Subtask 2, 3, 4, and 5: SVE System Operation and Maintenance, Optimization O&M site visits will be completed monthly following system startup. Subtasks 2, 3, 4, and 5 cover the costs associated with 11 months of system operation. The following work will be completed during each visit. SVE combined influent and system effluent samples will be collected following the first month of system operation for laboratory analysis by TO-15. Combined influent samples will only be collected for laboratory analysis quarterly thereafter, as the GAC vessel will be taken offline after 30 days of system operation. Air samples will be collected from the combined influent and all five VE wells on an annual basis, or during the (Quarter 4) O&M activities.
Semi-annual groundwater sampling will be performed as part of the (Quarter 2) O&M activities. In accordance with the ISRP, groundwater samples will be collected from monitoring wells MW01, MW02, MW05, MW06, MW012, and MW015 semi-annually. Following the completion of O&M (Quarter 2) activities, CDM Smith will prepare a Semiannual RASR. An Annual RASR will be prepared following completion of the (Quarter 4) O&M activities.

Mr. Kyle McMurray Florida Department of Environmental Protection Hazardous Waste Cleanup Section 2600 North Blair Stone Road, MS 4520 Tallahassee, Florida 32399-2400
Subject: Proposal for 2023 Operation and Maintenance Services
Nelson’s Cleaners
ERIC_4822
1203 West Highway 50
Clermont, Lake County, Florida
Dear Mr. McMurray:
CDM Smith Inc. (CDM Smith) is pleased to respond to the Florida Department of Environmental Protection (FDEP) Task Assignment Request to perform SVE system operation and maintenance (O&M) for Nelson's Cleaners (the Site) located at 1203 West Highway 50, Suite G, in Clermont, Lake County, Florida, ERIC_4822. Our project understanding, the scope of work, and project schedule are described below, and the cost estimate is also attached.
Project Understanding CDM Smith understands that FDEP requires one year of SVE system O&M, SVE system monitoring and optimization, groundwater monitoring, and semi-annual remediation status reports. Semi-annual remediation status reports were required by HSW Consulting, LLC (HSW) and described in the Interim Source Removal Plan (ISRP, HSW 2022) The project duration will be approximately 12 months. CDM Smith understands that the Site is an actively operating facility, and the site work must accommodate the Site operations.
Scope of Work CDM Smith’s proposed scope of work is described below. To complete this project, support services will be required from subcontractors. The laboratory and disposal services will be provided by companies with approved rates previously negotiated for use on the Waste Cleanup Program Contract HW681. The companies selected by CDM Smith for the Nelson’s Cleaners project are identified in the associated task descriptions.
Nelson's Cleaners O&M Proposal 06_23
Subtask 1 – Project Management Project management activities performed under this task consist of administrative functions required to maintain the project schedule, adhere to budget, and deliver the quality of the work products defined by CDM Smith's quality standards. We will prepare and submit invoices upon approval of each deliverable or milestone completion.
Project Kickoff and Progress Meetings CDM Smith expects to have a kickoff meeting or telephone conference with the FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.
Project Quality Technical Review CDM Smith's Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.
Project Status Reports CDM Smith's Project Manager will prepare and submit monthly status reports to accompany each invoice throughout this project. The Project Manager will also discuss the project's status with the FDEP bi-weekly until it is complete.
Subtasks 2, 3, 4, and 5 – SVE System Operation and Maintenance, Optimization O&M site visits will be completed monthly following system startup. Subtasks 2, 3, 4, and 5 cover the costs associated with 11 months of system operation. The following work will be completed during each visit.
System operation and alarm status and blower run time.
Moisture separator liquid volume.
System vacuums and velocity/flow rate at blower, before/after GAC, and after moisture separator.
Blower oil changes, based on manufacturer recommendations.
SVE well vacuum/pressure and flow rate.
Total organic vapors measured with a photoionization detector from operating SVE wells, system influent, and system effluent.
Water levels from MW001R, MW004, and MW014.
Adjust vacuums and injection pressures on the SVE wells to optimize the SVE system performance.
The information will be recorded by CDM Smith in the field on pre-prepared forms.
SVE combined influent and system effluent samples will be collected following the first month of system operation for laboratory analysis by TO-15. Combined influent samples will only be collected for laboratory analysis quarterly thereafter, as the GAC vessel will be taken offline after 30 days of system operation. Air samples will be collected from the combined influent and all five VE wells on an annual basis, or during the Subtask 5 (Quarter 4) O&M activities.
Semi-annual groundwater sampling will be performed as part of the Subtask 3 (Quarter 2) O&M activities. In accordance with the ISRP, groundwater samples will be collected from monitoring wells MW01, MW02, MW05, MW06, MW012, and MW015 semi-annually. The results of the semi-annual sampling will be presented in the Semi-Annual Remedial Action Status Report (RASR) prepared under Subtask 3. It is assumed that the annual groundwater monitoring event will take place during the Year 2 O&M activities, and costs associated with this sampling event are not included in this proposal.
Following the completion of Subtask 3 O&M (Quarter 2) activities, CDM Smith will prepare a Semiannual RASR. An Annual RASR will be prepared following completion of the Subtask 5 (Quarter 4) O&M activities. The reports will summarize the history of the Site, all field work completed by CDM Smith, field and laboratory data, conclusions, and recommendations. Additional elements of the reports will include the following:
Figures depicting site conditions.
Figures depicting groundwater data and estimated extent of impact.
Groundwater elevation contour map.
Tables summarizing field and analytical data.
Groundwater sampling logs and field equipment calibration records.
Laboratory analytical reports and chain of custody documentation.
Waste disposal manifests and weigh tickets.
CDM Smith assumes two drums of liquid IDW will be collected by the moisture separator requiring disposal over twelve months of system operation. Disposal services will be provided by Erwin Remediation, using rates previously approved for use on the Waste Cleanup Program Contract
HW681.
Deliverables: Semiannual Remedial Action Status Report, Annual Remedial Action Status Report
Subtasks 6 and 7 – Utilities CDM Smith will provide utilities for the SVE system and assume $700 per month including administrative costs.
Deliverable: Utilities Handling Fee, and receipt of six months of invoices (Cost Plus Fixed Fee)
Subcontractor Selection Costs for IDW removal and disposal were requested from Erwin Remediation, Clean Earth, ERS, and Clark Environmental. As the lowest bidder, Erwin Remediation was selected to remove and dispose of IDW. Based upon other cost estimates, Erwin Remediation appears to be the lowest cost handler of IDW across the state for this work.
Subcontractor Evaluation and supporting bids are included in Attachment A.
Schedule CDM Smith will schedule field activities based on availability of the subcontractors upon receipt of formal notice to proceed (NTP). The deliverables will be submitted as detailed below.
Subtasks 2 and 3: System O&M and Optimization Deliverables: Quarterly O&M Data Reports, Semiannual Remedial Action Status Report Due Date: March 30, 2024 Subtasks 4 and 5: System O&M and Optimization Deliverables: Quarterly O&M Data Report, Annual Remedial Action Status Report Due Date: September 30, 2024
Subtasks 6 and 7: Utilities Deliverables: Setup Fee, Utilities Handling Fee, and Paid Utility Invoices Due Date: Semi-Annually

