Task Assignment DC011E Summary
This document is a Task Assignment (TA) under Contract HW681 between the State of Florida Department of Environmental Protection (FDEP) and CDM Smith Inc., dated January 27, 2026, with completion scheduled for October 30, 2026. The TA authorizes work at Peterson Cleaners, located at 840 North Broadway Avenue, Bartow, Polk County, Florida (ERIC_5380). The project involves decommissioning a Soil Vapor Extraction (SVE) system per the contractor's proposed revised scope of work and cost estimate, with Benjamin Aoki serving as the Department Project Manager and Kathy Leggoe as the Contractor's Project Manager.
The contract value for this TA is $21,935.64 on a fixed unit rate basis, comprising four subtasks: Subtask 1 ($1,137.46) covers project management calculated at 13 percent of direct labor; Subtask 2 ($4,901.73) covers granular activated carbon (GAC) management and power disconnect, with field notes and laboratory chain of custody due by May 29, 2026; Subtask 3 ($14,339.94) covers SVE system removal and site restoration performed by subcontractor RSI LLC at $9,750.00; and Subtask 4 ($1,556.51) covers the SVE system decommissioning report due by August 28, 2026. The project includes subcontractor quotes from RSI LLC ($9,750.00 for system removal), Erwin Remediation ($475.00 for IDW disposal), and laboratory costs ($481.00). The SVE system will be transferred to the Sun State Supply Site (ERIC_5490) for reuse, with the existing blower stored at the TOC Apollo Site (ERIC_4697) following decommissioning activities.

Mr. Turner:
The price for the transportation and disposal (T&D) of non-hazardous drums of soil, mud, and water located in the State of Florida is listed below:
Non-Hazardous PFAS/PFOS Impacted Material:
Description Rate Unit
Profile Fee (Charged once per site) $ 350.00 Per Site T&D - Non-Haz PFOS/PFAS Impacted Soil/ Mud/Water $ 195.00 Per Drum
Non-Hazardous Material not Impacted with PFAS/PFOS:

Please ensure that all drum lids are properly secured and that drums are on a level surface like the surrounding area (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.
The prices listed are all inclusive without any additional fees or charges (fuel surcharge, low volume stop fee etc.) and are valid for all regions in the State of Florida except for the Florida Keys. Pricing for sites located in the Florida Keys will be bid on a case-by-case basis dependent on the volume of drums and the location.
This quote is valid for 90 days.
