About this file

This document is a Quality Assurance Surveillance Plan (QASP) for base operations and maintenance services at Fort Huachuca, Arizona. The QASP outlines performance standards and inspection procedures to evaluate a contractor's performance in areas such as service orders, preventive maintenance, supply operations, and plant operations. Acceptable performance is defined as maintaining standards below the Minimum Acceptable Standards of Performance in each functional area. Inspections will utilize random and planned sampling methods. Deficiencies may result in unsatisfactory inspection reports, contract discrepancy reports, or invoice deductions up to specified percentages for the associated contract line items when minimum standards are exceeded. The QASP details inspection checklists, sampling guides, and procedures for deductions to encourage contractor accountability and continuous quality improvement in fulfilling the requirements of the performance work statement.

QUALITY ASSURANCE SURVEILLANCE PLAN

(QASP)

FORT HUACHUCA, ARIZONA

CONTRACT NUMBER

W9124A-xx-C-xxxx

Nov 14, 2023

QUALITY ASSURANCE SURVEILLANCE PLAN

Directorate of Public Works

This Quality Assurance Surveillance Plan (QASP) is not part of the Request for Proposal or Invitation for Bids, nor will it be made part of any resulting contract. It is provided so that the Contractor is aware that the Government will perform Quality Assurance (QA) on contract services.

A. Introduction:

1. This QASP implements the Government Quality Assurance Program. It is designed to help the Contracting Officer's Representative (COR) effectively monitor and evaluate the Contractor’s performance. Its intent is to facilitate Contractor accountability and encourage the Contractor to control and continuously improve quality. The QASP provides for monitoring and surveilling through a combination of methods including:

a. Random and Planned Sampling.
b. 100 Percent Inspection (Inspection Checklists 2 & 5 Only).
c. Inspection Checklists.
d. Management Information System (MIS) Surveillance.
e. Customer Complaints.
f. Observations.
g. COR observations.
h. Unscheduled Inspection.

2. The Government is primarily focused on the final product or service the Contractor provides and not on the details of the Contractor’s methods of work.

a. Random Sampling is the principal method of surveillance. Sampling Guides are included in this QASP.

b. When Contractor actions are critical in nature, a 100% Inspection may be used as the method of surveillance.

c. A Surveillance Inspection Checklist may be used for surveillance of Contractor actions that are required in the Performance Work Statement (PWS) but not listed in the Performance Requirements Summary (PRS). This method of surveillance may also be used for in-depth surveillance of Contractor actions listed in the PRS when the Government suspects problems or has reason to be concerned. A Surveillance Inspection Checklist may be used for Unscheduled Inspections.

d. The General Fund Enterprise Business System (GFEBS) is the Army’s system of record and the data contained therein shall be used by the Government to evaluate the Contractor’s performance. Therefore, it is incumbent upon the Contractor to ensure that data is accurately entered and maintained in GFEBS per the PWS. Evaluating the Contractor's ability to use GFEBS will be accomplished using random, planned, or unscheduled surveillance methods.

e. The Government has the right to inspect and test all the services performed by the Contractor, to the maximum extent practicable, during the term of the contract. Therefore, observations of the Contractor’s work by a Government Quality Assurance Evaluator (QAE) will provide an additional basis for evaluating the Contractor's performance.

f. Observations of Contractor activities by the COR will also be used to evaluate of Contractor performance.

B. How to Use the Quality Assurance Surveillance Plan:

1. This plan has been developed for use by the COR, through QAEs, to evaluate Contractor performance. The COR shall accomplish this by assigning specific duties to each QAE. It is also the COR’s responsibility to ensure that Inspection Checklist inspections are conducted as required and that Customer Complaints are investigated.

2. Methods of Surveillance.

a. Random/Planned Sampling:

(1) Random Sampling: The QAEs will query GFEBS for Contractor completed work and assign a sequential number to each work document line item. Then, using the random table from ANSI/ASQC Z1.4-2003 or equivalent, based on population, determine which line items are to be inspected. The results will be recorded and maintained.

(2) Planned Sampling: The QAEs will select work to be inspected based on varying subjective criteria such as the specific type of work or the facility affected. Planned sampling may also involve only specific features of the work.

(3) For each inspection cycle, the tally of observations and defects shall be compared to the accept/reject criteria in the Sampling Guide.

