INTRODUCTION

TEC-AN, Inc. was assigned by Spur Design a mold inspection of the renovation area on the 7th floor and addition of the 8th floor of the Oklahoma Veterans Administration Hospital located 921

N.E. 13th Street, Oklahoma City, Oklahoma.

In a meeting and walk through of the 7th and 8th floor with Seth Cavin, principle of Spur Design and Joyce Filsinger, Chief of Projects for the VA concerning the mold inspection. The general discussion related to the length of time the project has been sitting dormant and the General

Contractor’s lack of adherence to the drawings and specifications for the 8th floor SICU addition.

The contracts with the 7th and 8th floor contractors were terminated in 2016 by the VA.

Part of the 7th floor OR expansion and all the 8th floor SICU addition were stopped midway during construction due to the use of non-specification materials, equipment and not utilizing proper installation activities. The stoppage occurred in 2016 and little or no construction activities have occurred to date. Both the partial renovated area of the 7th and all the 8th floor have no conditioned air system operational currently.

Several of the windows on the 8th floor leaked and there has been some attempt to seal out the water intrusion. The 8th floor roof has had several leaks which have been sealed, however the roof leaks continue to occur.

TEC-AN, Inc. conducted an additional mold inspection on June 14th, 2020 with the collection of sheetrock wall plug samples primarily under the windows of the 7th and 8th floors. To complete the inspection an additional site visit occurred on June 29th, 2020 to collect tape lift samples and mold air samples from the 7th and 8th floors.

The plug samples, tape lift samples and air samples were analyzed by Quantem Laboratories of

Oklahoma City, Oklahoma with results in the body of this report.

INSPECTION OBSERVIATIONS

A mold inspection was conducted by Mold Inspection & Testing (MI&T) from December 9th, 2016 through December 12th, 2016. In review there looks to have been several locations on the

7th floor where the molded sheetrock was removed and disposed. With the original paper backed fiberglass insulation left in place.

7th Floor OR Expansion

The Mold Inspection conducted by Mold Inspection & Testing (MI&T) contained pictures of molded sheetrock walls and wall insulation on the 7th floor. However, the only issue noted in the report was inspecting the 7th floor after a pipe rupture with a note of no mold was visible.

During the TEC-AN, Inc. inspection it was noted there was no noticeable molded sheetrock walls and, in several areas, there was paperbacked fiberglass insulation in the open wall areas.

The paperbacked fiberglass insulation appeared to be the original insulation used during renovation.

There is a considerable amount of building materials stored throughout the 7th floor which appears to have been placed to be installed by the original contractor.

8th Floor Surgical Intensive Care Unit

The windows showed signs of water intrusion on the sills, sides and the top between the window and the building window openings. There was water staining on the wood used as window spacers between the window and the building window opening. It was noted a caulking was applied in the gaps to try and stop the water intrusion.

Several exterior metal windowsills were too wide, and the contractor cut the excess off and caulked the edges. This did not stop the rainwater from blowing into the space between the sill and the window opening.

Water staining was seen on the fire proofing, on unfinished floors and on interior sheetrock throughout the 8th floor. It was noted there are several openings through the roof deck to the exterior of the building.

There is a considerable amount of building materials stored throughout the 8th floor which appears to have been placed to be installed by the original contractor.

Roof

The following are several problems observed while inspecting the roof:

1. Roof drains are not resisted to allow water to drain properly which leaves a substantial amount of rainwater to stand on the roof.

2. The roofing material is of substandard quality and is showing signs of weathering and

UV deterioration.

3. Several areas the metal cap on the parapet wall were too wide and allows rain under the cap potentially migrating down into the parapet wall and under the roofing material.

VA REPRESENTATIVE INTERVIEW

Gary Cook and Grayson Cook of TEC-AN, Inc. met with Ms. Joyce Filsinger, Chief of Projects, for the Oklahoma City VA and Mr. John Wilson, COR, for the Oklahoma City VA to review some findings and address questions concerning the mold remediation and the time line for re-starting the construction project for completion.

Ms. Filsinger stated there has been removal of sheetrock walls with mold staining on the 7th floor. Gary Cook indicated the paperbacked fiberglass insulation has not been removed and the wall space has not been cleaned.

Ms. Filsinger and Mr. Wilson indicated the 8th floor has a considerable amount of rainwater intrusion from the windows and non-weatherproofed areas of the roof penetrations. Throughout the 8th floor there is stored building materials, such as sheet rock, which has been wet several times over the past four years

Ms. Filsinger and Mr. Wilson are concerned about the potential of finishing the new operating rooms on the 7th floor and the new SICU on the 8th floor while showing signs of considerable amount of mold contamination. Gary Cook questioned the estimated time before construction would resume. Both indicated there is several steps including the development of the overall scope, design and bid documents to be compiled and the selection of the contractor thus making the potential start date eighteen months out.

Eighteen months would mean a minimum of six season changes and the 7th and 8th floors would be directly impacted by temperature fluctuations and additional water intrusion of the seasons.

As there is no conditioned air system working in these spaces, this will enhance unmitigated mold growth.

FINDINGS AND RESULTS

Listed below are the laboratory results by floor consisting of the plug samples, tape lift samples and the air monitoring samples. All areas of the 7th floor OR expansion and the new construction on the 8th floor SICU have signs of mold contamination.

This section will cover the different types of samples collected and the results.

Mold Plug Samples – There were ten mold plug samples collected from sheetrock walls under the windows along the south and west exterior walls of the 7th floor. Seven (7) of the ten (10) plug samples collected tested positive for mold.

RECOMMENDATIONS

1. Monitor all areas of the 7th and 8th floor and take the necessary steps to minimize mold growth until a new start date has been determined.

2. Repair all windows to assure a watertight seal.

3. Inspect and remove all building materials showing signs of mold contamination stored on the 7th and 8th floors.

4. Repair or replace roof to ensure all leaks have been stopped.

5. Remove all sheetrock walls and paperbacked fiberglass from the targeted area and clean the wall space with a biocide.

6. Replace the sheetrock walls with a mold resistance sheetrock wall.

7. Paint surfaces with a mold killing primer before finish paint is applied.

REMEDIATION PROCEDURES

1. All employees conducting remediation are to don the proper PPE.

2. Place HEP equipped air scrubbers in the remediation area.

3. Remove sheetrock, paperbacked fiberglass insulation, ducting and related waste.

4. Place waste in a 4-mil poly disposal bag, twist the bag opening and tape closed or wrap the waste in a 4-mil poly sheet and tape closed all the edges.

5. Transport all waste in a poly cart draped with 4-mil poly sheet.

6. Determine the best way to transport the waste out of the hospital.

7. Transport waste during the less active times in the hospital to transport waste.