Re: Non-Hazardous Drum Disposal
Ms. Fincham:
The price for the transportation and disposal (T&D) of 55-gallon drums of non-hazardous soil, mud, and water is listed below:
Impacted with Non-Hazardous Dry-Cleaning Solvents:
Description Rate Unit
T&D - Non-Haz Mud/Water $ 125.00 Per Drum
T&D - Non-Haz Soil $ 125.00 Per Drum
All drum lids should be properly secured, and drums should be placed on level ground (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.
The prices listed are all inclusive without any additional fees or charges (fuel surcharge, low volume stop fee etc.) and are valid for all regions in the State of Florida except for the Florida Keys.
Pricing for sites located in the Florida Keys will be bid on a case-by-case basis dependent on the volume of drums and the location.
Respectfully, Robert Donado Erwin Remediation, Inc.

Subject: FDEP IDW Rates for 2023
We are renewing the rates for IDW for 2023—I will need a general cost proposal for the pickup and disposal of all investigation derived waste associated with our FDEP projects for one year. The rates should include non-hazardous waste, liquid and solid, generally in 55-gallon drums. The majority of the IDW will most likely be contained in 55-gallon drums and consist of soil and groundwater from environmental site investigations, but we are going to start doing more PFAS work this year, so if there is a separate cost associated with PFAS IDW
(soil/Groundwater), please include it in your proposal.
The sites are located throughout the state, including Pensacola, Jacksonville, North Florida, South Florida, Tampa, and Orlando. The lowest bidder will win the work for the remainder of 2023.
If you are interested, please submit a proposal by Wednesday, January 11, 2023. If you are not interested in bidding, please reply back to let us know.
Thanks
Kara