(4) An auditor should be able to track from the surveillance activity to the Contracting Officer's actions described in a Contract Discrepancy Report (CDR).

(5) Unsatisfactory Inspections: Whenever an inspection result is unsatisfactory, an Unsatisfactory Inspection Report (UIR) shall be prepared by the QAE.

(6) If the Sampling Guide’s Minimum Acceptable Standards of Performance (MASP) is exceeded, the Contractor's performance is unsatisfactory for that cycle in the applicable functional area.

(7) If performance for any functional area is determined unsatisfactory for the lot, the COR shall generate a Contract Discrepancy Report (CDR).

(8) The CDR, along with the Inspection Checklist, is the documentation required to support a contract action.

(9) Any deficiencies or defects found during any surveillance activity, even if they do not render the service unsatisfactory, shall be corrected by the Contractor at no cost to the government.

b. 100 Percent Inspection (Inspection Checklists 2 & 5 Only)

c. Inspection Checklists:

(1) An Inspection Checklist shall be the basis for, and the record of, an inspection. The QAE will inspect each Contractor action for that PRS line item and enter the results on the Inspection Checklist. The QAE will also prepare an Unsatisfactory Inspection Report (UIR) for each unsatisfactory inspection.

d. MIS Surveillance: The QAE will use GFEBS to perform the inspection and enter the results of the inspection on the Inspection Checklist and determine if the inspection is satisfactory or unsatisfactory.

e. Customer Feedback: Customer feedback is an important method of detecting both satisfactory and unsatisfactory Contractor performance.

(1) The COR may receive feedback from customers about the quality of service provided by the Contractor. Complaints must be investigated and validated or rejected. If a complaint is found to be valid, that validation activity will be considered an unscheduled inspection, but will not a factor in determining whether the Contractor has exceeded the MASP for the inspection cycle.

f. Observations: The COR and QAE’s will observe Contractor performance on a regular basis as part of their normal work activities. Such observations may be documented and reported as Unscheduled inspection reports and the Contractor will be informed of these observations. These observations will not be counted toward the MASP for that inspection cycle.

3. Contractor Performance:

a. Good Performance: When a Contractor's Quality Control Program works, good performance results. If the results of the COR's surveillance shows consistently good performance, the amount of surveillance can be decreased.

(1) Reduced Inspection: Reduced inspection can be used when the following conditions have been met for a Sampling Guide:

(a) The preceding 4 lots, i.e. last 4 inspection cycles have been acceptable.

(b) The number of defects in each of the preceding 4 lots is less than one half of the acceptance number. For example, with a MASP of 10 percent and a sample size of 30, the acceptance number is 5, if two or less defects were found in each of the last 4 lots, reduced inspection could be used.

(c) The normal sample size is being used.

(d) The Contracting Officer and the COR agree to use reduced inspection.

(2) Reduced Sample Size and Acceptance/Rejection Numbers: Reduced inspection decreases the sample size. In addition, the acceptance and rejection numbers may change. In order to make the changes to the existing Sampling Guide, the following steps shall be used:

(a) Find the new sample size. Take the lot size and find the reduced sample size using the appropriate table in ANSI/ASQC Z1.4-2003 for reduced sampling.

(b) Use the MASP in the Sampling Guide and the new reduced sample size for the new acceptance and rejection numbers. Note that there is a gap between the acceptance and rejection numbers, e.g. sample size is 20 and MASP is 10 percent, accept is 5 and reject is 8, this means that the lot would not be rejected unless 8 defects were found. However, a number of defects greater than 5 will be cause for returning to normal inspection, e.g. return to the sample size and acceptance and rejection numbers used in the original Sampling Guide.

(3) Returning to Normal Inspection: When reduced inspection is in effect, return to normal inspection the next lot under the following conditions:

(a) When the number of defects exceeds the acceptance number under reduced sampling, or,

(b) The Contracting Officer and the COR deem it necessary to return to normal inspection.

(4) Returning to Reduced Inspection: If during the first month normal inspection has been resumed, the number of defects again found is less than 50 percent of the reject level, a return to reduced inspection may be done the next inspection cycle. If the number of defects is over 50 percent of the reject level, then normal sampling must be accomplished until 4 inspection cycles of less than 50 percent of reject level defects is found.

b. Poor Performance: When a Contractor's Quality Control Program is not working, poor performance is the result. If the COR's surveillance shows consistently poor performance, the surveillance criteria may be tightened.

