About this file
This is an Appendix A cover sheet and qualification form for a Request for Statements of Qualification (RFSQ) issued by the Santa Clara County Behavioral Health Services Department (BHSD) for Children, Youth, and Family System of Care Services (RFSQ-BHS-FY25-0066). The document outlines application pathways for two categories of providers: existing BHSD-contracted providers seeking to continue or expand current services (Section I), and new providers or existing providers expanding into new service areas (Section II). The RFSQ encompasses 19 distinct service programs organized into seven categories: Crisis Services (Mobile Response and Stabilization Services, Post Crisis Stabilization Services); Full Service Partnership Services (Youth and Transitional Age Youth programs); Outpatient Services (School-Based, Early Psychosis Intervention, general Outpatient Continuum, Specialty Outpatient, Early Childhood Mental Health, Transitional Age Youth, LGBTQ+, and Cross-Systems programs); Prevention and Early Intervention (School-Based Early Intervention and Community Youth Wellness Centers); Behavioral Health Placement Support Services (Therapeutic Behavioral Services and services for youth with intellectual or developmental disabilities); Residential Services (Short-Term Residential Therapeutic Program and Youth Crisis Residential); and Substance Use Treatment Services (Youth Outpatient and Residential programs). Services are available countywide or in specific regions including North County, Central County, East County, and South County, with school-based programs offered through designated school districts.
The document requires applicants to provide financial stability documentation, corrective action plans or performance improvement plans from the past five years, and staffing information related to current or similar programs. For residential programs (A16, A17, and A19), facilities must be located in Santa Clara County or within a 90-mile radius and possess necessary licensing before contract execution. Applicants must complete detailed certification statements confirming they have read the RFSQ, understand its requirements, possess capability to execute the proposed services, and commit to furnishing goods and services as specified. The form requires authorization signatures from authorized representatives of applicant organizations, with specific signature blocks provided for individuals, corporations, and other entity types. No response will be accepted without proper authorized signatures executed in ink.
