About this file
This document is a Task Assignment Change Order Form issued by the Florida Department of Environmental Protection (FDEP) for a professional services contract with CPH, Inc., an engineering and architectural consulting firm. The change order modifies Task Assignment No. TA35 under FDEP Contract No. CN210 for the Fort Zachary Taylor State Park Administration Building Replacement project located at Fort Zachary Taylor State Park in Florida. Key personnel include Kyle Bechtelheimer, P.E., as the consultant representative from CPH, Inc., and Ralph Perkins as the FDEP Contract Manager.
The change order is a time-only extension with no modification to the financial terms of the task. The original task amount of $49,099.09 (consisting of $2,000.00 in reimbursable expenses and $47,099.09 in fixed fee) remains unchanged with this change order. The original task execution date of July 29, 2022, is extended by 154 days to a new end date of December 29, 2022, to provide adequate time for design, permitting, and competitive bidding of the project. The consultant requested this extension in correspondence dated June 14, 2022, citing insufficient time within the original three-month period. The change order was executed and approved by all required signatures from FDEP representatives and the consultant by June 16, 2022.

Good afternoon, Rose.
I am the CPH PM for the Fort Zachary Taylor project. Looking through our paperwork, it does not appear we submitted a full schedule for this project.
I do see the TA states completion by July 29th but I do not know where that date came from.
Unfortunately, 3 months is not enough time for the completion of this project.
Could we please request an extension of time from July 29th to December 29th 2022?
This will give us adequate time for design, permitting, and bid of the project.
We will of course attempt to expedite the work as much as possible to complete it more timely, if possible.
Currently we have the survey completed and we should receive the geotechnical report this week or early next week at the latest.
We are currently working on the schematic design and will reach out soon to set up a design meeting.
Please let me know if you have any additional questions or need more information.
Thank you, Kyle Bechtelheimer, P.E.

Morning, I was recently hired as a PM by FDEP and will be taking over the Ft. Taylor 613930C ‐ Admin Building Replacement project from Jim Post.
I just talked to contracts and the request for additional time needs to come from the consultant – would you mind sending me a short email stating that CPH would like additional time for this project?
Thanks!

Rose, FYI TA 35 for the Ft. Zac Admin Bldg. expires on July 29th. It will need to be extended. I would go for a year considering the difficulty of permitting down there.
Thanks!
