About this file
This is a California Public Records Act (CPRA) confidentiality designation form for the County of Santa Clara's Behavioral Health Services Department in response to RFSQ-BHS-FY25-0066 for Children, Youth, and Family System of Care Services. The form permits offerors to request that specific sections of their proposal responses be designated as confidential under CPRA exemptions outlined in Government Code section 6250 et seq. Offerors must identify the exact page numbers and sections they wish to keep confidential and provide a legal basis for each exemption request. The form emphasizes that blanket designations of entire responses as "confidential" or "proprietary" are unacceptable, and any sections without a legally valid exemption explanation will be disclosed by the County in accordance with applicable law.
The form provides a structured table with eleven numbered line items for offerors to list confidentiality requests, with columns for page number, section identification, and explanation or legal basis for the exemption claim. Offerors must sign and date the completed form with an authorized signature and include the offeror name and printed name of the signatory. If an offeror is not requesting any confidentiality exemptions, they may check a designated box to indicate this election, though the County will treat any response lacking listed exemptions as non-confidential regardless of the box status.
