About this file
This is an RFP Submission Checklist designed to guide contractors in preparing complete and compliant proposals for a federal solicitation.
The checklist is organized into three proposal volumes. Volume I (Business Proposal) requires a table of contents, cover page with standard identifying information, executed Solicitation/Offer/Award form, representations and certifications, make-or-buy programs if applicable, waiver requests, deviation summaries, Attachment 6 (Limitations on Subcontracting), and acknowledgment of six amendments. Volume II (Technical Proposal) requires a table of contents and cover page with identifying information, followed by evaluation factor responses including Attachment 7 (Risk Assessment/Value Add) and Attachment 8 (Past Performance Questionnaire). Volume III (Price Proposal) requires a table of contents and cover page, with pricing consistent with the technical approach, breakdown of Section B contract schedule showing proposed prices for each contract line item number including options, and unit prices if required, with a total proposed price excluding options. All price amounts must be expressed to the nearest whole dollar except for hourly labor rates, and percentages to two decimal places. No cost information is permitted in Volumes I or II. The document emphasizes that all proposals must include a 90-day validity statement and note that the checklist itself should not be submitted with the proposal.
