About this file
This is a Fugitive Dust Control Plan Form used at Dugway Proving Ground for managing air quality compliance related to vehicle traffic on unpaved roads and parking areas. The form applies to vehicle travel and parking on unpaved routes, including construction-related traffic on interior and access roads, and requires completion by persons conducting these activities.
Contractors must select at least one Best Management Practice (BMP) from six options to stabilize surface soils where support equipment and vehicles operate: limiting vehicle mileage and speeds (BMP 18-01), applying and maintaining water on surface soils (BMP 18-02), applying and maintaining chemical stabilizers (BMP 18-03), applying and maintaining gravel (BMP 18-04), supplementing chemical stabilizers, water, or aggregate applications as necessary (BMP 18-05), or applying recycled asphalt to surface soils (BMP 18-06). If dust suppression is used, a logbook must be maintained documenting the date, time, type of suppression, amount in gallons, purpose, and location applied. The form requires general information including location, start and end dates, point of contact, email, and phone number. Contractors must comply with Utah Administrative Code R307-205-6(2) regarding trackout prevention by preventing material deposits on paved roads except at designated sites and promptly cleaning roads when materials are deposited. Completed forms and applicable logbook entries must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil upon completion of operations or annually by calendar year-end, whichever occurs first.