(1) Tightened Inspections: Tightened inspection can be used when the following conditions have occurred:

(a) The MASP has been exceeded for two out of three consecutive inspection cycles.

(b) The normal sample size is being used.

(c) The Contracting Officer and the COR agree to use tightened inspection.

(2) Tightened Sample Size and Acceptance/Rejection Numbers: Tightened inspection does not increase the sample size; however, the acceptance and rejection numbers do change. In order to make the changes to the existing Sampling Guide, use the MASP in the Sampling Guide to determine the new acceptance and rejection numbers from ANSI/ASQC Z1.4-2003 from the tables for increased inspection.

(3) Returning to Normal Inspection: When tightened inspection is in effect, return to normal inspection the next inspection cycle under the following conditions:

(a) The number of defects falls within the acceptance range under tightened inspection for five consecutive inspection cycles.

(b) The Contracting Officer and the COR approve returning to normal inspection.

c. When a defect is found, the QAE performing the inspection shall contact the Contractor's shop lead or manager of that functional area and inform the shop lead or manager of what was found to be defective.

d. If the COR's surveillance shows that the Contractor's performance is unsatisfactory (MASP exceeded), as described in this plan, all documentation supporting this decision must be sent to the Contracting Officer within three working days of the end of the surveillance period ends.

e. The COR will record all surveillance observations, inspection results, and actions taken on the appropriate documents. These documents shall be stored in an established contract file for the term of the contract.

f. When the COR observes that the Contractor's work is unsatisfactory, the COR must take action to encourage the Contractor to correct the unsatisfactory performance. The most common actions available to the COR are:

(1) Verbal Unwritten Communications with the Contractor, normally the Contractor's Project Manager, for minor discrepancies which do not cause delay, prevent or hinder an operation or a service, which can be corrected, onthespot, within a few minutes without additional parts or materials. The COR will make a Memorandum for Record of the discrepancy for the contract file and will furnish a copy of the Memorandum for Record to the Contracting Officer. Repeated minor discrepancies may warrant issuance of a CDR.

(2) Written Communications with the Contractor, normally the Project Manager, for repeated minor discrepancies. The COR will place a copy of the communications in the contract file and will provide an information copy to the Contracting Officer. Continued minor discrepancies may warrant issuance of a CDR.

(3) Unsatisfactory Inspection Report: An UIR will be sent to the contract Project Manager for each inspection of services which results in a failed inspection. The report will be sent by the COR within two working days following the date of the inspection. The UIR will serve as formal notification to the Contractor of the deficiency. The Contractor will return the UIR to the COR within two working days detailing the Contractor's actions to correct the unsatisfactory condition.

(4) Contract Discrepancy Report: When the Contractor's performance of work is unsatisfactory, the COR will issue the Contractor a CDR with a copy furnished to the Contracting Officer. The COR will issue the Contractor a CDR when:

(a) The UIR exceeds the MASP in any functional area.

(b) Necessary to correct deficiencies in Contractor's performance not directly related to inspections.

The COR will prepare and forward the CDR to the Contractor, the Contractor shall correct the discrepancy, if possible, and enter the corrective actions taken on the CDR, enter what actions the Contractor will take to prevent recurrence of the deficiency in the future, and return the CDR to the COR with copy to the Contracting Officer.

(5) Contracting Officer's Monthly Report: In a Fixed Price contract, the COR provides a monthly report to the Contracting Officer on the Contractor's performance. The COR will base the monthly report on the observed performance of Contractor actions as surveyed by Random/Planned Sampling, 100 percent review and 10 percent Inspection (inspection checklists # 2 & 5 only), Surveillance Inspection Checklists, Customer Complaints, and the COR's personal observations. The COR's report will be based on a descriptive evaluation provided by the QAE for the QAE's functional area(s) of responsibility, at the end of each month.

(6) Reductions in Invoice Payments: Payments to the Contractor will be reduced for unsatisfactory performance using the following methods:

(a) Contractor performance will be compared to PRS and MASP using the Government Quality Assurance Surveillance Plan.

