached.

Site Assessment Scope of Work CDM Smith understands that FDEP would like to complete the initial site assessment within approximately eight months, leaving four months as a contingency to complete the site assessment within one year. The subtasks are described below.

Subtask 1 – Project Management Project management activities performed under this task generally consist of administrative functions required to maintain the project schedule, adhere to budget, and deliver the quality of the work products defined by CDM Smith's quality standards. We will prepare and submit invoices upon approval of each deliverable or milestone completion.

Project Kickoff and Progress Meetings CDM Smith expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.

Project Quality Technical Review CDM Smith's Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.

Project Status Reports CDM Smith's Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project. The Project Manager will also discuss the status of the project with the FDEP on a bi-weekly basis until the project is complete.

Subtask 2 – Site Survey Underground Utility Survey and Boundary Survey The work plan identified only a portion of the underground utilities. CDM Smith will subcontract a utility location services company to provide a complete survey of the utilities and provide oversight for the Utility Survey.

The site boundary shown on the Putnam County Property Appraisers website does not appear to match up with the existing building and fence line. To ensure work is only performed on the site property, CDM Smith will have a professional land surveyor (PLS) perform a boundary survey prior to starting work.

Deliverable: Surveys, paid invoices to surveyors

Subtask 3 – Stage I Field Investigation Initially, a continuous lithologic boring will be cored using DPT to obtain detailed site-specific lithologic and stratigraphic information for the site at a location immediately north of the facility (Figure 1). The target boring depth will be 50 feet below land surface (bls), refusal, or the top of the SAS limestone.

Soil borings will be completed by CDM Smith at 17 locations (Figure 1). Borings will be installed using hand augers and concrete coring will be completed at each of the ten locations.

Soil samples will be collected for laboratory analyses at the following depths: 0 to 0.5 feet bls, 0.5 to two feet bls, and at 2-foot intervals thereafter to the water table (estimated at six feet bls). The soil samples will be submitted for laboratory analysis by United States Environmental Protection Agency (EPA) Method 8260 for VOCs. CDM Smith assumes up to 68 samples will be collected. A soil sampling schedule is provided in Table 1. CDM Smith will collect duplicate samples at each interval to be analyzed by SPLP but will identify which samples to run for SPLP after the initial data is received from the laboratory.

Nine of the soil borings will be completed as vapor extraction wells (VEW) for Modified Active Gas Sampling (MAGS™) testing (Figure 2). The VEW well screens will be installed to a depth of one foot above the water table (estimated at six feet bls) and the top of the screen will terminate at a depth of approximately 1-foot bls. The total depth of the VEWs is expected to be five feet bls. The VEWs will be constructed using five feet of 2-inch diameter, 0.010-inch factory-slotted polyvinyl chloride (PVC) well screen, coupled with one foot of solid PVC riser. Each of the VEWs will be installed using a stainless steel hand auger. 20/30 graded silica sand will be placed from the bottom of the borehole to approximately 0.5 feet above the well screen, followed by 0.5 feet of neat cement grout.

The VEWs will be completed within an 8-inch bolt-down flush-mount manhole.

MAGS testing will be performed for each VEW in general accordance with the Modified Active Gas Sampling Manual dated October 12, 2004, prepared by HSA Engineers & Scientists. More specifically:

To begin the MAGS testing, a step test will be performed at VEW001, where a vacuum of approximately 15 inches of water (“H2O) will be applied. Vacuum and flow rate readings will be recorded, along with vacuum readings at select monitor wells and designated vacuum monitoring points. Influent PID readings will also be recorded. The vacuum will be increased to approximately 30 “H2O, where additional data like that specified above will be recorded. The final step will be to increase the vacuum to approximately 45 “H2O, with the final set of data recorded. Actual vacuums applied during each step may vary based on site conditions. Each step is expected take 15 minutes, for a total of 45 minutes for the step test.

Using the optimum flow rate and vacuum determined from the step test, vapor samples will be collected at each VEW. The vapor samples will be collected using a 6-liter Summa canister. A purge time will be calculated based on the flow rate and estimated radius of influence, to ensure the collection of steady-state data. Vacuum and flow rate readings will be recorded, along with vacuum readings at select monitor wells and designated vacuum monitoring points, at 15-minute intervals for a total duration of one hour. Influent PID readings will also be recorded.

CDM Smith assumes that setup, testing, and breakdown for all tests will require one day. A total of three soil gas samples will be submitted to the laboratory for TO-15 analyses with standard reporting times.

Following MAGS™ testing, vertical groundwater profiling will be conducted at ten locations using a direct-push technology (DPT) drill rig equipped with a groundwater screen point sampling system.

