About this file
This is a Transition Out Plan exhibit for a California state contract involving Cal ICH (California Department of Health Care Quality, formerly Cal Department of Health Care Access and Information) and contractor transition services. The plan documents the structured handoff process from an outgoing contractor to an incoming contractor over an estimated six-month transition period (July 2027 through December 2027). The scope encompasses management of multiple integrated platforms and systems including the HDIS platform, Managed IT infrastructure, Informatica IDMC, Informatica MDM, Snowflake data platform, Tableau Cloud, software licensing, and quarterly data management processes. Key deliverables include complete documentation, standard operating procedures (SOPs), runbooks, system configurations, and training materials. The transition includes shadowing of Q1 data loads in July with the incoming contractor assuming full responsibility for Q2 loads from August through October 2027, followed by post-transition support from December 1-31, 2027. Entry criteria require approval of the transition plan, provisioned access across all platforms, and current documentation from the outgoing contractor, while exit criteria mandate completed handoff deliverables with Cal ICH and incoming contractor sign-off, demonstrated independent execution capability, and submission of a comprehensive Transition Out Report.
Governance structure establishes Cal ICH as the sponsor and approving authority, with the outgoing contractor responsible for knowledge transfer and documentation delivery and the incoming contractor assuming ownership per the defined schedule. The plan incorporates comprehensive risk management addressing access provisioning delays, role-based access control (RBAC) misalignments, OEM-bound licensing transfers (such as Datto), data quality concerns, and disaster recovery readiness, with mitigations including early access checklists, vendor engagement, and dual-run validations. Service level agreements (SLAs) are tracked for data load timeliness, ingestion success rates, incident response times, dashboard availability, and patch compliance, with monthly reporting on platform performance and cost optimization. Communication occurs through weekly transition stand-ups, bi-weekly workstream reviews, and monthly steering committee meetings using Teams, Jira, SharePoint, and email channels. The plan includes detailed task-level assignments across nine workstream categories with specific due dates, assigned ownership, and measurable deliverables, culminating in a formal cutover process with readiness review, configuration backup, validation testing, contingency rollback procedures, and final Cal ICH acceptance and sign-off.
