About this file
This is Attachment E of a solicitation for Design-Bid-Build, Phase III - Groundwater Treatment Plant at Naval Weapons Industrial Reserve Plant, Bethpage, New York (Contract N40085-26-R-0161). The attachment is a template form that all offerors, both large and small businesses, must complete to provide historical small business utilization and participation data for evaluation under Factor 5 of the procurement.
The form requires offerors to document small business subcontracting achievements for up to five previous projects submitted as corporate experience examples. For each project, offerors must provide the total project dollar value, total self-performed value, total subcontracted value, and actual dollar amounts and percentages for subcontracting across seven socioeconomic categories: Small Business Concerns (overall), Large Business Concerns, Small Disadvantaged Business Concerns (SDB), Women-Owned Small Business Concerns (WOSB), HUBZone Small Business Concerns, Veteran-Owned Small Business Concerns (VOSB), and Service-Disabled Veteran-Owned Small Business Concerns (SDVOSB). Offerors must also provide corresponding goal percentages when applicable. All percentages must be rounded to the nearest tenth of a percent. For any projects with unmet goals, offerors must provide detailed explanations beyond merely acknowledging the missed goal. Additionally, if a project shows zero small business subcontracting achievement, offerors must explain the circumstances. The form explicitly states that responses in alternative formats or on offeror-created forms will not be evaluated, and only the complete official Attachment E will be considered.
