About this file
This is an Addendum #1 issued on October 18, 2024, by the County of Santa Clara for RFSQ-BHS-FY25-0066 regarding Children, Youth, and Family System of Care Services. The addendum incorporates revisions to the original RFSQ and Appendix A-Cover Sheet. Current Community Care Partners (CCPs) applying to continue offering existing programs must submit Sections I and III of Appendix A, financial stability documentation, information on Corrective Action Plans (CAPs) and Performance Improvement Plans (PIPs) from the past five years specifically related to programs within the Behavioral Health Services Department (maximum three pages per program), and staffing information including position titles, language proficiencies, hiring processes, program closure history, and direct service staff vacancy rates over the past five years (maximum two pages per program). The revised Offeror's Conference date is Thursday, October 24, 2024. Current CCPs seeking evaluation must submit required documentation before the final proposal deadline according to Table 5 special deadlines in Section II.A. CCPs applying for new or additional programs beyond their current scope must follow the full evaluation process outlined in Section IV.
All submissions must comply with Response Format guidelines in Section IV.C and be compiled into a single PDF file using the naming convention "[Your Organization Name] - CCP Evaluation Submission.pdf." Applicant organizations must agree to the Scope of Work provided by the County for respective programs, submit financial stability documentation as required in RFSQ Section IV.K.1, and provide CAP/PIP information and staffing documentation as specified. The addendum clarifies that updated instructions for Appendix A now include guidance for indicating regions, specific school districts, or specialty populations for current services, with color highlights to identify new regions, districts, or populations.