(b) If the performance in any required service is unsatisfactory, and the poor performance is clearly the fault of the Contractor, an amount of money up to the percentage cost of the service, as stated in the PRS, will be withheld.

(c) The amount of money to be permanently deducted for unsatisfactory service will be computed as follows:

Example

If: Quality of completed work is unsatisfactory (MASP of 4.0 percent exceeded).

And: A=Monthly CLIN Invoiced amount is $100,000.

And: B=Deduction percentage is 10 percent.

And: C=Sample size is 20 units. (Based on AQL, sample size code letter is F)

And: D=Number of defects in the sample is 3 (Reject number is 3).

And: E=3 represents .15 percent of the sample size. (3 divided into 20 = .15)

Then: Deduction from the current month’s CLIN invoice is:

Formula = A x B x E = Deduction

Example:

Monthly CLIN Invoiced price (A)$100,000
X deduction percentage (B).10
$ 10,000
X Representative Units defective (reject) (E).15
Deduction$ 1,500

(d) The deduction will be taken from the current monthly CLIN amount invoiced as the CDR/deduction is reviewed and found to be valid.

(7) Re-Performance: The Contractor will be required to re-perform work, tasks or services previously provided to the Government that fall below the MASP of performance, as determined by any inspection method listed in this QASP. Such re-performance will be at no additional costs to the Government. Any re-performance requires re-inspection by the Government to ensure that the minimum acceptable standards have been achieved or that demonstrate compliance with the requirements of the PWS.

C. Revisions to this surveillance plan are the joint responsibility of the QAEs, the COR, and the Contracting Officer.

D. The rights of the Government and remedies described in this QASP are in addition to all other rights and remedies set forth in this solicitation. Specifically, the Government reserves its rights under the Inspection of Services clause and the Termination for Default clause. Any deductions pursuant to the PRS shall reflect the reduced value of services performed hereunder. The Contractor shall not be relieved of the responsibility for full performance of services hereunder.

Performance Requirements Summary

1. The purpose of this summary is to:

1.1. List the contract requirements considered most critical to acceptable contract performance.

1.2. List where the standard for each requirement is located in the PWS.

1.3. Show the maximum allowable degree of deviation from acceptable performance (AQL) that shall be allowed by the Government before contract performance is considered unsatisfactory.

1.4. List the inspection methods that the Government will use to evaluate the Contractor's performance in meeting the contract requirements.

1.5 List the deduction factor for each item that the Government will use in computing the deduction for unsatisfactory service.

2. The Government's primary quality assurance procedures are based on random and planned sampling of the recurring critical output products of the contract using the concepts of ANSI Z1.4-2003. Most contract requirements will be surveilled by random or planned inspections of Contractor work. Some contract requirements will be surveilled by use of contractor reports and by computer operations on data input into the Government Information System. Additionally, some areas will have 100 percent of the work inspected. Other areas will be reviewed periodically, i.e., monthly, quarterly, semiannually, etc.

3. The criteria for acceptable and unacceptable performance are derived from MIL STD 105e based on the lot size, sample size, and AQL for each contract requirement. When the number of defects in the Contractor's performance exceeds the acceptable level, the Contractor's performance is unsatisfactory.

GENERAL NOTES:

1. The percent (%) deduction listed is the percentage that may be deducted (for any work/action that can't be reperformed) from the corresponding contract line item (CLIN) to that service. Such deduction will be from the invoiced amount of the specified CLIN, for the month involved, where evidence of the Contractor’s failure to meet the minimum acceptable standards of performance listed in the PWS have documented.

2. Where deduction column is marked with (**) the timeliness of the performance is based on the overall effect that any delay has on the timely completion of the work involved. Note that planning, estimating, scheduling, dispatching, and work performance reporting are listed as component elements of various required services and are also listed separately, to reflect that the Government may inspect them for compliance with PWS and may require re-performance, should the minimum standards of performance not be achieved.

3. Chemicals must be maintained IAW all State, Federal and Local laws and regulations.

4. Failure to utilize the properly certified /qualified personnel to perform tasks shall be subject to re-performance.

5. Supply inventory shall be performed IAW the PWS and applicable Army Regulations.

6. Late submission of Deliverables may delay payment of billing invoices.

7. Re-performance of work that did not meet the MASP will be at no additional cost to the Government.
