About this file
This document contains a hazard tracking log template and details for a federal contract opportunity. The hazard tracking log template outlines the data fields required to track hazards, including hazard number, variant, subsystem, failure mode, initial and current severity and probability, and engineering, procedural, and warning mitigations.
The federal contract opportunity is for the Joint Light Tactical Vehicle (JLTV) Family of Vehicles follow-on production. The Department of the Army Materiel Command TACOM Life Cycle Management Command will hold an industry day on February 24-25, 2020 to provide information on requirements and terms for the competitive follow-on production contract, valued at over $5 billion for a five-year base with five one-year options. The opportunity includes one-on-one sessions, a static vehicle display, and questions must be submitted by certain dates to participate.

Content Requirements
| Hazard Tracking Log Content Requirements | |
| Each hazard recorded in the hazard tracking log shall include information for the following data fields: | |
| Required Data Field | Description/Definition of Required Data |
| Hazard Number | A unique alphanumeric code for each hazard. Formatted as XXX-###, where XXX identifies the contractor and ### is a unique number assigned to each hazard. |
| Variant | Variant of equipment the hazard affects: GP, UTL, CCWC, TRL. |
A single hazard may affect multiple equipment types.
| Subsystem | The subsystem the hazard is associated with. |
| Subsystem Description | Narrative describing the subsystem in which the hazard is located. Narratives for each subsystem shall be included in a separate single excel worksheet within the HTL excel file. The subsystem description shall not be included in every hazard record. |
| Date hazard identified | The date the hazard was identified/add to the hazard log. |
| Revision date | Date of last revision or update to the hazard record. |
| Status | The status of the hazard and its processing stage. |
Status definition:
New - Not reviewed by the ESOH WG Open - Reviewed by the ESOH WG Revised - Revision made after ESOH WG review, needs to be reviewed by ESOH WG again Rejected - Not credible or not applicable as determined by ESOH WG Eliminated - Closed by design as determined by the ESOH WG Closed - All mitigations have been implemented and verified by the ESOH WG
| Hazard Description | The hazard described in full detail. A complete description of the potential/actual hazards inherent in the item being analyzed, or resulting from normal actions or equipment failure, or handling of hazardous materials. |
| Life cycle occurence | The expected mission, time or period where the hazard would exist. |
The life cycle occurences are:
Operation - Any tasks performed by the crew Maintenance - Any task performed by the maintaner or the depot Transport - Any tasks related to transportation by another mode (i.e. vehicle, vessel, aircraft, etc.)
Storage - Any tasks related to storing the system Disposal - Any tasks related to demilitarizing or desposing of the vehicle
| Failure Mode | Failure modes which could result in the hazard. How the hazard would manifest itself during the life cycle. Failure modes generally answer the question of "how" it fails. |
| Hazard Outcome | Indicate the anticipated outcome(s): Death, Injury, Equipment Damage, and/or Environmental Damage |
| Initial Severity | Projected/expected "most reasonable credible" severity, including the rationale for the severity classification. |
| Initial Probability | Projected/expected probability of occurrence, including the rationale for probabiliy classification. |
| Intial Risk Level | Risk level based on projected/expected initial severity and probability. (High, Serious, Medium, Low) |
| Engineering mitigations | The various engineering and design features that have been implemented which reduce or eliminate the hazard. (i.e. design selection, safety devices) |
| Procedural mitigations | Procedures that reduce the probability of occurrence. Include refrence to technical manuals, training material, etc. where procedures are documented. |
| Warning/Caution/Notes (WCN)mitigations | Warnings, Cautions, and/or Notes in the technical manuals or included as labels on the vehicle which could reduce the probability of occurrence. Include specific warning/caution language. |
| Potential Mitigations | Potential mitigations (engineering, procedural, WCN) that have not been implemented but may be implemented in the future or if the level of risk warrants additional mitigation. |
| Current Severity | "Most reasonable credible" severity based on mitigations that are currently implemented. |
| Current Probability | Probability of occurrence based on mitigation measures that are currently implemented. |
| Current Risk Level | Risk level based on current severity and probability. |
| Is there a Critital Safety Item associated with this hazard? | Yes or No? If yes, what is the Critical Safety Item? |
| Safety Significant Software | Yes or No? If yes, include the Software Control Category and Software Criticality Index. |

PHL
| Preliminary Hazard List |
| The Preliminary Hazard List has been included to provide insight on the type of hazards to be considered for the hazard tracking log. The top-level hazards included as part of the PHL are incomplete and shall be further developed and included in the hazard tracking log. The PHL is not all inclusive. Additional hazards identified through contractor hazard analysis, evaluation, testing, etc. shall be included in the hazard tracking log. Hazards associated with integration of GFE, weapons systems, and add on kits shall be identified by the contractor and included in the hazard tracking log. |
The Initial RAC (I-RAC) assigned to the hazard is provided to assist the contractor in qualifying initial risks for the hazard tracking log. They are based on previous program phase Government testing and design analysis. The probability was determined by considering multiple individual casual factors within each top level hazard. The I-RAC provided represents the highest severity and probability assessed, and was created and assigned based on the guidance provided in MIL-STD-882E.
