About this file
This file is an Attachment J document that outlines a Full Service Partnership and Outpatient Continuum feedback loop process for mental health or behavioral health services delivery. The document describes a three-stage referral and service coordination workflow involving a Referral Center, participating agencies, and clinical providers. The referral process begins when a Referral Center receives a referral, identifies an appropriate program and agency, and submits the referral through MyAvatar. The receiving agency must contact the parent or caregiver to offer a first appointment within 10 business days. If the parent or caregiver cannot be reached after three phone attempts or declines services, the agency documents the disposition and notifies the referral source. The clinical provider then conducts a comprehensive assessment, reviews recommendations with the parent or caregiver, obtains necessary releases of information for care coordination, and transitions the client to treatment services while providing additional linkages and referrals as needed.
The feedback loop process includes clinical providers sending secure communications to referring sources to close the referral cycle. For Primary Care Physicians (PCPs), the process includes sharing the full assessment report with the diagnosis page unless contraindicated, along with status updates regarding service level changes, provider changes, discharge, or additional services. This structure ensures continuous coordination and information sharing between the referral source, service provider, and family throughout the service delivery continuum.
