ERIC_4749

Well Install and SSAR

CDM Smith Inc. (CDM) will conduct Well Installation and Supplemental Site Assessment. Specific subtasks are detailed below:

Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2: Monitoring Well Installation CDM Smith proposes to install shallow and intermediate well clusters east and west of existing MW- 8/MW-7 and MW-12/MW-13 well clusters (4 clusters total) and the installation of two (2) additional intermediate wells (one near MW-3 and one between MW-13 and MW-9). Off-site access agreements will need to install the proposed offsite wells. The off-site access will attempt to be secured. CDM Smith will subcontract utility location services to clear the boring locations prior to well drilling. In addition, CDM Smith will also request Sunshine 811 services to mark the public utilities.

Subtask 3: Groundwater Sampling After the new wells have been installed, 13 existing wells 10 new wells (23 total) will be sampled for VOHs by Method 8260B. Water levels will be recorded from all wells and the water levels will be collected in as short a time span as reasonable to minimize the potential effects of the changing tide on the water level elevations. Water quality data will be collected during well purging including dissolved oxygen, pH, conductivity, oxidation-reduction potential, temperature, and turbidity.

Subtask 4: Investigation-Derived Management Erwin Remediation for IDW was chosen upon being the lowest responsive and responsible bidder. Only 2 IDW bids are included as the 3rd bidder EnviroServe was non-responsive.

Subtask 5: Supplemental Site Assessment Report Preparation CDM Smith will prepare a Supplemental Site Assessment Report (SSAR) to document the completion of the site assessment work phase and recommend the next steps toward remediation or natural attenuation monitoring.

Site Lines, Inc for Utility Locate and Environmental Drilling Solutions, Inc for Drilling was chosen upon being the lowest responsive and responsible bidder.

CDM and the FDEP confirmed on June 29th, 2023, that an Access Agreement(s) with the current real property owner(s) of the site has been executed.

The final cost total for Task Assignment DC025B is $62,109.38.

Subject: McMullen Dry Cleaners – Well Installation and Supplemental Site Assessment

Proposal

ERIC_4749

7853 US Highway 301 South

Riverview, Hillsborough County, Florida

Dear Ms. Jean-Baptiste:

CDM Smith Inc. (CDM Smith) is pleased to submit to the Florida Department of Environmental

Protection (FDEP) this Task Assignment Request to provide monitoring well installation, groundwater monitoring, and report preparation services for the McMullen Dry Cleaners Site. Our project understanding, the scope of work and project schedule are described below, and the cost estimate is also attached.

Project Understanding

CDM Smith understands that FDEP is requesting well installation and groundwater monitoring services at the McMullen Dry Cleaners Site located in Riverview, Florida. A Supplemental Site

Assessment Report (SSAR) was submitted in March 2023 that recommended installation of additional off-site monitoring wells to delineate the extent of volatile organic halogens (VOHs) in groundwater. This task assignment will fulfill the well installation and groundwater monitoring requirements and a Supplemental SAR will be prepared to complete the site assessment phase at this site.

Scope of Work

Subtask 1 – Project Management

Project management activities performed under this task generally consist of administrative functions required to maintain the project schedule, adhere to budget, and deliver the quality of the work products defined by CDM Smith's quality standards. CDM Smith's project manager will be

Kathy Leggoe, PMP. We will prepare and submit invoices upon approval of each deliverable or milestone completion.

Project Kickoff and Progress Meetings

CDM Smith expects to have a kickoff meeting or telephone conference with FDEP to review the status of the project and address FDEP comments regarding the Task Assignment. CDM Smith will prepare meeting agendas and notes as part of this task.

Project Quality Technical Review

CDM Smith's Quality Management System guidelines will be followed. Internal quality reviews will be conducted on plans and on investigation findings and reports.

Project Status Reports

CDM Smith's Project Manager will prepare and submit monthly status reports to accompany each invoice throughout the duration of this project. The Project Manager will also discuss the status of the project with FDEP on a bi-weekly basis until the project is complete.

Subtask 2 – Monitoring Well Installation

CDM Smith proposes to install shallow and intermediate well clusters east and west of existing MW-

8/MW-7 and MW-12/MW-13 well clusters (4 clusters total) and the installation of two (2) additional intermediate wells (one near MW-3 and one between MW-13 and MW-9). Locations are provided in Figures 1 and 2.