Groundwater samples will be collected using DPT screen points at the following depth intervals: six to ten feet bls, 16 to 20 feet bls, 26 to 30 feet bls, and 36 to 40 feet bls or to depth of refusal or limestone, whichever occurs first. The groundwater samples will be collected directly, and laboratory analyzed for VOCs by EPA Method 8260 (Figure 3).

In addition, a continuous lithologic boring will be cored using DPT to obtain detailed site-specific lithologic and stratigraphic information for the site at a location immediately north of the facility (Figure 1). The target boring depth will be 50 feet bls, refusal, or the top of the SAS limestone. A detailed geologic log will be recorded, and the boring will be backfilled with grout using a tremie pipe.

At the completion of field events, the utilities that were marked during mobilization and all boring locations will be surveyed. Investigation-derived waste (IDW) collected up to this point will be characterized and transported off-site for disposal. A total of six drums of nonhazardous IDW are anticipated at the completion of Stage I.

Deliverable: Laboratory analytical reports, field documentation, boring logs, data tables, paid invoices for subcontractors

Subtask 4 – Stage II Field Investigation The monitoring well locations will be finalized based on the Stage I results. CDM Smith currently plans to install five additional permanent groundwater monitoring wells. Prior to well installation, continuous soil cores will be collected to the total depth. Using direct push, four shallow zone wells will be installed to a total depth of 15 feet bls. Each shallow monitoring well will be constructed using five feet of 1-inch diameter solid PVC riser, coupled with ten feet of 0.010-inch prepacked well screen. Using hollow stem augers, one intermediate zone monitoring well will be installed to a total depth of 28 feet bls. The intermediate monitoring well will be constructed using 23 feet of 2-inch diameter solid PVC riser, coupled with five feet of 0.010-inch factory-slotted PVC well screen. Sand pack will be installed to a depth of one foot above the screen, followed by a 1-foot-thick fine-grained sand seal, and the remainder of the borehole will be grouted. The wells will be developed following completion.

The wells will be purged and sampled for VOHs following installation and a full round of water levels will be collected to map the potentiometric surface. The monitoring wells will be surveyed at the completion of Subtask 4, and IDW will be characterized and transported off-site for disposal. A total of five drums of nonhazardous IDW are anticipated at the completion of Stage II.

Deliverable: Completed well logs, laboratory analytical reports, paid invoices

Subtask 5 – Site Assessment Report CDM Smith will prepare a Site Assessment Report in general accordance with the requirements of FDEP Chapter 62-780, FAC. The report will summarize the history of the Site, physical and geologic conditions, all field work completed by CDM Smith, field and laboratory data, conclusions, and recommendations. Additional elements of the report will include the following:

Figures depicting site conditions

Figures depicting soil and groundwater data and estimated extent of impact

Groundwater elevation contour map

Tables summarizing field and analytical data

Soil/lithological logs

Monitor well completion reports

Groundwater sampling logs and field equipment calibration records

Laboratory analytical reports and chain of custody documentation

Waste disposal manifests and weigh tickets

Subcontractors Subcontracted services are required for drilling, surveying, utility location, and IDW disposal. The bidding, results, and subcontractor selections are described below, and Attachment A includes a summary of the subcontractor cost comparisons.

Drilling Request for Quotations (RFQs) were sent to three pre-qualified drilling companies for all drilling services. The lowest cost bid was from Groundwater Protection. The quotes are included in Attachment A.

Surveying Southeastern Surveying and Mapping is a primary subcontractor for HW681 and other bids were not solicited for this project. The quote is included in Attachment A.

Utility Location Bids were obtained from Blood Hound, GPRS Inc., and Site Lines LLC to complete the utility survey.

Site Lines LLC was the lowest bid. The quotes are included in Attachment A.

IDW Disposal Rates for IDW removal and disposal were previously requested for HW681 from Erwin Remediation, Hepaco, and Enviroserve (formerly Clark Environmental). Erwin Remediation was the lowest cost and has been selected as the IDW disposal subcontractor for this project as shown in Attachment A.

Assumptions This scope and schedule have been prepared predicated on unknown site conditions. The scope and schedule may change depending on surveys and the drilling conditions. Drillers and other subcontractors’ availability may be limited and impact the schedule. In addition, this scope does not consider any weather delays. CDM Smith will communicate with the FDEP in a timely manner if this occurs to prepare the necessary paperwork for change orders.

Schedule CDM Smith’s proposed project schedule is shown below, with targeted completion of the Site Assessment process within eight months. The final dates on the schedule depend on Notice to Proceed (NTP) and subcontractor availability. Once CDM Smith receives NTP, the subcontractor’s availability will be confirmed, and the schedule may be adjusted. The deliverables and due dates are summarized in Table 2 below.
