About this file
This is a Task Assignment Change Order Form (Change Order No. CO1) issued by the Florida Department of Environmental Protection (FDEP) under Contract No. CN210, Task Assignment No. TA48. The change order modifies a consulting services agreement between FDEP and CPH, Inc., a consulting firm, for the Eden Garden State Park ADA Path to Waterfront & New Boat Dock project (Project No. 61452C) located in Florida. The contract is managed by Ralph Perkins for FDEP and Jason Toole, P.E., for CPH, Inc.
The change order adds $14,465.86 in additional services for ecological and permitting work that was not included in the original task scope. Specifically, the services consist of four subtasks: NwFWMD Permitting ($3,391.71), NwFWMD Field Meeting ($1,949.76), U.S. Army Corps of Engineers Permitting ($6,130.56), and ACOE Request for Additional Information Response ($2,993.83). These services address wetland impacts discovered during preliminary work that require permitting coordination and documentation. The change order also reflects a time reduction of 404 calendar days, with the original task end date of April 11, 2026, accelerated to March 3, 2025. The new task end date with all change orders is March 3, 2025. All deliverables require FDEP approval before payment is made, and costs are allocated through FDEP's budgeting system with Fund Code U3100 and Object Code 134000.