Off-site Access Agreements

Off-site access agreements will need to install the proposed offsite wells as listed below.

• Proposed monitoring wells southeast and southwest of monitoring well cluster MW-8/MW-

7, located at 8001 Highway 301 South, Riverview, Florida, property owner “8001 US 301

LLC”, parcel ID “U-17-30-20-2RI-000000-00007.1.”

• Proposed monitoring wells east and west of monitoring well cluster MW-12/MW-13 and intermediate well between MW-13 and MW-9, located at 8017 US Highway 301 S, Frederick

Ramlow Jr and Elinor J, parcel ID’s “U-17-30-20-2RI-000003-00008.0 and U-17-30-20-2RI-

000003-00009.1”

The off-site access will attempt to be secured by the following steps:

CDM Smith personnel will conduct a site visit up to two times to meet with the site owners and/or tenant to get a signed off-site access agreement. If the access agreement is not signed during the first two times in person, CDM Smith send access request letters to each applicable property owner via certified US Mail or Federal Express so receipt of the letter can be verified and placed in the project files. Each letter will include the standard FDEP language, Non-Source permission to enter property forms for signature, and a proposed map that will show the location of the proposed groundwater monitoring well(s). If access to the proposed drilling locations is denied by the property owners, the FDEP will be notified, and next steps will be determined.

Approximately 30 days are estimated to complete this subtask.

Underground Utility Survey

CDM Smith will subcontract utility location services to clear the boring locations prior to well drilling. In addition, CDM Smith will also request Sunshine 811 services to mark the public utilities.

Drilling

Continuous formation samples will be collected using a soil core driven to a 30-ft depth and a boring log will be recorded. The monitoring wells will be installed using hollow-stem augers and completed as two-inch diameter PVC wells. The upper surficial aquifer wells will be 12-ft-deep with

10-ft screens and the lower surficial aquifer wells will be 30-ft-deep with 5-ft screens. Sand pack and a fine-grained sand seal will be installed, and the remainder of the borehole annulus grouted.

The subcontractor will develop the wells following completion. The wells will be surveyed for location and elevation by a professional surveyor. Drill cuttings and well development water will be containerized as investigation-derived waste (IDW) in drums for disposal. CDM Smith estimates 8 water and 16 soil drum IDW will require disposal under Subtask 2.

Surveying

The new wells will be surveyed by Southeastern Surveying and mapping.

Approximately 6 days are estimated to complete this subtask.

Deliverable: Field documentation, boring logs, well construction details and permits, updated tables and figures

Subtask 3 – Groundwater Sampling

After the new wells have been installed, 13 existing wells 10 new wells (23 total) will be sampled for VOHs by Method 8260B.

Groundwater Sampling Procedures

Well purging and sample collection will be completed in accordance with FDEP Standard Operation

Procedure (SOP)-001/01, effective date of April 16, 2018, and as outlined in Chapter 62-780, Florida Administrative Code (FAC). CDM Smith will employ the low-flow purge and sampling approach using a peristaltic pump and flow-through cell. A YSI-556 multi-parameter probe and a turbidimeter will be used to measure the water quality. All instrumentation will be properly calibrated and documented. Purge water will be containerized as IDW in drums for disposal. CDM

Smith assumes 4 drums of IDW will require disposal under Subtask 3 (23 wells, 5 gal each = 115 gallons = up to 4 drums for sampling).

Approximately 2 days are estimated to complete this subtask.

Deliverable: Field documentation, groundwater sampling logs, laboratory report, and updated tables

Subtask 4 – Investigation-Derived Management

All IDW including soil cuttings, well development water, and groundwater sampling purge water will be containerized in 55-gallon drums and staged on-site. CDM Smith estimates a total of 28 drums of IDW will be collected for disposal as nonhazardous waste. Pricing to disposed of as PFAS impacted has been included because shallow groundwater previously showed impacts above target levels. One soil and one water sample will be collected for disposal characterization.

