About this file
This document is a TSA Form 14XX Media Sanitization Certificate template used by the Transportation Security Administration to document the proper disposal and sanitization of computer media and hardware. The form is completed by the individual performing media sanitization and must be submitted to the Accountable Property Officer (APO).
Section I requires contact information including the name, site code, and phone number of the person performing the sanitization. Section II captures hardware information including CPU barcode number, CPU serial number, hard drive serial number with make and model, and unit serial number with model. Section III requires identification of the media classification level—Unclassified, Sensitive But Unclassified, or Classified (Secret or Above)—with a note that classified media may only be reused at the current classification level or higher. Section IV contains certification checkboxes confirming that the sanitization was performed according to TSA MD 1400.3 Information Security Policy's Information Security Policy Handbook, that all overwritable areas were sanitized, that labels were removed or obliterated, and that a completed TSA Form 14XX indicates the disposition method (Degaussed, Destroyed, Overwritten, or Removed). The form requires signatures and dates from both the individual performing the sanitization and a witnessing government official, including their respective site codes and phone numbers.
