Quarterly GWM

CDM Smith Inc. (CDM) will conduct Quarterly Groundwater Monitoring. Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2, 3, 4, 5: Quarterly Groundwater Sampling Contractor will mobilize to the Site to collect groundwater samples from monitoring wells MW07D and MW13D on a quarterly basis, assuming the first quarter will be August 2023; second quarter will be November 2023, third quarter will be February 2024, and fourth quarter will be May 2024.

All monitoring wells will be sampled in accordance with FDEP SOPs. Field parameter data will be collected (dissolved oxygen, pH, conductivity, oxidation reduction potential, temperature, turbidity). Depth-to-groundwater measurements will be collected from all 13 monitoring wells.

Subtask 6: Annual Groundwater Monitoring Report Upon completion of groundwater sampling field work, Contractor will prepare an Annual Groundwater Monitoring Report. This report will summarize all monitoring work completed, field and laboratory data, investigation-derived waste (IDW) management, conclusions, and recommendations.

Erwin Remediation for IDW was chosen upon being the lowest responsive and responsible bidder. Only 2 IDW bids are included as the 3rd bidder EnviroServe was non-responsive.

CDM and the FDEP confirmed on June 29th, 2023, that an Access Agreement(s) with the current real property owner(s) of the site has been executed.

The final cost total for Task Assignment DC012C is $14,319.17.

Mr. Kyle McMurray Florida Department of Environmental Protection Division of Waste Management 2600 North Blair Stone Road Tallahassee, Florida 32399

Subject: Task Assignment Request—Quarterly Groundwater Monitoring Carriage Trade Dry Cleaners 38225 12th Avenue Zephyrhills, Florida

ERIC_5232

Dear Mr. McMurray:

CDM Smith Inc. is pleased to submit this Task Assignment Request for Groundwater Monitoring of the Carriage Trade Dry Cleaners (the Site) located at 38225 12th Avenue, Zephyrhills, Pasco County, Florida, ERIC_5232. The scope of work, proposed deliverable, and deliverable schedule are summarized below and the cost estimate is also attached.

This Task Assignment, when executed, shall be incorporated in, and become part of Contract No.

HW681, between the Florida Department of Environmental Protection (FDEP), and CDM Smith, Inc. (Contractor), dated January 5, 2022, hereafter referred to as the Contract.

Scope of Work The proposed scope of work includes the following elements:

Subtask 1: Project Management Project management activities performed under this task consist of those generally administrative functions required to maintain the project on schedule, within budget, and that the quality of the work products defined within this scope is consistent with CDM Smith’s quality standards. We will prepare and submit invoices upon approval of each deliverable or milestone completion.

Project Kickoff and Progress Meetings Contractor expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.

ERIC_5232_Carriage Trade_GWMR Proposal 06_23

Project Resource Management Contractor will manage, allocate, and coordinate necessary resources for completion of tasks related to groundwater sampling.

Project Quality Technical Review Contractor’s Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.

Project Status Reports Contractor’s Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project.

Subtask 2 through 5: Quarterly Groundwater Sampling Contractor will mobilize to the Site to collect groundwater samples from monitoring wells MW07D and MW13D on a quarterly basis, assuming the first quarter will be August 2023;

second quarter will be November 2023, third quarter will be February 2024, and fourth quarter will be May 2024.

All monitoring wells will be sampled in accordance with FDEP SOPs (FS 2200) for low-flow sampling using a peristaltic pump. A YSI-556 multi-parameter probe and a turbidimeter will be used to collect field data. Field parameter data will be collected (dissolved oxygen, pH, conductivity, oxidation reduction potential, temperature, turbidity) using properly calibrated probes, and recorded. Depth-to-groundwater measurements will be collected from all 13 monitoring wells at the Site prior to the collection of groundwater samples.

The groundwater samples will be collected in laboratory supplied containers, placed on ice, and delivered to a NELAP certified laboratory for analysis by United States Environmental Protection Agency (EPA) Method 8260C for volatile organic halocarbons (VOHs).

All purge water will be containerized and transferred to a Florida Department of Transportation-approved 55-gallon drum. One disposal event is planned for all purge water collected under this proposal. The disposal services will be provided by Erwin Environmental and one drum of nonhazardous water for both events is assumed.

Deliverables: Field notes, Laboratory COCs per quarter

Subtask 6: Annual Groundwater Monitoring Report Upon completion of groundwater sampling field work, Contractor will prepare an Annual Groundwater Monitoring Report. This report will summarize all monitoring work completed, field and laboratory data, investigation-derived waste (IDW) management, conclusions, and recommendations.

Deliverable: Groundwater Monitoring Report

ERIC_5232_Carriage Trade_GWMR Proposal 06_23

Schedule:

Contractor will schedule the field reconnaissance after the receipt of formal notice to proceed (NTP). The deliverables will be submitted as detailed below:

Subtask 2: Quarterly Groundwater Sampling – Quarter 1

Re: Non-Hazardous Drum Disposal

Ms. Fincham:

The price for the transportation and disposal (T&D) of 55-gallon drums of non-hazardous soil, mud, and water is listed below:

Impacted with Non-Hazardous Dry-Cleaning Solvents:

Description Rate Unit

T&D - Non-Haz Mud/Water $ 125.00 Per Drum

T&D - Non-Haz Soil $ 125.00 Per Drum

All drum lids should be properly secured, and drums should be placed on level ground (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.

The prices listed are all inclusive without any additional fees or charges (fuel surcharge, low volume stop fee etc.) and are valid for all regions in the State of Florida except for the Florida Keys.

Pricing for sites located in the Florida Keys will be bid on a case-by-case basis dependent on the volume of drums and the location.

Respectfully, Robert Donado Erwin Remediation, Inc.

Subject: FDEP IDW Rates for 2023

We are renewing the rates for IDW for 2023—I will need a general cost proposal for the pickup and disposal of all investigation derived waste associated with our FDEP projects for one year. The rates should include non-hazardous waste, liquid and solid, generally in 55-gallon drums. The majority of the IDW will most likely be contained in 55-gallon drums and consist of soil and groundwater from environmental site investigations, but we are going to start doing more PFAS work this year, so if there is a separate cost associated with PFAS IDW

(soil/Groundwater), please include it in your proposal.

The sites are located throughout the state, including Pensacola, Jacksonville, North Florida, South Florida, Tampa, and Orlando. The lowest bidder will win the work for the remainder of 2023.

If you are interested, please submit a proposal by Wednesday, January 11, 2023. If you are not interested in bidding, please reply back to let us know.

Thanks

Kara