Deliverable: Waste disposal documentation

Subtask 5 – Supplemental Site Assessment Report Preparation

CDM Smith will prepare a Supplemental Site Assessment Report (SSAR) to document the completion of the site assessment work phase and recommend the next steps toward remediation or natural attenuation monitoring. This report will include reporting for Subtasks 2 to 4 above. This report will include updated figures and data tables along with any notable exceedances in on-site or off-site wells and CDM Smith’s conclusions and recommendations. The report will be in letter format.

Deliverable: SSAR

SITE LINES LLC PROPOSAL

SITE LOCATION

McMullen Dry Cleaners Site – 7853 US Highway 301 South, Riverview, FL

PROPOSED SCOPE OF WORK

Site Lines (“Company”) will use Ground Penetrating Radar (GPR) and conventional electromagnetic (EM) pipe and cable locators to determine the location of underground utilities at the site identified by CDM Smith (“Client”).

RESULTS OF THE INVESTIGATION

The estimated depth and location of utilities identified during the investigation will be marked on the ground surface using spray paint or other method to be agreed upon by the parties. Designated utilities will also be documented on a field sketch.

The American Public Works Association Uniform Color Codes for the marking of underground utilities will be used.

PROJECT FEE

The fee to complete the project as described within this proposal is: Eight Hundred Dollars [$800.00].

Client shall pay Company in full within thirty (30) days of the invoice date, unless the parties have agreed to another payment arrangement in writing. If during the investigation, Company determines that the goals of the project cannot be achieved with the resources available to Company, Company will notify Client as soon as reasonably possible. Under such circumstances, Company will charge Client only for time expended and materials used to date.

CLIENT REQUIREMENTS

Client is responsible for all site access, maintenance of traffic (MOT), site clearing and/or any other logistical considerations necessary to conduct the utility locate.

Client will also provide all pertinent maps of the project site, including those that show the boundaries of the utility locate, pertinent landmarks, and known utilities (if available). Company is not a substitute for Chapter 556, Florida Statutes.

Accordingly, Client is responsible for calling Sunshine State One Call of Florida

(811) before undertaking any invasive activities, whether through Company, any other entity, or on its own.

DESCRIPTION OF COMPANY INVESTIGATION AND LIMITATIONS

A GSSI SIR-4000 GPR system will be used with a 900, 400, or 270 megahertz antenna configuration depending on site conditions and desired targets. The GPR will assist in locating nonmetallic and metallic utilities. Physical surface conditions, subsurface conditions and technical limitations effect the ability of GPR to resolve targeted utilities and/or subsurface structures. Depth and/or diameter of the utility, soil type, moisture/ionic content of the soil, and electrical properties of the soil compared to that of the utility can prevent the GPR from identifying utilities at the site. The diameter of a utility cannot be determined by GPR data. Estimated depths of utilities are directly affected by soil conditions and the material of the utility. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities.

An RD 8000 will be used for the EM survey. Electromagnetic induction which transmits a signal directly into the desired target and passive power detection which locates naturally occurring magnetic fields in energized lines will be identified by detecting and following the signals with a receiver. The EM survey will assist in finding only metallic utilities. The magnetic field and/or induced signal should emanate from the target in a circular geometry. When this occurs, the receiver will receive its peak signal directly above the target. However, when metallic utilities are running near each other, or there is environmental interference from fences, cars or overhead power lines, the signal can become distorted and the peak no longer can be directly over the utility. Care must be taken when doing any invasive work around the utilities. A margin of error of +/- 20-25% in depth should be expected when performing invasive activities. Company’s personnel are highly experienced and certified operators of GPR and EM pipe and cable locators.

Due to the limitations described above, when Client is working around indicated utilities, a minimum of two (2)-feet clearance from either side of the marked utility will be maintained. For absolute confirmation, Client is responsible for exposing any indicated underground utility.

Company will conduct utility investigations in accordance with the level of care and generally accepted professional practices as ordinarily exercised by members of the same profession practicing in the same locality under similar conditions.

For these reasons, Company expresses no guarantee that all utilities and underground structures will be identified and located during this project.

Company is responsible only for locating underground utilities, the presence of which is both known and indicated on site maps to be provided by Client to Company before Company performs the utility locate. Company will use its best efforts to locate both known and unknown utilities within the project boundaries.
