About this file
This is a Historical Small Business Utilization form (Attachment E) required for evaluation under Factor 5 of solicitation N40085-26-R-0161 for the Design-Bid-Build Phase III Groundwater Treatment Plant Construction at Naval Weapons Industrial Reserve Plant, Bethpage, New York. ALL offerors, regardless of size classification, must complete this form to support their corporate experience submissions under Factor 1.
The form requires offerors to document small business subcontracting utilization and achievement across up to five historical projects. For each project, offerors must provide: total project dollar value, total self-performed value, total subcontracted value, and a detailed breakdown of subcontracting dollars and percentages across seven categories—overall Small Business Concerns (SB), Large Business Concerns, Small Disadvantaged Business Concerns (SDB), Women-Owned Small Business Concerns (WOSB), HUBZone Small Business Concerns, Veteran-Owned Small Business Concerns (VOSB), and Service-Disabled Veteran-Owned Small Business Concerns (SDVOSB). All percentages must be rounded to the nearest tenth of a percent. Offerors must indicate whether they served as prime contractor, subcontractor, or joint venture member, and whether a subcontracting plan was required. For any unmet small business goals, detailed explanations are mandatory—simply acknowledging a missed goal is insufficient. Similarly, if a project shows zero small business subcontracting achievement, explanation is required. The form must be completed in the provided format; alternative formats or attachments will not be evaluated.
