About this file
This is a Fugitive Dust Control Plan form used by Dugway Proving Ground for construction and demolition activities involving backfilling of previously excavated or trenched areas. The form documents the implementation of best management practices (BMPs) to minimize fugitive dust emissions during backfill operations.
The form requires contractors to select at least one BMP from four critical mitigation areas: stabilizing backfill material when not actively handling (BMPs 06-01 through 06-03), stabilizing backfill material during handling (BMPs 06-04 through 06-08), stabilizing soil at completion of backfilling (BMPs 06-09 through 06-10), and stabilizing material while using pipe padder equipment (BMPs 06-11 through 06-13). BMPs include techniques such as watering materials to maintain moisture, applying chemical stabilizers, covering materials, controlling drop heights during loading, and mixing moist soil with dry soil to achieve optimum moisture. If suppression methods are used, contractors must maintain a logbook documenting the date, time, type of suppressant (water or chemical), amount in gallons, purpose, and location of application. The form also requires compliance with Utah Administrative Code R307-205-6(2) regarding trackout prevention, mandating that materials not be deposited on paved roads except at designated sites, with prompt cleanup of any deposited materials. Completed forms and corresponding logbook entries must be submitted to the Environmental Program's Air Program Manager at boyd.roberts.civ@mail.mil upon operation completion or annually by calendar year-end, whichever occurs first.

Backfilling Area Previously Excavated or Trenched Activity Description: The process to refill an area(s) in which material has been removed.
General Information
Location(s): Start Date: End Date:
Point of Contact: Email: Phone Number:
Description of site:
Fugitive Dust Emission Control Techniques Select at least one best management practice (BMP) for each critical mitigation area:
Stabilize backfill material when not actively handling.
BMP 06-01 Water backfill material to maintain moisture or to form crust.
BMP 06-02 Apply and maintain a chemical stabilizer to backfill material to form crust.
BMP 06-03 Cover (natural or synthetic) or enclose backfill material when not actively handling.
Stabilize backfill material during handling
BMP 06-04 Empty loader bucket slowly and minimize drop height from loader bucket.
BMP 06-05 Dedicate water truck or large hose to backfilling equipment and apply water as needed.
BMP 06-06 Mix moist soil with dry soil until the optimum moisture is reached.
BMP 06-07 Apply and mix water into the backfill material until optimum moisture is reached.
BMP 06-08 Apply and mix water and chemical solution into the backfill material until optimum moisture is reached.
(Name of chemical: )
Stabilize soil at completion of backfilling activity BMP 06-09 Apply water and maintain disturbed soils in a stable condition.
BMP 06-10 Apply and maintain a chemical stabilizer on disturbed soils to form a crust. (Name of chemical: )
Stabilize material while using pipe padder equipment BMP 06-11 Mix moist soil with dry soil until the optimum moisture is reached.
BMP 06-12 Dedicate water truck or large hose to equipment and apply water as needed.
BMP 06-13 Not applicable
Logbook Requirement If suppression is used, a logbook is required, at minimum, it must contain: the date, time, type of suppression used (i.e. water/chemical suppressant), amount (in gallons), purpose and location applied.
Trackout Prevention In accordance with Utah Administrative Code R307-205-6(2), any person(s) performing fugitive dust emission activities will to the maximum extent possible, keep material from being deposited onto any paved road other than a designated deposit site. Any such person(s) who deposits materials that may create fugitive dust on a paved road shall clean the road promptly.
*By signing you attest that you have read and understand the requirements of the DPG FDCP and will follow the BMPs you have selected.
Signature Date This form is to be completed and kept on record according to the requirements found in the Dugway Proving Ground (DPG) Fugitive Dust Control Plan (FDCP). Copies of form(s) and corresponding logbook entries (if applicable) shall be provided to EP, Air Program Manager, upon the completion of operation(s) and/or annually at the end of each calendar year whichever occurs first. Submit completed form and applicable logbook entries in hardcopy or electronically, via email to Environmental Program’s (EP) Air Program Manager: boyd.roberts.civ@mail.mil
