About this file
This is a Fugitive Dust Control Plan form template used by Dugway Proving Ground for managing air quality during aggregate material storage operations. The form (FDCP-03) applies specifically to stockpiles of soils, rocks, and debris materials stored on-site for future use or export.
The form requires completion of general information including location, dates, point of contact details, and site description. Operators must select at least one best management practice (BMP) from three critical mitigation areas: stabilizing surface soils where equipment operates, stabilizing stockpile materials during handling, and stabilizing stockpiles after handling. BMP options include pre-watering, chemical stabilizers, paving, material removal from downwind sides, height reduction, wind screens, watering to form crusts, covers, and wind barriers. For operations using chemical stabilizers, the specific chemical name must be identified. A logbook is mandatory for all stockpiling activities, documenting minimum information of dates and quantities of materials added or removed, and if suppression methods are used, additional entries must include date, time, suppression type, amount in gallons, purpose, and location applied. The form includes trackout prevention requirements per Utah Administrative Code R307-205-6(2), requiring operators to prevent material deposition on paved roads and promptly clean any contaminated surfaces. Completed forms and logbook entries must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil upon operation completion or annually by calendar year-end, whichever occurs first.

Dugway Proving Ground Fugitive Dust Control Plan Form 03 Storage, Hauling and Handling of Aggregate Material
Stockpiles (Storage) of Aggregate Materials for Future Use or Export
Activity Description: A large accumulated pile of materials, soils, rocks, debris that are being stored on site intended for use or awaiting transport to another location.
General Information
Location(s): Start Date: End Date:
Point of Contact: Email: Phone Number:
Description of site:
Fugitive Dust Emission Control Techniques Select at least one best management practice (BMP) for each critical mitigation area:
Stabilize surface soils where support equipment and vehicles will operate
BMP 03-01 Pre-water and maintain surface soils in a stabilized condition.
BMP 03-02 Apply and maintain a chemical stabilizer to surface soils. (Name of chemical: ) BMP 03-03 Pave area(s).
Stabilize stockpile materials during handling
BMP 03-04 Remove material from the downwind side of the stockpile, when safe to do so.
BMP 03-05 Reduce height.
BMP 03-06 Create wind screen.
Stabilize stockpiles after handing
BMP 03-07 Water stockpiles to form a crust immediately.
BMP 03-08 Apply and maintain a chemical stabilizer to all outer surfaces of the stockpiles. (Name of chemical: ) BMP 03-09 Provide and maintain wind barriers on 3 sides of the pile.
BMP 03-10 Apply a cover (natural or synthetic).
BMP 03-11 Wind screen.
BMP 03-12 Avoid steep sides to prevent material sloughing.
BMP 03-13 Reduce height.
Logbook Requirement If aggregate materials are being stockpiled, a logbook is required, at minimum, it must contain: the date and amount(s) of aggregate material added or removed to the pile. If suppression is used, additional logbook entries are required, at minimum, entries must contain: the date, time, type of suppression used (i.e. water/chemical suppressant), amount (in gallons), purpose and location applied.
Trackout Prevention In accordance with Utah Administrative Code R307-205-6(2), any person(s) performing fugitive dust emission activities will to the maximum extent possible, keep material from being deposited onto any paved road other than a designated deposit site. Any such person(s) who deposits materials that may create fugitive dust on a paved road shall clean the road promptly.
*By signing you attest that you have read and understand the requirements of the DPG FDCP and will follow the BMPs you have selected.
Signature Date This form is to be completed and kept on record according to the requirements found in the Dugway Proving Ground (DPG) Fugitive Dust Control Plan (FDCP). Copies of form(s) and corresponding logbook entries (if applicable) shall be provided to EP, Air Program Manager, upon the completion of operation(s) and/or annually at the end of each calendar year whichever occurs first. Submit completed form and applicable logbook entries in hardcopy or electronically, via email to Environmental Program’s (EP) Air Program Manager: boyd.roberts.civ@mail.mil
